Histogenetic Classification Predicts Outcomes in 130 Adults With Chronic Intestinal Pseudo-Obstruction.

INTRODUCTION: Chronic intestinal pseudo-obstruction (CIPO) is a rare, heterogeneous disorder associated with severe morbidity. Genetic variants and histopathological lesions have been described, but their combined evaluation has rarely been assessed in adult CIPO cohorts. This study aimed to evaluate the contribution of integrated genetic and histological analyses in adults with CIPO. METHODS: We conducted a retrospective observational study of adults with CIPO followed at a national tertiary referral center. Patients with CIPO underwent genomic profiling, including next-generation sequencing

[Clinical features of Amyand's hernia: from intraoperative finding to successful repair].

Amyand's hernia is rare among inguinal hernias (< 1% of patients). Inflammation of appendix in hernial sac is observed in less than 0.1% of cases. Diagnosis of this hernia is difficult because preoperative diagnostic methods often fail to visualize appendix. Thus, final diagnosis is often established intraoperatively. The authors present successful treatment of Amyand's hernia in a 54-year-old patient with recurrent inguinal hernia and 3 previous surgical corrections (the last one 7 years ago). Hernia was characterized by periodic self-reduction of protrusion. Within 72 hours before hospitaliz

[Primary humoral immunodeficiencies associated with enteropathies: An update].

Inflammatory enteropathies are frequent in primary immune deficiencies (PID) (up to 21%). Their pathophysiology is complex and combines immune abnormalities (loss of tolerance, deficit in mucosal IgA secretion and altered infectious response) with an abnormal environment (dysbiosis, production of pro-inflammatory endotoxins, epigenetic and epitranscriptomic alterations). This leads to a deregulated inflammatory state of the gastrointestinal tract and thus to enteropathy. These conditions are heterogeneous and manifest not only as an inflammatory bowel disease but also as in lymphocytic, autoim

Fistulotomy for superficial or minimal sphincter-involving fistulae in perianal Crohn's disease: do they heal?

BACKGROUND: Fistulotomy is highly effective (> 90%) for cryptoglandular fistula-on-ano, but fistulotomy in perianal Crohn's disease (pCD) is limited due to increased risk of recurrent fistulae, diarrhea, and fecal incontinence. We hypothesized that superficial fistulotomy resulted in wound healing in most patients. METHOD: We conducted a single-center retrospective review of adult patients with pCD who underwent fistulotomy from 1999 to 2022. Baseline characteristics, pCD characteristics, and short- and long-term surgical and functional outcomes were reported. Matched-pair Wilcoxon signed-ran

GM-CSF promotes pro-inflammatory macrophage activation associated with Akt/mTOR signaling during experimental colitis.

BACKGROUND: Ulcerative colitis (UC) is an intestinal immune disorder of unknown etiology. Mounting evidence reveals a central role of macrophages in the hemostatic balance of gut immunity, and dysfunctional macrophages are associated with UC pathogenesis. Granulocyte macrophage-colony stimulating factor (GM-CSF) is an essential modulator of macrophages and has recently been recognized as a potential target in many autoimmune disorders. However, the action of GM-CSF in gut inflammation remains unspecified. METHODS: The significance of GM-CSF in UC and its mechanism of action were investigated.

CBC-derived inflammatory indices predict adverse outcomes in geriatric emergency patients with acute infectious diarrhea: a MIMIC-IV analysis.

Acute infectious diarrhea remains a leading cause of morbidity and mortality worldwide. Similar to pediatric populations, geriatric patients represent a highly vulnerable group with increased hospitalization rates, prolonged length of stay, heightened dehydration susceptibility, diminished physiological reserve, and elevated mortality risk. While pediatric populations benefit from well-established clinical severity scores such as Vesikari and Clark scales, analogous prognostic tools specifically validated for geriatric patients with acute infectious diarrhea remain lacking. Specific inflammato

Integrated, Cross-Entity Information on Preventive Measures for Bowel, Breast, and Prostate Cancer: Evaluation Study of the Web Application "Prevent-Take-Up".

BACKGROUND: About 40% of cancers are preventable through evidence-based interventions; however, uptake remains suboptimal. Knowledge and acceptance of primary and secondary preventive measures in the general population is not sufficient. We hypothesized that a web-based tool providing comprehensive, easily accessible, and individualized information on preventive strategies for multiple tumor entities could support informed decisions. OBJECTIVE: This study aimed to evaluate an interactive web application offering guideline-based, risk-adapted information on preventive measures for colorectal,

Association Between Biomarkers of Environmental Enteric Dysfunction and Linear Growth and Neurodevelopment in Guatemalan Infants.

Growing evidence implicates environmental enteric dysfunction (EED) as a driver of poor growth and neurodevelopment (ND) in early childhood. To investigate these findings, a cross-sectional study examining associations between biomarkers reflecting various domains of EED and growth and ND in Guatemalan infants was conducted. A subset of 114 cohort infants was randomly selected from a 2017-2019 population-based cohort study of 499 infants in rural southwest Guatemala. Growth and neurodevelopmental performance were assessed at a household visit at ∼13 months of age using the Mullen Scales of Ear

A novel insertion/deletion in APC promotor 1B is associated with both gastric and colon polyposis.

Pathogenic variants in the APC gene are classically associated with autosomal dominant familial adenomatous polyposis (FAP), characterized by tens-to-thousands of colonic adenomatous polyps and a high-penetrance predisposition to colorectal cancer. More recently, specific PVs in the YY1 binding motif of APC promoter 1B have been associated with autosomal dominant gastric adenocarcinoma and proximal polyposis of the stomach (GAPPS), characterized by tens-to-thousands of fundic gland polyps and a predisposition to gastric cancer but which are only rarely associated with features consistent with
