Chronic anal fissure (CAF) is a common benign anorectal condition characterised by severe pain and internal anal sphincter (IAS) hypertonia. Although lateral internal sphincterotomy (LIS) remains the surgical gold standard, the risk of postoperative faecal incontinence remains a clinically significant concern. We describe a minimally invasive hybrid technique combining 1470-nm diode laser fissurectomy with a limited closed LIS. The laser ablates the chronic ulcer triad whilst a tailored sphincterotomy confined to the distal one-third of the IAS addresses the underlying hypertonia. Based on pub

BACKGROUND: Early clinical presentation of chikungunya fever and other arbovirus infections is often nonspecific, potentially delaying diagnosis, particularly in settings where these diseases are less frequently considered. Competent vectors are expanding in Switzerland, increasing the risk of autochthonous arbovirus transmission.
CASE SERIES: We report seven cases in a group of travellers returning from Kenya in summer 2025. All patients presented with fever, arthralgia affecting the small joints of the hands and feet, nausea, diarrhoea, and rash. Additional symptoms included generalised lym

OBJECTIVES: Seronegative spondyloarthropathy (SpA) such as ankylosing spondylitis (AS) affects patients during their working years and may contribute to work disability (WD). We determined the prevalence of WD (not working due to illness) and limitations in work productivity in AS using surveys, including the Work Limitations Questionnaire (WLQ).
METHODS: This cross sectional study consisted of 203 patients with SpA received a mailed questionnaire asking about work status, the WLQ, HAQ, BASDAI, BASFI, BAS-G and Functional Comorbidity Index. Relationships between WD, WLQ, demograghics and dise

BACKGROUND: Bariatric surgery is a prevalent and effective treatment for morbid obesity, yet its potential long-term effects on anorectal health remain an under-investigated aspect of post-operative care. This study aimed to meticulously evaluate the incidence and the degree of severity of two common anorectal conditions, anal fissures and hemorrhoids, in a substantial cohort of patients following either sleeve gastrectomy (SG) or Roux-en-Y gastric bypass (RYGB), two frequently performed bariatric procedures. Understanding these potential post-surgical morbidities is crucial for a comprehensiv

OBJECTIVE: Chronic anal fissures (CAF) is a common cause of severe anorectal pain in adults causing poor quality of life. In case of failure of conservative treatments, lateral internal sphincterotomy (LIS) is an effective cure. We aimed to analyse the outcomes of LIS in a consecutive patients' series operated in a high volume Surgical Coloproctology Unit.
METHODS: A retrospective cohort study was conducted on 422 patients with CAF undergoing surgical treatment between January 2015 and December 2019 at our Surgical Coloproctology Unit. Patients over the age of 18, not responding or recurring

BACKGROUND: Lateral internal sphincterotomy is a well-established treatment for chronic anal fissures. Building on this, probe-guided intersphincteric lateral internal sphincterotomy uses a probe-assisted paramedian incision to integrate fissurectomy with sphincterotomy while prophylactically addressing potential crypt pathology. This anatomically guided, standardized approach broadens its applicability to both typical and complex fissures while remaining safe, reproducible, and technically accessible, particularly for less experienced surgeons.
IMPACT OF INNOVATION: This modified technique i

BACKGROUND: Anal fissure, a common anorectal disorder, usually presents with pain, bleeding, and itching during or after defecation. These fissures can be classified as acute, chronic, or acute on chronic, depending on the duration of symptoms (6 weeks or less for acute and more than 6 weeks for chronic). Traditional treatment involves medical interventions to reduce sphincter tone and improve blood flow. Topical calcium channel blockers (CCBs) have shown promising results in conservative management. This study aimed to compare the efficacy and safety of Nifedipine versus Diltiazem ointments i

BACKGROUND: Anal fissure, fistula-in-ano, and anorectal abscess are common benign anorectal conditions that contribute substantially to pain, morbidity, repeated hospital visits, and procedure-related workload in surgical practice. Because these conditions differ in presentation, anatomical characteristics, and treatment pathways, institution-specific data on their clinical profile and early outcomes are useful for improving decision-making and follow-up in tertiary care settings.
OBJECTIVES: To describe the clinical profile and symptom pattern of patients with anal fissure, fistula-in-ano, a

