Atraumatic Spontaneous Splenic Rupture Complicating Decompensated Chronic Liver Disease With Portal Hypertension: A Case Report.

Atraumatic spontaneous splenic rupture (ASR) is a rare, potentially fatal condition accounting for fewer than 0.5% of all splenic ruptures, with reported mortality of up to 20% when diagnosis is delayed. We report a 63-year-old male with decompensated chronic liver disease and portal hypertension who presented with acute epigastric pain, progressive abdominal distension, and absolute constipation without antecedent trauma. Abdominal ultrasonography identified splenomegaly with a 5 × 6 cm anechoic intraparenchymal collection with subcapsular and perisplenic extension and moderate hemoperitoneum

Investigating the effect of buprenorphine on pain in the acute phase of patients with mild traumatic brain injury: a double-blind clinical trial.

INTRODUCTION: TBI is one of the main causes of neurological, behavioral, and physiological disorders. The aim of this pilot research was to assess the efficacy and safety of using buprenorphine in treatment of mild TBI complications. METHODS: This double-blind, randomized clinical trial included 36 mild TBI patients aged 18-65 years with mild TBI. Patients received either Buprenorphine (8 mg oral tablet) (18 cases), alongside standard TBI care (18 cases). Main outcome were pain intensity using the Visual Analogue Scale (VAS), cognition using the Working Memory Questionnaire, Glasgow Outcome S

Factors Affecting the Clinical Response to Desmopressin Treatment for Primary Monosymptomatic Nocturnal Enuresis in Children.

INTRODUCTION: Desmopressin is the first-line treatment option for primary monosymptomatic nocturnal enuresis (MNE), particularly when nocturnal polyuria is a key symptom. This study aimed to investigate the factors affecting clinical response to desmopressin and relapse status in children with primary MNE during follow-up. METHODS: Files of 576 paediatric patients presenting with primary MNE between September 2022 and March 2025, who commenced desmopressin treatment, were analysed retrospectively. The patients' demographic and clinical data were recorded. The clinical response to treatment wa

[Stress urinary incontinence in women].

Stress urinary incontinence (SUI) is an involuntary loss of urine triggered by physical activities, coughing, or changes in body position. It affects 14% of women in France and is associated with risk factors including obesity, pregnancy, traumatic deliveries, hormonal changes, age, intense physical activity, pelvic surgeries, chronic cough and constipation, The impact on patients is significant, affecting their quality of life and leading to high healthcare costs.Diagnosis relies on questionnaires and a detailed clinical examination to distinguish between types of incontinence. The initial tr

Increased Risk of Stroke in Constipation Patients: Systematic Review and Meta-analysis.

OBJECTIVE: Constipation is commonly seen among patients with cardiovascular diseases and is linked to adverse outcomes. However, the association between constipation and the risk of stroke remains conflicting. Therefore, we aim to conduct a systematic review and meta-analysis to summarize the available data on this topic. METHODS: We identified potentially eligible studies from the MEDLINE and EMBASE databases, searching from inception to May 2024, to investigate the association between constipation and stroke. To be included, studies needed to compare the incidence of stroke between groups w

Novel insights into carbohydrate-to-fiber ratio and constipation: NHANES findings suggest a turning point.

BACKGROUND: Constipation, a widespread global issue, prompted an investigation into the intricate relationship between dietary factors and this condition. This study delves into the association between carbohydrate-to-fiber (CF) ratio and constipation, employing data from the National Health and Nutrition Examination Survey (NHANES) and a comprehensive meta-analysis. METHODS: Utilizing NHANES data (2005-2010) from 7752 adults, constipation was defined based on stool type (Bristol Stool Form Scale types 1 or 2). The CF ratio was calculated, revealing an L-shaped, non-linear association with co

Association Between Constipation and Fecal Incontinence in Community-Dwelling Older Adults in Japan.

OBJECTIVES: With a close relation between constipation and fecal incontinence, constipation is thought to be a risk factor for fecal incontinence in older adults. However, the limited number of studies available have provided inconsistent results. We conducted a large-sample population-based study to evaluate the association between constipation and fecal incontinence. DESIGN: Cross-sectional community-based survey. SETTING AND PARTICIPANTS: Independent community-dwelling older adults in Japan ≥75 years of age in 2023 who were not using laxatives, enemas, or undergoing fecal disimpaction. M

Constipation and Psychiatric Disorders: A Bidirectional Mendelian Randomization Study.

