Objective validation of the Crohn's Anal Fistula Quality of Life questionnaire using quantitative MRI fistula volume in perianal fistulising Crohn's disease.

BACKGROUND: The Crohn's Anal Fistula Quality of Life (CAF-QoL) questionnaire is the first disease-specific patient-reported outcome measure (PROM) for perianal fistulising Crohn's disease (pfCD). However, validation against objective measures of disease burden is limited. We evaluated the relationship between CAF-QoL and quantitative MRI fistula volume to determine whether MRI-derived disease burden supports the construct validity of this PROM. METHODS: Adults with pfCD were prospectively recruited from a tertiary referral centre. Patients underwent pelvic MRI and completed CAF-QoL within 3 m

Laser Fissurectomy Combined with Limited Lateral Internal Sphincterotomy for Chronic Anal Fissure: A Minimally Invasive Technique.

Chronic anal fissure (CAF) is a common benign anorectal condition characterised by severe pain and internal anal sphincter (IAS) hypertonia. Although lateral internal sphincterotomy (LIS) remains the surgical gold standard, the risk of postoperative faecal incontinence remains a clinically significant concern. We describe a minimally invasive hybrid technique combining 1470-nm diode laser fissurectomy with a limited closed LIS. The laser ablates the chronic ulcer triad whilst a tailored sphincterotomy confined to the distal one-third of the IAS addresses the underlying hypertonia. Based on pub

Imaging of the Anal/Perianal Fistula.

Perianal fistulas can result in notable morbidity and reduced quality of life for those affected, most often men 30 to 40 years old. A thorough understanding of a patient's fistula anatomy is crucial in determining appropriate treatment; MRI is highly effective in providing this information. Additionally, MRI can be used following treatment to determine success or failure and aid with potential next steps. This article provides a comprehensive review of the MRI appearance of perianal fistulas before and after treatment, with emphasis on imaging features that support accurate characterization a

Accurate Localization of the Internal Opening in a Cryptoglandular Anal Fistula: Complementary Roles of Goodsall's Rule, MRI, and Intraoperative Assessment: A Case Report.

Accurate localization of the internal opening is essential for successful surgical management of cryptoglandular anal fistulas. Although Goodsall's rule remains a useful clinical guide, accurate delineation of fistula anatomy often requires complementary assessment with magnetic resonance imaging (MRI) and intraoperative evaluation. We report the case of a 34-year-old healthy man presenting with a recurrent perianal fistula characterized by intermittent swelling and purulent discharge. The external opening was located at the 2 o'clock position, more than 3 cm from the anal verge. Preoperative

Disease characteristics, outcomes, and poor prognostic factors of perianal fistula in Korean patients with Crohn's disease: a multicenter, retrospective observational study.

BACKGROUND/AIMS: A high proportion (39%-50%) of Korean patients with Crohn's disease (CD) suffer from perianal fistula, and real-world data on clinical practice remain limited. This study aimed to identify characteristics of perianal fistulizing CD (pfCD), treatment patterns, disease outcomes, and factors associated with poor prognosis in clinical practice in Korea. METHODS: This was a multicenter, retrospective, observational study using chart reviews and clinical assessment of adult patients with pfCD from 15 medical centers. Patients were aged ≥ 19 years and had been treated for a newly di

Does omission of a loose seton before sphincter-preserving surgery achieve non-inferior fistula closure compared with routine loose seton placement in adults with single-tract cryptoglandular anal fistula in the Netherlands? Study protocol for the SETON multicentre randomised controlled non-inferiority trial and cost-utility analysis.

INTRODUCTION: Cryptoglandular anal fistulas are burdensome, often recurrent and frequently treated in two stages using a loose seton before definitive sphincter-preserving surgery. Although this practice is common, high-quality evidence supporting improved healing or functional outcomes is lacking. A trial is needed to determine whether omitting routine seton placement can reduce procedures, patient burden and costs without compromising fistula closure. METHODS AND ANALYSIS: The SETON trial is a multicentre, randomised, unblinded non-inferiority trial with an integrated cost-utility analysis.

Chikungunya case cluster with nonspecific symptoms: implications for primary care in Switzerland.

