Gut-brain immune dysregulation in Parkinson's disease: a hypothesis-forming narrative review of trained immunity.

Parkinson's disease (PD) is increasingly understood as a multisystem disorder in which gastrointestinal dysfunction, immune remodeling, protein aggregation, and central neurodegeneration interact across the disease course. Although gut microbiota alterations are repeatedly reported in PD, taxonomic differences alone do not explain how peripheral ecological signals acquire neurological relevance. This review therefore focuses on immune translation: the processes through which dysbiosis, microbial metabolites, intestinal barrier stress, and mucosal inflammation may reshape peripheral immunity an

Patient Knowledge of GLP-1 Receptor Agonists for Obesity and Exploration of Potential Household-Level Effects in an Urban Health System.

BACKGROUND: The use of glucagon-like peptide-1 receptor agonists (GLP-1 RAs) for obesity management has gained widespread popularity and media attention. However, little is known about patients' knowledge related to GLP-1 RAs (e.g., prescribing indications, side effects), how patients currently learn and wish to learn about GLP-1 RAs, and how GLP-1 RA use may impact weight-related behaviors and attitudes within households (e.g., food purchasing patterns). OBJECTIVE: This study evaluated these questions among diverse adults recruited from a large, urban health system. METHODS: Participants we

Extracting Symptoms Relevant to Geriatric Syndromes from Japanese Pharmaceutical Care Records: A Natural Language Processing Analysis.

Geriatric syndromes are multifactorial conditions associated with adverse outcomes in older adults, but how community pharmacists capture related symptoms remains unclear. Therefore, a natural language processing (NLP) model was used to analyze pharmaceutical care records. The records of 90651 patients (≥65 y of age) who had prescriptions filled at 58 community pharmacy branches in Japan between January and December 2022 were analyzed. Patients without the home-based patient dispensing fee (hereafter referred to as outpatients) and patients receiving home-based care were included in the study.

Development and internal validation of a prediction model for suboptimal bowel preparation in individuals aged ≤50 years undergoing colonoscopy.

BACKGROUND: The incidence of early-onset colorectal cancer (EOCRC) has increased in recent decades, leading to more frequent colonoscopy use in younger adults. Adequate bowel preparation is essential for colonoscopy quality, but predictors of suboptimal bowel preparation and individualized risk assessment tools remain insufficiently defined in this population. METHODS: We conducted a retrospective cohort study of 2,283 consecutive individuals aged ≤50 years who underwent complete colonoscopy between June 2017 and December 2025. Bowel preparation quality was assessed using the Boston Bowel Pre

Resistance to Thyroid Hormone Alpha: Clinical Spectrum, Diagnostic Clues, and Therapeutic Strategies - A Narrative Review.

Resistance to thyroid hormone alpha (RTHα) is a rare, inherited disorder of thyroid hormone action caused by pathogenic variants in the thyroid hormone receptor alpha gene. Unlike resistance to thyroid hormone beta, RTHα typically presents with near-normal thyroid function tests and a non-elevated thyroid-stimulating hormone (TSH), contributing to substantial underdiagnosis. The biochemical hallmark is a low or low-normal free thyroxine (FT4) level with a disproportionately high free triiodothyronine (FT3) level, yielding a reduced FT4:FT3 ratio and often a low or low-normal reverse T3. GLP-1 receptor agonists in perimenopause and early postmenopause: clinical role, cardiometabolic benefits and knowledge gaps.

Perimenopause and early postmenopause represent a critical metabolic transition with adverse changes in body composition, insulin sensitivity, lipid metabolism and cardiometabolic health, contributing to increased rates of obesity, type 2 diabetes and cardiovascular disease. As the prevalence of obesity continues to rise among midlife women, effective therapeutic strategies are increasingly needed. Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have emerged as highly effective treatments for obesity and type 2 diabetes through their effects on appetite regulation, weight reduction, gluc

The Impact of Dietary Fiber Supplementation on Gut-Brain Axis Biomarkers and Symptoms in Functional Constipation: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND AIMS: Dietary fiber is first-line for functional constipation (FC), but its mechanism remains unclear. Emerging evidence suggests gut-brain axis modulation. We conducted a meta-analysis of randomized controlled trials (RCTs) to evaluate fiber's clinical efficacy and its effects on gut-brain axis biomarkers in FC. METHODS: We searched PubMed, Embase, Web of Science, and Cochrane CENTRAL from inception to January 2026 for RCTs assessing fiber supplementation in Rome-diagnosed FC patients. Primary outcomes: spontaneous bowel movements (SBMs), complete SBMs (CSBMs), stool consistency.

Targeted in vitro suppression of Parkinson's disease-associated Desulfovibrio by selective probiotic and potential probiotic strains.

INRODUCTION: Emerging evidence links fecal Desulfovibrio (DSV) abundance with Parkinson's disease (PD) severity. While probiotics may relieve symptoms, their direct effects on DSV remain unexplored. METHODS: This study evaluated the inhibitory potential of 32 probiotic and potential probiotic strains against three DSV strains isolated from three individual PD patients. Antimicrobial activity was quantified using agar well diffusion and microplate assays, and the operational minimum inhibitory con centration (MIC₉₀) was determined based on the total crude dry matter content of cell-free supern

Decades Later: Recurrent Gastrointestinal Sequelae in a Patient With Remote Metastatic Osteosarcoma to the Stomach and Colon.

Osteosarcoma is a primary malignant bone tumor with a well-established tendency for pulmonary metastasis; however, gastrointestinal (GI) involvement is exceedingly rare. Intra-abdominal metastases may occur years to decades after the initial diagnosis. Hematogenous spread, following pulmonary metastasis, is the presumed mechanism. Due to delayed presentation and atypical manifestations, recognition of GI complications in patients with a remote history of osteosarcoma is essential. In this report, we present a case of an 80-year-old male with chronic GI complications decades after original meta

Netupitant-palonosetron combined with dexamethasone for preventing chemotherapy-induced nausea and vomiting in high-risk patients with colorectal cancer receiving moderately emetogenic chemotherapy: A retrospective study.

Although netupitant-palonosetron (NEPA) is an established option for preventing chemotherapy-induced nausea and vomiting (CINV) in patients receiving highly emetogenic chemotherapy, its efficacy in patients with high-risk features undergoing moderately emetogenic chemotherapy (MEC) remains insufficiently investigated. Therefore, the present study evaluated the efficacy of NEPA combined with dexamethasone for preventing CINV in high-risk patients with colorectal cancer (CRC) receiving MEC. This retrospective analysis included 182 patients with CRC receiving MEC who had at least one high-risk fa

Focal Hypoganglionosis-Associated Adult-Onset Megacolon: A Localized Enteric Neuropathy Distinct From Chronic Intestinal Pseudo-Obstruction.

Adult-onset megacolon is a heterogeneous disorder traditionally classified within the spectrum of idiopathic megacolon or chronic intestinal pseudo-obstruction (CIPO). However, accumulating evidence suggests that a subset of patients demonstrates distinct clinicopathologic features characterized by localized motor dysfunction resulting in focal colonic narrowing, proximal megacolon, and localized hypoganglionosis. In this review, we refer to this phenotype as focal hypoganglionosis-associated adult-onset megacolon (FHAM). Clinically, patients with FHAM frequently present with severe chronic co
