BACKGROUND: After the failure of conservative therapy, the most effective surgical treatment for chronic anal fissures (CAFs) is lateral internal sphincterotomy. However, the choice of the procedure must be always evaluated carefully due to the risk of long-term anal continence impairment. The aim of the present study is to report the outcomes of fissurectomy with or without associated anoplasty.
METHODS: This study is a single-center retrospective study including patients with CAFs in whom conservative medical and non-medical treatments failed and who underwent fissurectomy with or without a

Over 40% of children with Crohn's disease (CD) develop perianal lesions (skin tags and/or fissures), which are associated with the risk of perianal fistula. It is unknown how reliably perianal symptoms predict perianal lesions. We recruited pediatric patients with CD who were undergoing colonoscopy for clinical indications. Patients self-reported recent perianal symptoms. We retrospectively reviewed the perianal exam documented on colonoscopy reports and in the electronic health record to obtain details of perianal lesions if present. Thirty-three patients were included: median age 15.6 years

An anal fissure is a benign and painful ulcer extending from the pectinate line to the anal margin. It leads to an increase in the resting pressure of the internal anal sphincter and the pressure within the anal canal, resulting in local ischemia and impaired wound healing. Anal fissures are mostly located in the posterior midline. They are primarily caused by local trauma to the anoderm, often due to the passage of hard stools, irritation from diarrhea, or anorectal surgery. For both acute and chronic anal fissures, several treatment options are available, and surgery typically reserved as a

BACKGROUND: Despite the superior success rate of lateral internal sphincterotomy for managing chronic anal fissure (CAF), there is a trend towards sphincter-preserving treatment due to the increased risk of incontinence. Botulinum toxin (BT) and fissurectomy are two sphincter-preserving options for CAF. We aim to assess if combining fissurectomy with botox treatment is superior to botulinum toxin alone in the management of CAF.
METHODS: This retrospective cohort study was conducted across two Sydney hospitals over 7 years. All patients with a CAF managed with either BT and fissurectomy (group

BACKGROUND: Botulinum toxin (BT) is a nonsurgical alternative to lateral internal sphincterotomy (LIS). While there are promising results, there is still a gap in knowledge regarding long-term outcomes and the predictors for healing after BT.
METHODS: Chronic anal fissure (CAF) patients treated with 100 IU BT with a minimum 5-year follow-up were analyzed retrospectively. Patients with persistent or recurrent fissures after their first BT injection were offered either a second BT injection or LIS. Healing was defined as complete symptom resolution with fissure epithelization. The primary outco

BACKGROUND: Anal injuries, such as lacerations and fissures, are challenging to diagnose because of their anatomical complexity. Endoanal ultrasound (EAUS) has proven to be a reliable tool for detailed visualization of anal structures but relies on expert interpretation. Artificial intelligence (AI) may offer a solution for more accurate and consistent diagnoses. This study aims to develop and test a convolutional neural network (CNN)-based algorithm for automatic classification of fissures and anal lacerations (internal and external) on EUAS.
METHODS: A single-center retrospective study anal

Lateral internal sphincterotomy (LIS) has long been regarded as the standard treatment for chronic anal fissure (CAF). Both open and closed methods are practiced for LIS. In this study evaluated and compared the early clinical outcome in terms of patient morbidity and procedure related complications of both the methods. This was a quasi-experimental study conducted in the department of Surgery, Sir Salimullah Medical College Mitford Hospital, Dhaka, Bangladesh (SSMCMH), Dhaka, Bangladesh from May 2018 to April 2019. Fifty (50) patients of chronic anal fissure had been chosen purposively and di

An anal fissure is a painful lesion of the anal canal. The acute form often heals spontaneously and should be treated conservatively. The treatment of chronic anal fissures is the subject of many studies but the treatment recommendations of international guidelines are contradictory. Conservative treatment consists of chemical sphincterotomy using sphincter-relaxing drugs, such as calcium channel antagonists, nitrates or botulinum toxin and leads to healing in only about 50% of patients with a prolonged healing time and a high rate of recurrence. In contrast surgical treatment is much more eff

