Long-term outcomes of antegrade continence enema in adult myelomeningocele patients.

INTRODUCTION: There is limited data on long-term outcomes of antegrade continence enema (ACE) procedures in the treatment of chronic constipation and fecal incontinence in patients with myelomeningocele. These procedures are often performed by urologists for pediatric populations, with limited data on long-term follow-up regarding antegrade continence enema usage and outcomes as patients progress through adulthood. This study provides new insight regarding long term ACE usage and impact on life for a large cohort of adult myelomeningocele patients. METHODS: We performed a retrospective chart

Variability in Health Care Utilization and Perioperative Outcomes Among Urinary Diversion Patients: Analysis of the National Surgical Quality Improvement Program Database.

INTRODUCTION: Bladder cancer (BC) urinary diversion (UD) outcomes remain the benchmark to which all other UDs are compared. However, a sizable proportion are performed for other invasive cancers or benign etiologies of the bladder. We aim to compare health care utilization (HU) and 30-day morbidity among benign and malignant UD etiologies. METHODS: The American College of Surgeons National Surgical Quality Improvement Program was used to collect data on 20,885 patients who underwent UD for various indications: BC, gastrointestinal/gynecologic cancer (GC), radiation (R), interstitial cystitis/

Sacral Neurostimulation in Neurogenic Lower Urinary Tract and Bowel Dysfunction Caused by Multiple System Atrophy.

OBJECTIVE: Multiple system atrophy (MSA) is a progressive neurodegenerative disorder categorized as an atypical Parkinsonian syndrome, affecting extrapyramidal, pyramidal, cerebellar, and autonomic systems. Lower urinary tract dysfunction (LUTD), including urgency, frequency, urge incontinence, and incomplete voiding, is a prevalent symptom, often appearing early in the disease. Conservative treatments, such as anticholinergic and alpha-blocker therapies, frequently fail due to poor tolerance and limited efficacy. Many patients also experience bowel symptoms, including constipation and fecal i

Application and outcomes of a standardized, evidence-based institutional approach to simple tethered cord management.

OBJECTIVE: There is significant evidence in the literature and decades of practice experience that support filum terminale sectioning as an effective intervention to improve the symptoms of simple tethered cord (sTC). The diagnosis of sTC and indications for sTC surgery vary widely across the US and Canada, although the overall incidence of sTC surgery has been increasing across the US. This study sought to create and evaluate an evidence-based algorithm for the management of sTC. METHODS: An institutional standard for sTC management was established, based on an evidence-based medicine analys

Spina Bifida.

Spina bifida (SB) is the most common neural tube defect. Myelomeningocele is the most common and severe form of SB, leading to a variety of sequelae especially with complications in the central nervous, lower urinary, gastrointestinal, and musculoskeletal systems. The care of children with SB is not only medically complex but requires coordination across various medical specialties and disciplines. This article provides a broad overview of different aspects of management for children with SB.

Effect of Sacral Neuromodulation for Neurogenic Bladder and Bowel Dysfunction in Spinal Dysraphism: A Prospective Cohort Study.

BACKGROUND AND OBJECTIVES: Although plenty of evidence supports the effectiveness of sacral neuromodulation (SNM) in improving urination and defecation, few studies concerned its effect on the patients with spinal dysraphism (SD). This study aimed to evaluate the effects of SNM on SD-induced neurogenic bladder and bowel dysfunction. METHODS: We prospectively followed the patients with SD who underwent SNM in our department from May 2019 to June 2024. Fusion images of sacrococcygeal computed tomography and magnetic resonance sacral plexus nerve images were used as essential references for preo

Utilizing Predictive Analytics to Understand Neurogenic Bladder Symptom Score (NBSS) Variations in Adults With Acquired Spinal Cord Injury.

INTRODUCTION: Individuals with spinal cord injury (SCI) have varying bladder health trajectories after their injury. We explored whether a predictive machine learning model could identify which variables impact urinary symptoms. METHODS: We used 238 variables from the Neurogenic Bladder Research Group SCI registry for a Decision Tree analysis (eCHAID technique). The primary outcomes were the baseline Neurogenic Bladder Symptom Score (NBSS), and the change from the baseline NBSS at 1-year follow up (measured as better/worse than the median change). RESULTS: Among the 1479 participants, mean b

Single Cell Profiling in the Sox10Dom Hirschsprung Mouse Implicates Hox Genes in Enteric Neuron Trajectory Allocation.

BACKGROUND & AIMS: Enteric nervous system (ENS) development requires migration, proliferation, and differentiation of progenitors for normal gastrointestinal (GI) motility. Sox10 deficit causes aganglionosis, modeling Hirschsprung disease (HSCR), and disrupts ratios of postnatal enteric neurons in proximal ganglionated bowel. How Sox10 deficiency alters enteric neuron ratios is unclear. Sox10's prominent expression in enteric neural crest-derived progenitors (ENCPs) and lack of this gene in mature enteric neurons led us to examine Sox10Dom effects in early ENS development. METHODS: Immunohist

Robotic pediatric augmentation cystoplasty: Outcomes over a 15-year experience.

INTRODUCTION: Augmentation cystoplasty (AC) is performed for the management of neurogenic bladder in select patients with upper urinary tract deterioration and urinary incontinence refractory to conservative treatments. Traditionally, open augmentation ileocystoplasty (OAI) has been the gold standard, but growing evidence shows that performing pediatric urologic procedures robotically has been associated with shorter hospitalizations, improved cosmesis and reduced scar visibility, and reduced pain. Our institution performed the first intracorporeal robotic-assisted laparoscopic augmentation il

Early nursing interventions for neurogenic bowel dysfunction in acute-phase spinal cord injury patients: A non-concurrent controlled trial.

BACKGROUND: Traumatic spinal cord injury (SCI) often leads to neurogenic bowel dysfunction (NBD), causing significant gastrointestinal issues that adversely affect patients' quality of life (QoL). The current study evaluates the effectiveness of early nursing interventions in managing NBD among SCI patients. METHODS: A non-concurrent controlled trial design included 125 participants with acute-phase SCI. The control group (n = 56) received standard nursing care, while the intervention group (n = 69) underwent personalized gastrointestinal management. Data were collected via NBD scores and the

A Comparative Study of Paralympic Veterans with Either a Spinal Cord Injury or an Amputation: Implications for Personalized Nutritional Advice.

Background: Dietary advice for Paralympic athletes (PAs) with a spinal cord injury (PAs-SCI) requires particular attention and has been widely studied. However, currently, no particular attention has been addressed to nutritional guidelines for athletes with an amputation (PAs-AMP). This study aimed at filling up this gap, at least partially, and compared veteran PAs-SCI with PAs-AMP. Methods: A sample of 25 male PAs (12 with SCI and 13 with AMP), recruited during two training camps, was submitted to the following questionnaires: allergy questionnaire for athletes (AQUA), Nordic Musculoskeleta

Conus medullaris and cauda equina syndrome as complications of non-imaging guided epidural steroid injection: two case reports with comprehensive interdisciplinary work-up.

INTRODUCTION: Epidural steroid injection (ESI) is commonly performed in the outpatient setting for relieving lumbosacral radicular pain, i.e., sciatica. Neurological and neuro-urological adverse events are rare but devastating when occurring. This led to a warning of the U.S. Food and Drug Administration about the use of corticosteroids in the spine. Reviews on causes and numbers of complications are lacking. CASE PRESENTATION: Two cases from our tertiary spine center who sustained conus medullaris and cauda equina syndrome following ESI in a secondary care setting not affiliated with our ins