BACKGROUND: Early clinical presentation of chikungunya fever and other arbovirus infections is often nonspecific, potentially delaying diagnosis, particularly in settings where these diseases are less frequently considered. Competent vectors are expanding in Switzerland, increasing the risk of autochthonous arbovirus transmission. CASE SERIES: We report seven cases in a group of travellers returning from Kenya in summer 2025. All patients presented with fever, arthralgia affecting the small joints of the hands and feet, nausea, diarrhoea, and rash. Additional symptoms included generalised lym

Long-term clinical outcomes of stem cell-based interventions in Crohn's-related perianal fistula: A systematic review and meta-analysis.

BACKGROUND: Perianal fistulas are a debilitating complication of Crohn's disease, frequently refractory to conventional medical and surgical therapy. Stem cell-based interventions, particularly mesenchymal stem cells (MSCs), have emerged as a promising therapeutic alternative, yet long-term outcomes remain uncertain. METHODS: Web of Science, PubMed, Embase, Scopus, and the Cochrane Library were searched through September 1, 2024. Studies evaluating stem cell or stem cell-derived products for Crohn's disease-associated perianal fistulas with ≥12 months' follow-up were included. Efficacy outcom

Efficacy and safety of tumor necrosis factor-α inhibitors and mesenchymal stem cells in the treatment of fistulizing Crohn's disease: a systematic review and network meta-analysis.

BACKGROUND: Tumor necrosis factor-α inhibitors (anti-TNF-α) and mesenchymal stem cells (MSCs) have been proven to be effective for Perianal fistulizing Crohn's disease (PFCD). However, no head-to-head clinical trials have directly comparing these two therapies. Therefore, using a network meta-analysis and systematic review, we evaluated the efficacy and safety of anti-TNF-α and MSCs in PFCD patients. METHODS: We included randomized controlled trials (RCTs) evaluating anti-TNF-α or MSCs against placebo (PBO) in PFCD patients. The primary efficacy outcomes were fistula remission and fistula res

The Aesthetic Form of Perezhivanie: An Ethnographic Study of Becoming an Environmental Scholar-Activist.

End-stage renal disease is closely associated with changes in bone and mineral metabolism. In recent times, osteoporosis has become important among hemodialysis (HD) patients. In this study, the investigators sought to evaluate the relationship between bone mineral density (BMD) and biochemical markers of bone turnover among HD patients. A total of 70 uremic patients on a maintenance HD program for at least 1 y were enrolled in the study. All patients were treated with conventional bicarbonated HD for 5 h through the use of low-flux hollow-fiber dialyzers. Bone densitometry was measured by dua

Endoscopic vacuum therapy for leaky cavities: is it possible?

A 71-year-old male patient with a history of bladder neoplasia underwent Bricker-type surgery, during which an iatrogenic rectal injury occurred. During surgery, an unsuccessful suture attempt was made, leading to the appearance of fecaluria after 48 hours. A computed tomography scan revealed a small continuity defect in the rectal wall, accompanied by a 25-mm adjacent collection. Percutaneous drainage was placed in an attempt to achieve spontaneous closure, but this was unsuccessful. A rectoscopy was performed, identifying a wall defect in the mid-rectum. A review with a paediatric gastroscop

Gynaecological gastrointestinal crossroad: rectouterine fistula presenting as menochezia.

Recto-uterine fistula is an exceptionally rare clinical entity characterised by an abnormal communication between the rectum and uterine cavity. We report the case of a young woman who presented with cyclical passage of blood per rectum coinciding with menstruation-a symptom known as menochezia. Her symptoms began approximately 2 weeks after undergoing a dilatation and curettage procedure performed for second-trimester miscarriage. MRI revealed a fistulous tract of 5 mm between the posterior uterine wall and anterior rectal wall, confirming the diagnosis. Laparoscopic and hysteroscopic evaluat

Clinical Trial Endpoints for Perianal Fistulizing Crohn's Disease.

Perianal fistulizing Crohn's disease (pCD), a structurally damaging condition of the anorectum and perineum, can result in debilitating symptoms and significantly impair quality of life. The lack of high-quality randomized controlled trials (RCTs) with primary endpoints specifically validated for pCD has contributed to suboptimal treatment recommendations. This review discusses considerations for pCD RCTs and provides expert opinion on current endpoints in the context of guidance from regulatory authorities. A challenge designing pCD RCTs is that placebo response rates and treatment effect siz
