Skip segment Hirschsprung Disease: Unicentric experience with four cases and systematic literature review.

INTRODUCTION: Skip lesion aganglionosis is characterized by intestinal aganglionosis interrupted by portions of normally ganglionated bowel that is located in between segments of aganglionosis. Even though this is a well-known entity, only scattered cases have been reported so far. The aim of this manuscript is to present our series of patients and perform a systematic literature review, to offer a critical overview of possible management. PATIENTS AND METHODS: We hereby present a case series of 4 patients diagnosed during the last five years in our Centre in accordance to SCARE guidelines. A

Mobile health applications for parents of children with Hirschsprung disease: a requirement analysis.

UNLABELLED: Hirschsprung disease (HD) is a congenital gastrointestinal disorder that occurs due to the absence of nerve cells in segments of the large intestine, resulting in impaired propulsive bowel movements. Postoperative complications and the need for continuous care place parents of affected children under significant educational challenges. Mobile health (mHealth) applications can serve as effective tools to enhance parental education and self-management. The aim of this study was to identify the requirements and functional needs of a mHealth application specifically designed for parent

The PODCAST-question: postoperative dilation versus calibration after surgical treatment of Hirschsprung's disease-what is necessary?

PURPOSE: Postoperative anal dilation after surgery for Hirschsprung disease (HD) still remains common practice. This study aimed to evaluate the benefit of routine dilations compared with a single postoperative calibration. METHODS: Patients who underwent HD surgery within the first year of life at the University Children’s Hospital Zurich or the University Hospital Frankfurt were retrospectively reviewed. Two groups were compared: those with serial postoperative dilations (PD) and those with a single calibration (PC). Primary outcomes included anastomotic stricture, Hirschsprung-associated en

Rare Struma Ovarii Variant of Incidental Ovarian Dermoid Cyst in a Cadaveric Donor: A Case Report.

Post-operative adhesion is a common complication of abdominopelvic surgical procedures, including a hysterectomy. However, little is known about post-surgical tissue growths, atypical of fibrous adhesions. This case study evaluates a hysterectomized 80-year-old female anatomical donor who revealed intra-abdominal adhesions and bilateral abnormal tissue adnexal masses upon dissection and seeks to evaluate the causes of these abnormalities and potential consequences. Histologic analysis of the bilateral masses determined atrophied ovarian stromal tissue with a mature teratoma composed of respira

Definitive surgery for Hirschsprung Disease between 3 and 12 months achieves best outcomes: A systematic review with meta-analysis.

INTRODUCTION: Optimal age for definitive pull-through in Hirschsprung disease (HD) is debated. Both early and delayed interventions may influence short- and long-term outcomes, but current guidelines do not provide a clear recommendation. METHODS: A systematic review was conducted following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Nineteen studies published between 1998 and 2025, including 3980 pediatric patients with HD undergoing definitive pull-through surgery, were included. Age at surgery was stratified using predefined thresholds (neonatal,

Dual roles of GDNF in enteric glial cell plasticity: direct transdifferentiation via the CaMKII/NeuroD1 pathway and cooperative regulation in a neural stem cell-inducing medium.

BACKGROUND: Hirschsprung disease (HSCR) is a congenital condition featuring aganglionosis in the distal colon, causing functional obstruction. While EGF and bFGF are well-characterized neurogenic factors, the precise mechanistic role of GDNF in modulating enteric glial cell plasticity remains incompletely understood. METHODS: EGCs were identified via proteomic profiling and immunofluorescence in Ednrb⁻/⁻ mice modeling HSCR. EGC/PK060399egfr and primary EGCs were induced with neural stem cell-inducing medium (NSC-Med). Morphological changes, EdU assay, immunofluorescence, RT‒qPCR, and Western

Systematic structure-based analysis of RET variants in MEN2A and Hirschsprung's disease, and the paradoxical co-occurrence of both conditions.

Variants in the human receptor tyrosine kinase RET can cause RET loss-of-function and Hirschsprung's disease (HSCR), while activating RET variants drive cancers including multiple endocrine neoplasia type 2 (MEN2). Paradoxically, some variants cause both HSCR and MEN2A. We curated 77 RET extracellular positions associated with HSCR, MEN2A or both and used a structure-based approach to predict the effects of variants at these positions on RET structure. Approximately 90% of HSCR-associated positions can, upon mutation, disrupt intramolecular interactions stabilizing RET tertiary structure via d

Toxic Megacolon With a Positive Clostridioides difficile Antigen but Negative Toxin Assay.