BACKGROUND: Observational studies have shown a link between constipation (CN) and psychiatric disorders, including Schizophrenia (SP), Bipolar disorder (BD), Schizoaffective disorder (SD), and Parkinson's disease (PD). However, it is still unknown whether CN affects the occurrence and development of psychiatric disorders or whether psychiatric disorders cause the occurrence and development of CN. Therefore, this study used Mendelian randomization (MR) analysis to evaluate the relationship between CN and psychiatric disorders. METHOD: We used genome-wide association studies (GWAS) to assess th

Factors associated with constipation in infants from six to 12 months of age: a case-control study.

Brain-gut axis disorders (BIGDs) are expressed as a combination of recurrent gastrointestinal symptoms without the presence of anatomical abnormalities, inflammation, or tissue damage. From 6-12 months of age there is a change in the defecatory pattern, which is characterized by a gradual decrease in the frequency of bowel movements and an increase in the consistency of the stool. Factors associated with functional constipation in this age group have not been well identified.

Efficacy and safety of the muscarinic receptor agonist KarXT (Xanomeline-Trospium) in schizophrenia: a systematic review, meta-analysis and Bayesian sensitivity analysis.

BACKGROUND: Schizophrenia significantly impacts global health, with existing treatments primarily focusing on positive symptoms and often causing considerable side effects. KarXT (Xanomeline-Trospium), a novel treatment combining xanomeline, a muscarinic M1/M4 receptor agonist, and trospium, targets a broader range of symptoms including negative and cognitive deficits, potentially with fewer side effects. This systematic review and meta-analysis evaluates the efficacy and safety of KarXT in treating schizophrenia, assessing symptom reduction and safety profiles compared to placebo. METHODS: W

Impact of Fermented Dairy on Gastrointestinal Health and Associated Biomarkers.

In this narrative review, we examined observational and randomized controlled trials investigating the effects of fermented dairy foods, including yogurt, fermented milk, kefir, and cheese, on gastrointestinal (GI) symptoms and/or GI biomarkers of health. Studies that recorded GI symptoms such as intestinal discomfort, flatulence, gastroenteritis, diarrhea, and constipation were included. GI health biomarkers encompassed measurements of intestinal integrity or permeability (eg, endotoxemia, zonulin, 2-arachidonoylglycerol), immune responses (eg, TNFα, IL-6, high-sensitivity CRP [hs-CRP], IFNγ,

Effect of Six Months of Gallbladder Flush on Size and Symptoms of A Patient With Cholelithiasis: A Case Report.

BACKGROUND: Cholelithiasis is a chronic recurrent hepatobiliary disease caused by impaired metabolism of cholesterol, bilirubin, and bile acids. A gallbladder flush (GBF) involves consuming olive oil, lemon juice, and apple cider vinegar, along with dietary restrictions. Previous studies have shown that GBF helps dissolve stones; however, it is provided only for a short duration and does not incorporate dietary modifications. Hence, this case report aims to evaluate the effect of repeated GBF sessions combined with an apple-based diet on a patient with cholelithiasis. METHODS: A 59-year-old m

Relative fat mass and constipation in United States adults ‒ A cross-sectional study of 11,380 participants from NHANES 2005-2010.

BACKGROUND: constipation is a common gastrointestinal disorder closely associated with obesity. Relative fat mass (RFM) is a newer anthropometric index that offers a more precise reflection of body fat distribution than traditional methods. Despite its advantages, the potential link between RFM and the likelihood of experiencing constipation has not been thoroughly examined. This study was therefore designed to explore the association between RFM and constipation Methods: data were obtained from the 2005-2010 cycles of the National Health and Nutrition Examination Survey (NHANES). Thirty years of paediatric colonic motility assessment modalities: a systematic review.

INTRODUCTION: Disorders of paediatric colonic motility may arise following operative repair of congenital colorectal conditions, as well as spontaneously. This may lead to symptoms of faecal incontinence, in addition to constipation. Management of such typically requires several investigations, clinic appointments and treatment strategies. Different investigation modalities are available to objectively and accurately describe gastrointestinal transit time. In the paediatric setting, most of these investigations are invasive, involve radiation, or require patient hospital admission. This system

Gut-Brain Axis in Comorbid Constipation and Mood Disorders: Navigating the Efficacy-Tolerability Dilemma Through Emerging Evidence and Future Directions.

BACKGROUND: The comorbidity of constipation and mood disorders (anxiety/depression) creates a vicious cycle of symptom superposition and treatment resistance. The persistent "efficacy-tolerability" dilemma-whereby effective treatment for one condition may exacerbate the other-remains unresolved. METHODS: This narrative review is based on a systematic search of PubMed, Embase, and Cochrane Library (2015-2026), focusing on epidemiology, gut-brain axis mechanisms, pharmacotherapy limitations, and emerging strategies. The search formula was: ("constipation" OR "functional constipation") AND ("moo
