Severe resistant hypercalcemia complicating rhabdomyolysis: a case report and review of the literature.

BACKGROUND: Rhabdomyolysis is a clinical syndrome that follows destruction of skeletal muscles releasing a number of toxic substances and electrolytes causing hyperkalemia, hyperphosphatemia, and hypocalcemia. However, hypercalcemia may arise during the recovery phase of rhabdomyolysis and can be life-threatening. CASE PRESENTATION: A 31-year-old previously healthy Egyptian man was admitted to the surgical unit in the teaching hospital for 4-day history of peri-anal pain, fever, vomiting, and diarrhea. He was found to have peri-anal abscess for which he underwent multiple sessions of surgical

Long-term clinical outcome of a weekly 2-chlorodeoxyadenosine regimen in treatment-naïve patients with hairy cell leukemia.

Hairy cell leukemia (HCL) has a favorable clinical outcome with appropriate treatment; however, further research is needed on managing patients with relapsed or refractory disease and the risk of infection during prolonged periods. This study examined the long-term effects of 2-chlorodeoxyadenosine (2-CdA), particularly using a weekly infusion protocol, in treatment-naïve patients with HCL. This retrospective study evaluated the long-term follow-up data from 21 South Korean patients diagnosed with HCL. Among them, 20 patients were treated with a weekly infusion protocol (0.14 mg/kg/day over 5-

Racial disparity in inflammatory bowel disease-related complications: a nationwide cohort study.

BACKGROUND: Racial disparities in inflammatory bowel disease (IBD)-related complications are increasingly recognized, yet nationwide data remain limited. This study examined racial differences in IBD-related complications across diverse patient populations. METHODS: We analyzed data from the Nationwide Inpatient Sample 2016-2021, on over 1.7 million weighted hospitalizations for IBD. Adults with Crohn's disease (CD) or ulcerative colitis (UC) were identified using ICD-10 codes. Key outcomes included anal abscess, intestinal obstruction, rectal bleeding and anal fissure/fistula, were compared

Anal Canal Duplication With Abscess Formation and Associated Sacrococcygeal Hamartoma in an Adolescent: A Report of a Rare Case.

Anal canal duplication (ACD) is a rare congenital malformation, typically diagnosed in pediatric populations, and often associated with other anomalies. Due to its nonspecific symptoms, it may be misdiagnosed or diagnosed late, particularly in adolescents or adults. We report the case of a 17-year-old female patient who presented with rectal pain, fever, and purulent perianal discharge. Physical examination revealed an accessory anal opening posterior to the native anus. Magnetic resonance imaging (MRI) demonstrated a perirectal abscess, a duplicated anal canal, and a suspicious presacral mass

Anal Abscess and Fistula.

Anorectal abscesses and fistulas are common surgical conditions. Most often these are idiopathic and caused by cryptoglandular infections. Diagnosis relies on focused history and physical examination findings as well as imaging studies to determine the extent of sphincter involvement and exact location. Treatment of an abscess is prompt surgical drainage. Surgical management of a fistula is focused on preservation of continence while achieving healing. Optimal surgical treatment is based on anatomy of the fistula tract. Surgical options include fistulotomy, seton placement, advancement flap, L

Does antibiotic therapy for anal abscess reduce the risk of fistula surgery? A retrospective study.

BACKGROUND: Management of first-time perianal abscesses remains controversial. While the French school of proctology systematically investigates fistulas, other approaches advocate simple incision, given that over 60 % of patients do not develop recurrence. The role of post-incision antibiotic therapy is also debated. This study aimed at evaluating our department's approach to managing first perianal abscesses and identifying predictive factors for recurrence. METHODS: We retrospectively included all patients presenting in 2019 with a first perianal abscess treated by incision under local ane

Efficacy of amoxicillin/clavulanic acid after surgical drainage of perianal abscess in the prevention of the development of anal fistula (PERIQxA study): study protocol for a multicenter randomized, double-blind clinical trial.

BACKGROUND: Anorectal fistula, which is a relatively common pathology, is the chronic manifestation of the acute perirectal process that forms an anal abscess. The development of a fistula after incision and drainage of an anal abscess is seen in approximately 26-37%. Its treatment is a relevant topic, and the role of the use of antibiotic therapy in its prevention remains controversial, after the publication of several studies with contradictory results and several methodological limitations. Our hypothesis is that the combination of amoxicillin and clavulanic acid will reduce the incidence o

Anorectal Infections in Neutropenic Leukemia Patients: A Common Clinical Challenge.

