In vivo CRISPR screens identify dual functions of DNMT3A in mediating tumor immune evasion.

Cancer immunotherapy has achieved remarkable clinical efficacy across multiple cancers but remains ineffective for most patients, and the mechanisms underlying resistance remain incompletely understood. Here, we perform in vivo CRISPR-Cas9 screens in female mice and identify DNA methyltransferase 3 A (DNMT3A) as a key driver of tumor immune evasion through both methyltransferase-dependent and -independent mechanisms. DNMT3A represses TNFRSF1A and TNFRSF1B expression, enabling tumor cells to evade TNFα-induced killing. Independently of its methyltransferase activity, DNMT3A interacts with NFYA

Hyperactive Delirium Temporally Associated With Hydroxyzine in a Child With Succinic Semialdehyde Dehydrogenase Deficiency Presenting With Psychotic Symptoms: A Case Report.

BACKGROUND: Succinic semialdehyde dehydrogenase deficiency (SSADHD) is a rare autosomal recessive disorder of gamma-aminobutyric acid (GABA) metabolism caused by biallelic variants in ALDH5A1. The phenotype includes developmental delay, intellectual disability, seizures, sleep disturbance, anxiety, and other neurobehavioral symptoms. Psychotic symptoms are increasingly recognized in SSADHD but remain uncommon and incompletely characterized in children. In medically and neurodevelopmentally vulnerable patients, acute psychiatric decompensation may also be complicated by delirium, especially whe

Tirzepatide-induced pneumatosis intestinalis: a rare but clinically significant gastrointestinal adverse effect.

OBJECTIVES: Glucagon-like peptide 1 receptor agonists such as tirzepatide are increasingly prescribed to treat type 2 diabetes mellitus and obesity because they improve glycemic control and promote weight loss. Although gastrointestinal adverse effects such as nausea, vomiting, constipation, and delayed gastric emptying are well recognized, very rare cases of pneumatosis intestinalis have also been reported in association with this class of medications. We report the first documented histologic diagnosis of a possible tirzepatide-associated pneumatosis intestinalis. METHODS: A 55-year-old man

The Relationship between Cardiovascular Function and Decreased Intra-Abdominal Pressure Following Stool Evacuation.

Abdominal organs, though less than one-tenth of body mass, receive about 30% of cardiac output, with two-thirds passing through the descending aorta. The abdominal cavity contents-including organs, adipose tissue, and stool-affect peripheral vascular resistance. This study evaluated the impact of stool evacuation exceeding routine daily volume on intra-abdominal vascular resistance and systemic blood pressure. We retrospectively reviewed records of 56 patients who underwent tap water enemas for bloating and constipation. Episodes with >400 g body weight reduction after evacuation were analyzed

Delayed Multi-Compartment Traumatic Hernias After Blunt Abdominal Trauma: A Case-Based Review of Diagnosis and Staged Management.

BackgroundTraumatic diaphragmatic hernias (TDHs) and traumatic abdominal wall hernias (TAWHs) are uncommon sequelae of blunt abdominal trauma (BAT) and may be missed during the initial evaluation. Delayed presentation can occur months to years later, resulting in incarceration, obstruction, or respiratory compromise.Case PresentationA 61-year-old woman presented 1 year after a motor vehicle collision with abdominal pain, nausea, constipation, and dyspnea. Computed tomography demonstrated an incarcerated left diaphragmatic hernia containing colon and omentum, along with a right flank hernia con

Neurogenic colonic motility patterns and gut transit disrupted in a mouse model of Rett Syndrome.

Gastrointestinal dysfunction is a prominent feature of neurodevelopmental disorders and contributes significantly to visceral discomfort, but the specific etiology is unclear. In particular, it is unknown which cell types within central, autonomic or enteric nervous systems, gut muscle, epithelium or immune system are the principal drivers of gut dysfunction. We report that the Mecp2 mouse model of Rett syndrome, a neurodevelopmental disorder with profound gut dysmotility, exhibits impaired whole gut transit times and constipation in vivo as well as striking alterations to propagating neurogen

The link between inflammation and autonomic dysfunction in alpha-synucleinopathies - narrative review.

