Extent of Vagus Innervation to the Pelvic Organs: A Scoping Review.

CONTEXT: The vagus nerve (VN) has diverse impacts on the gastrointestinal tract, but there is conflicting and limited information on any pelvic organ innervation. Spinal cord neurological disease affects pelvic organ function, and the possibility of extraspinal innervation has implications for function and potential therapy development. OBJECTIVE: We aimed to explore current literature evidence on VN in relation to the pelvic viscera, to map the distal VN, anatomically and functionally. EVIDENCE ACQUISITION: The Preferred Reporting Items for Systematic Reviews and Meta-analyses extension for

Screening colonoscopy utilization among adults living with spina bifida.

PurposeSpina bifida (SB) is the most common neural tube defect, often accompanied by neurogenic bladder and bowel dysfunction, hydrocephalus, and orthopedic sequelae. As life expectancy increases for individuals with SB, understanding disparities in healthcare utilization becomes crucial. This study examined colorectal cancer (CRC) screening colonoscopy utilization rates among adults living with SB.MethodUsing the California Health Care Access and Information database (2005-2017), a matched control group was established to compare colonoscopy utilization. The dual role of capsaicin in inflammatory bowel disease: from TRPV1-mediated protection to neurogenic inflammation.

Capsaicin, the pungent vanilloid of chili pepper, is a widely consumed dietary constituent that exerts pleiotropic actions on physiological and pathophysiological processes, including anti-inflammatory, antioxidant, antiproliferative, metabolic and cardioprotective effects. Capsaicin is a small, highly lipophilic molecule that is not appreciably hydrolyzed by digestive enzymes in the gut lumen; it is absorbed across the gastrointestinal epithelium largely in its intact form, mainly by passive diffusion, and enters the portal circulation, where it undergoes extensive first-pass metabolism in th

[Lumbar Spinal Disease or Peripheral Nerve Disorder? The Role of Neurological Examination in Differential Diagnosis].

Lower-limb numbness, pain, and weakness are common clinical complaints that require differentiation between cauda equina or radicular lesions associated with lumbar spinal disease and peripheral nerve disorders. Because degenerative findings on lumbar imaging are common, particularly among older adults, the responsible lesion should not be determined based on imaging findings alone. In this context, neurological examination contributes to anatomical localization and is central to the differential diagnosis. Particular emphasis is placed on interpreting muscle weakness and sensory disturbance a

Conus Medullaris Syndrome Following Traumatic Thoracolumbar Burst Fractures: Clinical Presentation, Surgical Management, and Early Outcomes in Three Cases.

Burst fractures of the thoracolumbar spine can carry a neurological deficit, particularly where the conus medullaris is injured. Conus medullaris syndrome (CMS) typically disturbs bladder, bowel, and sexual function while sparing much of the leg power, and its earliest signs are easily missed. We report three patients in whom a traumatic thoracolumbar burst fracture produced features of CMS and describe their operative management. A 57-year-old man with an L1 burst fracture and intact leg power presented with bowel and bladder incontinence and underwent posterior stabilization (D11-L3), regain

Substance P receptor blockade is associated with increased survival of patients with C. difficile infection.

Clostridioides difficile is a major cause of healthcare associated infections. C. difficile infection (CDI) is a toxin-mediated disease driven by the secretion of two large protein toxins, Toxin A (TcdA) and Toxin B (TcdB). TcdA and TcdB cause neurogenic inflammation that can contribute to intestinal inflammation in rat and mouse models. Substance P (SP), a potent vasoactive neuropeptide, is a major contributor to C. difficile toxin induced inflammation in animal models. SP acts on multiple receptors including the high affinity receptor NK1-R, and lower affinity receptors ( Mrgprb2 and NK2-R)

Language-supervised electrocardiogram interpretation with vision transformer.

PURPOSE: Most artificial intelligence models for electrocardiogram (ECG) interpretation rely on task-specific supervised learning, requiring dedicated labeled datasets for each diagnostic task and limiting scalability across diverse cardiac conditions. This study investigated whether vision-language learning from paired ECG recordings and summary reports could enable transferable ECG representations without task-specific supervised training. METHODS: We developed ECG-VLM, a contrastive vision-language model consisting of a Transformer-based image encoder and a text encoder, trained on 800,034

Assessing bladder, bowel and sexual symptoms: a neurologist's guide.

Symptoms related to bladder, bowel and sexual dysfunction are common in patients with neurological diseases and are associated with significant morbidity and impacts on quality of life. However, compared with other neurological problems, they are often less visible, under-ascertained and under-treated. This review aims to provide neurologists with an evidence-based, structured approach to evaluating bladder, bowel and sexual dysfunction in patients with neurological disorders. As with any other presenting concern, this requires a structured approach to history, examination and the judicious us

Transient receptor potential channels as emerging therapeutic targets: mechanisms and therapeutic insights in inflammatory bowel disease.

Transient Receptor Potential channels (TRPs) are a family of non-selective cation channels widely distributed across diverse tissues and cells in mammals. They participate in numerous physiological processes, including environmental stimulus perception, signal transduction, neurotransmitter release, and insulin secretion. The involvement of TRPs in pain syndromes, neurodegenerative diseases, metabolic disorders, and tumorigenesis has established them as prominent targets in translational medicine. Inflammatory Bowel Disease (IBD), encompassing Crohn's disease (CD) and ulcerative colitis (UC),

Advanced neurosurgical intervention of recurrent tethered cord syndrome with split cord malformation type 1: an illustrative case.

PURPOSE: Recurrent tethered cord syndrome (TCS) with split cord malformation (SCM) is a rare presentation with distinct surgical challenges to successful patient outcomes. SCMs are rare and can progress to cause significant neurological deficits including pain, weakness, sensory disturbance, bowel and/or bladder dysfunction. In these circumstances, surgical management of TSC with SCM is indicated. Reoperation for recurrent TCS, particularly in the context of retained or regrown bony septa and prior spinal instrumentation, is technically demanding and carries elevated risk. ILLUSTRATIVE CASE:

Rare Case of Anterior Sacral Meningocele in a 30-Year-Old Male: Surgical Excision Using the Posterior Sagittal Approach.

Anterior sacral meningocele (ASM) is an uncommon congenital spinal abnormality in which the meningeal sac herniates through an abnormality in the anterior sacrum into the presacral space. It is more observed in females and usually asymptomatic; large ASMs may cause pressure effects on nearby pelvic structures. A 30-year-old male presented with prolonged constipation and recurrent episodes of urinary retention, which developed to complete urinary obstruction. Ultrasound demonstrated a large presacral cystic mass, and MRI confirmed the diagnosis of an ASM. Pelvic x-ray revealed the classic Scimi

Effect of 600-Hz Sacral Root Stimulation on Lower Urinary Tract and Bowel Function in Three Individuals With Spinal Cord Injury: A Case Series.

Spinal cord injury typically leads to neurogenic bladder dysfunction, often including detrusor-sphincter dyssynergia. Sacral anterior root stimulation can empty the neurogenic bladder, but this emptying can be impeded by reflex contractions of the urethral sphincter. The sacral sensory roots are typically transected (rhizotomy) to reduce these reflex contractions, but this rhizotomy also impairs desirable reflexes (e.g., sexual function) and sacral sensation, if present. Preclinical experiments have shown that sacral nerve stimulation at 600 Hz can promote bladder emptying while reducing ureth