Anorectal infections in neutropenic leukemia patients are a significant and potentially life-threatening complication. The pathogenesis of this condition is not entirely understood and believed to be multifactorial, including mucosal injury as a result of cytotoxic drugs, profound neutropenia and impaired host defense. Establishing an early diagnosis is key and often made clinically on the basis of signs and symptoms, but also from imaging studies demonstrating perianal inflammation or fluid collection. The management of anorectal infections in neutropenic leukemia patients is not straightforw

Complex Tunneling Perirectal Abscess: Intra-abdominal and Extraperitoneal Extension of a Persistent Perirectal Abscess.

Deep-tissue extension of perianal and perirectal abscesses, while rare, requires timely diagnosis and emergent surgical intervention to prevent serious secondary complications. This report evaluates a case of intra-abdominal and extraperitoneal extension of a persistent perirectal abscess that required comprehensive irrigation, drainage, and debridement of multiple abscess-associated cavities. This report follows the case of a 24-year-old African-American female presenting to the ED with mild fevers, nausea, abdominal distension, and lower abdominal pain following a persistent perirectal absce

[Anal Pathology in General Practice: A Syndromic Approach to Diagnosis].

Anal pathologies represent a common reason for consultation in general and specialized medical practice, often misinterpreted by patients as hemorrhoids. This review article focuses on providing a guide for the initial diagnosis and management of major anal pathologies, aiming to assist general practitioners and gastroenterologists in their daily practice. The differential diagnoses of the most prevalent syndromes, including anal tumor syndrome, prolapse syndrome, and painful syndrome are explored. The importance of a detailed anamnesis and a directed physical examination is emphasized. The ar

A Mendelian Randomization Study of the Connection Between Exogenous Hormones and Perianal Abscess in Pediatric Patients.

Background: Recent years have witnessed the hypothesis that bioavailable testosterone (BT) might be closely related to the development of inflammatory diseases, especially anal abscess (AA), a common inflammatory ailment with unclear pathogenesis. Given that AA is more prevalent among males, this study investigates the causal relationship between BT and AA. Objective: To explore the causal link between BT and AA, a Mendelian randomization (MR) study was conducted using large-scale genomic data. Materials and Methods: Utilizing genomic data from the UK Biobank and IEU OpenGWAS databases, a two-

[Current WSES-AAST guidelines on anorectal emergencies-Summary and comments].

In 2019, the World Society of Emergency Surgery (WSES) and the American Association for the Surgery of Trauma (AAST) generated consensus recommendations for the treatment of anorectal emergencies in Parma, Italy, and published a guideline in 2021. This is the first global guideline dealing with this important topic for surgeons' everyday work. Seven anorectal emergencies were discussed and the guideline recommendations were given according to the GRADE (Grading of Recommendations Assessment, Development and Evaluation) system.

Rapid development of cefiderocol resistance in a carbapenem-resistant Pseudomonas aeruginosa isolate associated with mutations in the pyoverdine biosynthesis pathway.

Here we report the in vivo development of cefiderocol resistance within 11 days after therapy initiation in a critically ill patient with bloodstream infection, infection of peri-anal fistula, and pneumonia caused by a VIM-2 harbouring, carbapenem-resistant Pseudomonas aeruginosa. Compared to a cefiderocol-naïve P. aeruginosa blood culture isolate, agar diffusion susceptibility testing found a reduced cefiderocol inhibition zone diameter in a P. aeruginosa recovered from peri-anal abscess tissue cultures after initiation of cefiderocol therapy. Subsequent whole-genome sequencing suggested that

Recurrent Syncope Episodes during Spinal Anesthesia for Perianal Abscess Drainage: A Case Report Emphasizing Pain as a Trigger.

BACKGROUND Vasovagal syncope is a loss of consciousness caused by decreased arterial pressure and cerebral blood flow. The characteristic features of vasovagal syncope include cardiovascular inhibition caused by neural reflexes, accompanied by vasodilation and bradycardia. To date, there is little literature to report several episodes of syncope under spinal anesthesia during the perioperative period for drainage of an anal abscess. The purpose of this article is to alert clinical practitioners to the early identification of the underlying causes of vasovagal syncope and to facilitate timely a

Rectum Perforation Secondary to Ingested Chicken Bone Presenting as Peri-Anal Abscess: A Case Report.

Rectal perforation secondary to an ingested foreign body is a rare occurrence that can be challenging to diagnose. It may initially present as a perianal abscess. Herein, we report a rare incident involving a patient who presented with a perianal abscess. The initial assessment and an abdominal CT scan revealed a large horseshoe perianal abscess with a small linear hyperdensity noted near the anal verge. The patient was taken to the operating room, where he was found to have perforated the rectum due to an ingested chicken bone. The procedure involved the incision and drainage of the abscess,