BACKGROUND: Chronic anal fissures (CAF) are a common proctological condition that significantly impacts patients' quality of life. Lateral internal sphincterotomy (LIS) is widely considered the gold-standard treatment for CAF; however, postoperative fecal incontinence remains a potential risk. The aim of this study is to evaluate long-term incontinence rates following traditional LIS for CAF.
METHODS: A retrospective analysis was conducted on patients with CAF who underwent traditional LIS. Patients with any degree of continence impairment prior to surgery, as well as those who experienced ev

Anal fissure causes pain and bleeding during or after bowel movements, significantly impacting individuals' quality of life. Current treatments aim to interrupt this cycle but have associated risks and limitations. The emergence of arginine, crucial for protein creation and nitric oxide (NO) production, presents an intriguing therapeutic avenue by the impact on reducing anal sphincter pressure and enhancing anoderm blood flow, due to its roles in vasodilation, anti-inflammatory responses, and collagen synthesis, which can promote wound healing and highlighting its potential as an alternative t

INTRODUCTION: An anal fissure is a common anorectal condition characterised by a linear tear in the lower anal canal, causing severe pain and rectal bleeding. Topical chemical sphincterotomy using agents such as diltiazem has become a popular non-surgical treatment option.
METHODS: This prospective observational study was carried out over 12 months at the Department of Surgery, Himalayan Institute of Medical Sciences, Dehradun, India. Six hundred patients aged 18-60 years with acute, acute-on-chronic, or chronic anal fissure and no previous treatment history were enrolled. Patients received 2

This evidence-based practical review is focused on the clinical features, investigations, and treatment of hemorrhoids, chronic anal fissures, dyssynergic defecation (DD), and fecal incontinence (FI), which are arguably the most common benign anorectal diseases encountered by gastroenterologists. These diseases are associated with bowel disturbances, which should be evaluated preferably with questionnaires, and with anal weakness and/or DD, often evident on a thorough digital rectal examination. Fissures and DD are closely linked to constipation, whereas FI is typically associated with diarrhe

Anal fissures are a common anorectal condition typically occurring in the midline. They often present with pain and bleeding though presentation can be variable. Most fissures are associated with hypertonic anal sphincter, though other etiologies include infection, inflammatory bowel disease, and malignancy. Diagnosis is primarily clinical. Initial management involves increased fiber and fluid intake, and symptom management. Chronic fissures may require topical nitrates, calcium channel blockers, botulinum toxin, or surgery. Lateral internal sphincterotomy is the gold standard surgical managem

Anal fissures (AF) are painful anorectal disorders for which topical calcium-channel blockers (CCBs), such as amlodipine besylate (AMD), are recommended to avoid systemic adverse effects associated with oral therapy. However, the topical use of AMD has been limited by its poor aqueous solubility and chemical instability. This study focused on developing and characterizing a 1% w/w AMD-loaded microemulsion (ME) for topical application. Optimal composition was selected using solubility screening and pseudo-ternary phase diagrams. The lead formulation, M5, demonstrated favorable properties: nanome

BACKGROUND: Chronic anal fissures in patients with Crohn's disease (CD) remain a significant therapeutic challenge, particularly when linked to active perianal disease. Conventional treatments often fail, highlighting the need for alternative approaches. This study explores the efficacy and safety of freshly collected autologous adipose tissue injection (AATI) for treating chronic fissures in patients with CD.
METHODS: Nine patients with CD with anal fissures were included. The primary outcome was complete healing (CH) at 3 months after last AATI, defined as full fissure re-epithelialization

Among the various treatment modalities for chronic anal fissures, lateral internal sphincterotomy, acknowledged as the gold standard, is associated with superior healing rates but entails a higher risk of complications due to its invasive nature. Conversely, botulinum toxin injection, a minimally invasive alternative, remains a viable therapeutic option for select patient populations despite its relatively higher recurrence rate, owing to its favorable safety profile. The objective of this study was to compare the botulinum toxin and sphincterotomy efficacy in the management of chronic anal fi

INTRODUCTION: Chronic anal fissure is a common clinical entity seen in surgical practice. Lateral internal sphincterotomy is considered the gold standard for treating chronic fissures. But recent developments in understanding the pathophysiology of anal fissures have helped to develop better conservative treatment options. In this study, we compare the healing, symptomatic relief, and side effects of topical 2% Diltiazem gel versus lateral internal sphincterotomy in the treatment of chronic anal fissures.