INTRODUCTION: Alpha-synucleinopathies are a group of disorders characterized by the accumulation of the pathological form of alpha-synuclein. This group comprises Parkinson's disease, Parkinson's disease dementia, multiple system atrophy, dementia with Lewy bodies, pure autonomic failure, and idiopathic REM sleep behavior disorder. Despite sharing a similar molecular basis, these disorders differ in their clinical presentation. Characteristic manifestations include signs of autonomic failure such as orthostatic hypotension, postural orthostatic tachycardia syndrome, constipation, gastroparesis

Comparison of cardiometabolic benefits and tolerability in patients on sustained low doses of semaglutide or tirzepatide.

Semaglutide and tirzepatide are typically evaluated through dose escalation, yet many patients in routine care do not reach a recommended maintenance dose due to tolerability, access, cost, supply, or individualized goals. Here, we used repeated 0.25 mg only semaglutide and 2.5 mg only tirzepatide prescription records as real-world proxies for sustained treatment at the lowest approved dose, a prescribing pattern that may overlap with, but does not establish, intentional microdosing. We analyzed de-identified electronic health records (EHRs) from a federated system of 29 million patients. Amon

Oral 5-HT3 receptor antagonist for at-home management of acute FPIES episodes in adults: A real-world observational study.

BACKGROUND: Food protein-induced enterocolitis syndrome (FPIES) is increasingly recognized in adults. Because adults encounter diverse food environments, ingestion of trigger foods may occur despite dietary avoidance. However, the real-world occurrence of post-diagnosis FPIES episodes and strategies for at-home management remain poorly described. We aimed to characterize post-diagnosis FPIES episodes in adults and evaluate the real-world use of orally administered granisetron as a patient-initiated rescue treatment. METHODS: We conducted a retrospective observational study of adults diagnosed

H-type anorectovaginal fistula with anterior ectopic anus in a 15-month-old girl: a case report.

INTRODUCTION AND IMPORTANCE: H-type anorectovaginal fistula is a rare anorectal malformation characterized by an abnormal communication between the rectum and vagina despite a normally formed or near-normal anus. Diagnosis may be delayed due to the absence of obvious external anatomical abnormalities, making clinical recognition essential, particularly in resource-limited settings. CASE PRESENTATION: We report a 15-month-old girl presenting with fecal vaginal discharge and chronic constipation since early infancy. Clinical examination revealed an anterior ectopic stenotic anus with vaginal fe

Population Differences in the Epidemiology, Phenotype, and Genetics of Hirschsprung Disease in the United States.

IMPORTANCE: Understanding population differences in epidemiology, clinical presentation, and genetic architecture remains a major challenge for all rare genetic disorders. Hirschsprung disease (HSCR), despite being the commonest cause of neonatal intestinal obstruction, has been poorly studied with respect to its significant heterogeneity across U.S. populations. OBJECTIVE: To characterize self-identified race and ethnicity differences in HSCR incidence, clinical presentation, and genetic architecture in the United States from diverse data sources. DESIGN SETTING AND PARTICIPANTS: We used re

Single-Stage Versus Staged Repair of Rectovestibular Fistula in Girls: A Systematic Review and Meta-analysis.

PURPOSE: The optimal repair strategy for rectovestibular fistula remains uncertain. We compared single-stage with staged repair and estimated complication and functional-outcome rates across operative techniques. METHODS: PubMed/MEDLINE, Scopus, and Web of Science were searched from inception through May 20, 2026. Studies of girls with congenital anorectal-malformation-associated rectovestibular fistula undergoing definitive repair were eligible. Random-effects models pooled logit-transformed single-arm proportions and risk ratios (RRs) for direct comparisons. Risk of bias was assessed with d

Daily consumption of sulfate- and magnesium-rich mineral water enhances bowel function in adults with chronic constipation: a randomized double-blind real-world trial.

