Colostomy for Bowel Diversion in Aggressive Diverticulitis With Colovesical and Colocutaneous Fistulae: A Case Report.

Fistulizing diverticulitis is an uncommon and aggressive manifestation of diverticular disease. Most patients develop a single fistula; multiple synchronous fistulae are less commonly reported. We present a case of a 55-year-old man with a history of anorectal fistula repair who presented with recurrent abdominal pain, urinary symptoms, and weight loss. Imaging revealed complicated sigmoid diverticulitis with multiple fistulae involving the bladder, rectum, and abdominal wall. Initial management included abscess drainage and a diverting transverse loop colostomy, which successfully controlled

Complex transsphincteric ano-rectal fistula with circumferential perirectal abscess and periprostatic extension: the added value of contrast-enhanced pelvic MRI.

Complex perianal fistulas can harbor extensive secondary extensions and pelvic sepsis despite minimal systemic symptoms and normal inflammatory markers, creating a high risk of incomplete surgery when anatomy is underestimated. We report a case of a complex transsphincteric fistula (Parks classification) with suspected supralevator involvement in which preoperative MRI provided decisive anatomic mapping for operative planning. Multiplanar T2-weighted imaging delineated the primary tract, its relationship to the sphincter complex and levator plate, and extension into the ischioanal/ischiorectal

Surgical and functional outcomes of endorectal advancement flap repair in anorectal fistulas.

INTRODUCTION: The surgical management of anorectal fistulas requires achieving fistula healing while preserving anal sphincter function. The endorectal advancement flap is a well-established sphincter-sparing technique; however, outcome data in specific patient populations remain clinically relevant. This study aimed to evaluate surgical success and functional outcomes of the endorectal advancement flap in patients with cryptoglandular disease. METHODS: We conducted a retrospective analysis of adult patients who underwent elective endorectal advancement flap surgery between 2016 and 2022 at t

High-resolution direct magnetic resonance imaging fistulography with hydrogen peroxide for diagnosing anorectal fistula: A preliminary retrospective study.

BACKGROUND: Fistula-in-ano is an abnormal tunnel formation linking the anal canal with the perineum and perianal skin. Multiple imagining methods are available to evaluate it, among which magnetic resonance imaging (MRI) is the most advanced noninvasive preoperative method. However, it is limited in its visualization function. AIM: To investigate the use of intraluminal MRI for perianal fistulas via a novel direct MRI fistulography method. METHODS: We mixed 3% hydrogen peroxide (HP) with gadolinium for HPMRI fistulography, retrospectively analyzing 60 cases of complex/recurrent fistula-in-an

Understanding the anatomical basis of anorectal fistulas and their surgical management: exploring different types for enhanced precision and safety.

Anorectal fistulas remain one of the most challenging conditions in colorectal surgery and require precise anatomical knowledge for successful management. This comprehensive review synthesizes the current evidence on the anatomical foundations of fistula development and treatment, particularly focusing on the cryptoglandular hypothesis and its clinical implications. A systematic analysis of the recent literature has examined the relationship between anatomical structures and fistula formation, classification systems, diagnostic modalities, and therapeutic approaches. The review revealed that a

Catheter Injectable Multifunctional Biomaterial for the Treatment of Infected Enterocutaneous Fistulas.

Enterocutaneous fistulas (ECF) are challenging to treat contributing to high morbidity and high mortality rates, significantly impacting the quality of life of the patients. Its susceptibility to antibiotic-resistant infections often leads to chronic inflammation, complicating treatment with conventional methods. Here, 18NC75-10P-1IL is reported, which is a multi-functional shear-thinning hydrogel comprised of gelatin and nanosilicates for injectability, an ionic liquid for bactericidal effects, and platelet rich fibrin fraction for pro-healing properties; this biomaterial is engineered for th

Preclinical testing of a new radiofrequency ablation device in a porcine perianal fistula model.

PURPOSE: Anorectal fistulas present a treatment challenge, with conventional surgical methods potentially resulting in complications such as fecal incontinence. To improve patient outcomes, more effective and minimally invasive therapies are critically needed. In this study, an optimal porcine model for the creation of anorectal fistulas was developed and used to evaluate the efficacy of radiofrequency ablation (RFA) therapy. METHODS: Two distinct but related experiments were conducted. In the first experiment, a reliable and standardized porcine anorectal fistula model was developed. In the

The Role of Magnetic Resonance Imaging in Pre-operative Assessment of Anorectal Fistula With Surgical Correlation.

Background Anorectal fistulas are chronic inflammations of peri-anal tissues that form a connection between the perineal skin and the anal canal. Accurate preoperative evaluation of the main fistula's trajectory and pelvic tissues is essential for effective surgical treatment of anal fistulas. The inability to detect concealed lesions may result in the recurrence of fistulas and the conversion of a simple fistula into a complex fistula. Magnetic resonance imaging (MRI) imaging can detect concealed pathways and abscesses, thereby exposing the intricate connection between the fistula and anal-sp

Closure of Anal Fistula with Laser- FiLaC™: a Novel Sphincter-Saving Procedure for Complex Fistula In-Ano.

There are several surgical options described for the treatment of anorectal fistulas, specially in complex cases where recurrence rates and the possibility of postoperative complications are still high. Laser- FiLaC™ has been described in this study as an option in the management of anorectal fistula. The aim of this study was to assess the novel radial laser probe treatment in complex fistula in-ano and report the success rate and recurrence rate. We studied retrospectively 56 patients who, according to our hospital patient records, underwent radial laser probe surgery between March 2019 and

Cellular angiofibroma arising from the rectocutaneous fistula in an adult: A case report.

BACKGROUND: Rectocutaneous fistulae are common. The infection originates within the anal glands and subsequently extends into adjacent regions, ultimately resulting in fistula development. Cellular angiofibroma (CAF), also known as an angiomyofibroblastoma-like tumor, is a rare benign soft tissue neoplasm predominantly observed in the scrotum, perineum, and inguinal area in males and in the vulva in females. We describe the first documented case CAF that developed within a rectocutaneous fistula and manifested as a perineal mass. CASE SUMMARY: In the outpatient setting, a 52-year-old male pat

Aerobic Microbiological Spectrum and Antibiotic Resistance in Children Operated for Anorectal Abscesses.

(1) Background: Anorectal abscesses are a relatively rare pathology in childhood. Most often, male children under 1 year of age are affected. The importance of microbiological examination for the diagnosis and treatment of such patients remains debatable among surgeons, resulting in scarce data being available in the literature. We aimed to identify the aerobic microbiological spectrum and antibiotic resistance of isolates in children undergoing operation to treat anorectal abscesses. (2) Methods: We performed a case series of 102 children diagnosed and operated for anorectal abscesses over a