The diagnosis of Clostridioides difficile infection (CDI) relies on a combination of clinical presentation and laboratory testing. However, a critical diagnostic discordance occurs when laboratory results contradict a rapidly evolving clinical picture. This is a case of a 78-year-old man with four comorbidities who presented with watery diarrhea and a C. difficile screen-positive but toxin-negative stool sample. His main risk factors included advanced age and the prolonged use of a proton pump inhibitor. Despite his initial negative toxin assay, his condition progressively worsened over three

Development and validation of a computer program for histoanatomical morphometric analysis of the bowel wall in children with Hirschsprung's disease.

BACKGROUND: Developing new diagnostic imaging methods that provide detailed visualization of the histoanatomy of tissues and organs requires precise histomorphometric evaluation. With a particular focus on bowel, this study aimed to develop and validate a computer program for reliable and efficient assessment of bowel wall histomorphometry. METHODS: A MATLAB-based computer program was developed in-house to manually delineate and automatically calculate mean layer thickness of the muscularis propria layers, submucosa and mucosa in histopathology images of bowel wall specimens from patients oper

Research at the Paris Foundling Hospital, Part 2: After the Revolution.

BACKGROUND: We know little about how neonatal research changed at the Paris Foundling Hospital following the revolution. SUMMARY: The number of unwanted children rose, and 5,392 infants were admitted in 1826 - a quarter of all infants born in Paris; 26% of them died in the infirmary before transport. The infants' appalling mortality was associated with artificial nutrition, and transfer to mercenary nurses in the countryside was organized. In the 1830s sedentary nurses began to run short, and nearly all babies were fed artificially at some time. François Chaussier was director at the Maternit

Exosome-transmitted microRNA-323a-3p participated in the occurrence of Hirschsprung's disease.

BACKGROUND: Hirschsprung's disease (HSCR) is caused by defective enteric neural crest cell (ENCC) migration. Exosome-transmitted microRNAs are implicated in HSCR pathogenesis, but mechanisms remain unclear. METHODS: Plasma exosomes and colon tissues were collected from HSCR patients and controls. We assessed the effects of exosomal miR-323a-3p on the proliferation and migration of immortalized ENCC-derived neural cell line (iENC) in vitro using CCK-8, EdU and Transwell assays, and its impact on ENCC migration in vivo using a zebrafish model. RESULTS: Exosomal miR-323a-3p was significantly up

Fulminant Toxic Megacolon Without Identifiable Underlying Cause Requiring Emergent Total Colectomy: A Case Report.

Toxic megacolon is a life-threatening condition characterized by acute colonic dilation and systemic toxicity, most commonly associated with inflammatory or infectious colitis. However, ischemic colitis represents an important and underrecognized etiology in elderly patients with significant vascular comorbidities. We report the case of a 71-year-old male with a history of long-standing hypertension, type 2 diabetes mellitus, and chronic kidney disease, receiving losartan, nifedipine, and insulin therapy, who presented with five days of progressive diffuse abdominal pain, severe distension, ob

Quality of life of Hirschsprung disease patients with a stoma.

INTRODUCTION: Hirschsprung disease (HSCR) is a genetic disorder leading to gastrointestinal obstruction due to the absence of ganglion cells in the submucosal and myenteric plexuses. Treatment typically involves a pull-through surgery, sometimes starting with a colostomy. The COVID-19 pandemic has resulted in restrictions on general patient services, i.e., non-COVID-19 patients, at health facilities, leading to reduced hospital visits, including HSCR patients with stomas. This study aimed to determine whether there were differences in the quality of life (QoL) of HSCR patients with stomas befo

Histochemical characterization of muscularis macrophage loss and disruption of a miR-93-5p-AHNAK neuro-immune axis in Hirschsprung's disease.

Hirschsprung's disease (HSCR) is a congenital disorder of the distal intestine characterized by aganglionosis of the enteric nervous system. While defective migration of neural crest-derived precursors is a well-established hallmark, how the intestinal microenvironment contributes to impaired neuronal support remains poorly understood. Here, we combined histochemical analyses of human HSCR colon with functional assays to investigate the role of muscularis macrophages (MMs) and a miR-93-5p-AHNAK pathway in regulating enteric neurons. Using immunohistochemistry and immunofluorescence on ganglion