INTRODUCTION: Functional constipation is one of the most common disorders of gut-brain interaction. Adequate hydration is recommended as first-line therapy, yet evidence regarding the efficacy of sulfate- and magnesium-rich mineral water remains limited. Therefore, this study investigated its efficacy and safety in a nationwide decentralized randomized controlled trial. METHODS: In this randomized, double-blind, controlled parallel-group trial, adults with chronic constipation according to the Rome IV criteria consumed either 1 L/day of sulfate- and magnesium-rich mineral water (HSM) or a low

Avoidant/restrictive food intake across the gastrointestinal tract.

BACKGROUND: Avoidant/restrictive food intake disorder (ARFID) is characterized by reduced dietary intake leading to medical and/or psychosocial impairment, independent of body image concerns. We aim to assess ARFID screening and characteristics in patients with gastrointestinal disorders versus healthy volunteers (HV), and its associations with symptom severity, extraintestinal symptoms and depression scores. METHODS: We analyzed 92 HV and 467 patients from seven tertiary cohorts: irritable bowel syndrome (IBS, n=88), functional dyspepsia (FD, n=83), constipation (n=59), reflux (n=107), globu

Heat-inactivated probiotic-fermented tamarind pulp alleviates constipation by modulating gut tryptophan catabolism and the AhR NLRP3 axis.

INTRODUCTION: Constipation is a common gastrointestinal disorder with gut dysbiosis, and existing laxatives often have limitations, necessitating the development of safer and more effective nutritional interventions. OBJECTIVE: This study aimed to investigate the alleviating effects and potential mechanisms of probiotic-fermented tamarind pulp (FTP) on loperamide-induced constipation in mice. METHODS: A constipated mouse model was established and treated with low and high-dose FTP (LD‑FTP and HD‑FTP). Assessments included gastrointestinal transit, histopathology, serum neurotransmitters/infl

The Gut-Brain Axis in Parkinson's Disease: From Clinical Observation toward Mechanism-Based Precision Medicine.

Gastrointestinal dysfunction is increasingly recognized as a core feature of Parkinson's disease (PD), affecting quality of life, nutrition, medication response, and potentially the earliest stages of pathogenesis. This perspective traces the evolution of the gut from a neglected source of non-motor symptoms to a central component of biological models of PD. Gastrointestinal manifestations are physiologically heterogeneous and require objective phenotyping to distinguish dysphagia, gastroparesis, slow transit, defecatory dysfunction, and overlapping mechanisms. The body-first/brain-first frame

Case Report: Immune checkpoint inhibitor-induced gastrointestinal toxicity: the diagnostic challenge of sequential ileus and colitis.

INTRODUCTION: Immune checkpoint inhibitors can cause diverse immune-related adverse events, the early recognition of which is critical. Intestinal pseudo-obstruction caused by immune-mediated gastrointestinal dysautonomia is a rare, potentially fatal, and under-recognized toxicity that may be difficult to distinguish from more common gastrointestinal complications. METHODS: We describe the clinical course, investigation, and management of a patient with metastatic esophageal mucosal melanoma who developed sequential gastrointestinal toxicities following combination ipilimumab and nivolumab th

One-year outcomes for patients undergoing antegrade continence enema procedures without prior rectal enema therapy: a Pediatric Colorectal and Pelvic Learning Consortium study.

PURPOSE: Antegrade continence enemas (ACE) delivered through surgically placed channels help achieve social continence in children with colorectal disorders. Historically, a successful rectal enema program was required before ACE creation, but high-volume centers increasingly perform ACE upfront, particularly for patients intolerant of rectal enemas. This study describes a cohort of patients who underwent upfront ACE for bowel management within a multicenter consortium of high-volume pediatric colorectal centers, with 30-day and one-year outcomes. METHODS: We retrospectively evaluated patient