BACKGROUND: Parikarthika refers to the condition in which the individual experiences a sensation of pain as if the Guda is being cut around with scissors. This condition can be correlated to the fissure in ano. It has been described as an acute superficial break in the continuity of the anoderm, usually in the posterior midline of the anal canal. The incidence of anal fissures is around 1 in 350 adults. Management can be divided into conservative, operative, and adjuvant therapies. The main challenge in the management of fissure in ano is the increased rate of recurrence. Here comes the import

BACKGROUND: Chronic anal fissures (CAF) are a common anorectal condition that significantly impacts patients' quality of life. Treatment options include pharmacological therapies, such as botulinum toxin (BT) and topical nitroglycerin (TNG). Despite their widespread use, the relative efficacy and safety of BT and TNG remain controversial. This systematic review and meta-analysis is aimed at comparing the effectiveness, recurrence rates, and adverse effects of BT and TNG in the treatment of CAF.
METHODS: A comprehensive search was conducted across multiple databases, including PubMed, Scopus,

AIM: The aim of this study was to investigate if the presence of a prominent sentinel pile (SP) had any impact on the treatment success of botulinum toxin (BT) injection, as well as the clinical presentation of patients with chronic anal fissure (CAF).
METHODS: Patients with CAF with or without prominent sentinel piles underwent BT injection. In addition to objective healing, a detailed symptom severity score (REALISE) immediately before and 6 months after BT injection was recorded. This was a retrospective, single-center observational cohort study including consecutive patients treated in a

Chronic anal fissure (CAF) is a common anorectal disorder associated with severe pain and impaired quality of life. Botulinum toxin A (BTA) injection has emerged as a minimally invasive alternative to lateral internal sphincterotomy; however, data regarding high-dose Dysport® protocols and the clinical relevance of booster injections remain limited. This retrospective single-center observational study was conducted at the General Surgery Clinic of Mersin City Training and Research Hospital. Patients treated for CAF between January 1, 2025, and December 1, 2025, were identified through institut

BACKGROUND: Chronic anal fissure is a painful condition that can greatly affect quality of life. When conservative treatment fails, lateral internal anal sphincterotomy is performed to promote healing. The procedure may involve partial or complete sphincter division, which can influence the risk of fecal incontinence. This study aimed to compare the outcomes of tailoring the length of the lateral internal anal sphincter with either the whole length or the fissure apex and their effect on fecal continence in the treatment of chronic anal fissure.
METHODS: This comparative, prospective, observa

BACKGROUND: Chronic anal fissure is a common, benign anorectal condition associated with impaired quality of life. Despite the availability of several therapeutic options, considerable variability persists in its management, particularly regarding the role of botulinum toxin type A administration within the current treatment algorithms. Although botulinum toxin type A injection is a widely used approach to achieve chemical sphincterotomy, its real-world efficacy and safety across different clinical scenarios remain insufficiently characterised.
AIM: Our objective is to evaluate the efficacy a

BACKGROUND: Acute anal fissure is a common cause of severe pain in the anorectal region. The standard treatment is the topical application of a calcium channel blocker or glyceryl trinitrate. Despite acute anal fissure being a common proctologic condition, data on the healing rates and long-term outcomes remain scarce. This study aimed to evaluate data from our centre, with a special focus on long-term follow-up and recurrent disease.
METHODS: All consecutive patients who presented with acute anal fissure between January 2016 and December 2016 were retrospectively identified. Patients were in

This prospective open-label comparative study evaluated the efficacy of various conservative treatments for acute anal fissure (AAF) in accelerating epithelialization and reducing pain. A total of 120 patients with symptom duration ≤6 weeks were randomized into four groups: control (hygiene only), dietary therapy (fiber-rich diet + psyllium), monotherapy with a sphincter relaxant (0.2% nitroglycerin ointment), and combined therapy (diet + nitroglycerin). The primary outcome was time to complete epithelialization; secondary outcomes included pain dynamics on the Visual Analog Scale (VAS) and in
