OBJECTIVE: We report 4 cases of a special anal tumor featuring a long history of inflammatory signs and fistulas in that area.
DESIGN: A retrospective study from the histologic diagnosis back to the first symptom and up to the last follow-up or death.
PATIENTS: We studied all patients with a histologic diagnosis of mucinous adenocarcinoma of the anus admitted to our hospital.

PURPOSE: This study was undertaken to identify clinical characteristics, natural history, and results of medical and surgical treatment of anal fissures in Crohn's disease.
METHODS: This is a retrospective review of patients with Crohn's disease and anal fissure.
RESULTS: Of the 56 study patients, 49 (84 percent) had symptomatic fissures. Fissures were most commonly (66 percent) located in the posterior midline, and 18 patients (32 percent) had multiple fissures. Fissures healed in one-half of patients treated medically. In a prospective study on 224 patients with so-called high-fistula in ano (189 transsphincteric, 35 suprasphincteric) the long-term results of a sphincter-saving operation technique were assessed. The follow-up period was 1 to 7.5 years. This technique consists of one-stage fistulectomy as well as of drainage of the intersphincteric space by internal sphincterotomy. During the last 10 years (1982-1992) 370 patients have had a pelvic pouch constructed in the Colorectal Unit at the Department of Surgery II, Sahlgrenska Hospital. The complications and the eventual outcome in 307 patients (median follow-up 5.5 years ranging from 1.5 to 11 years) have been analysed. The overall complication rate was 22%. Anal abscess-fistulae have a tendency to recur based on variations in anatomic configuration and complexities of operative treatment and problematic postoperative healing. A classification of superficial, simple, complex, and recurrent fistulae was used to track outcome. Recurrence rates are substantially higher for those fistulae classified as complex. The reasons for this are discussed in this article.

A prospective study of a policy of selective immediate fistulotomy in the management of acute primary anal abscesses was performed. Eighty-nine patients (74%) underwent simple drainage only, as no internal openings were found during drainage of pus (group A). Thirty-one patients (26%) had drainage of pus and immediate fistulotomy (group B). Follow up for groups A and B occurred at a median of 122 weeks (104-136 weeks) and 121 weeks (104-136 weeks), respectively. No patient in group A had residual problems with anal continence whilst two patients (6.5%) from group B had minor anal incontinence

Chronic anal fissures have a varied and often confusing clinical presentation. To clarify this picture, we retrospectively studied 172 patients to determine associations between patient characteristics and manifestations of these lesions. We found that anterior fissures and sentinel piles were significantly more common in women; pruritus and rectal drainage occurred significantly more often in men; and patients with a longer duration of symptoms and those with pruritus and drainage were more likely to have a fistula. The results of operative treatment of anal abscesses with reviewed retrospectively for a period of three years with the object of correlating the microbiological findings on culture from the abscess with the presence of a fistula. Patients who had not been controlled postoperatively were requested to attend for anal examination. Fistula were demonstrated in 27 out of 83 patients. No fistulae nor recurrences of abscesses were found in patients in whom only bacteria not derived from the intestine were cultured. Culture from an anal abscesses is thus of considerable prognostic value.

Anal lesions were registered in 19 of 72 patients (26%) operated on for Crohn's disease at the Tampere University Hospital during 1966-1988. Abscess imaging with leukocyte scintigraphy is described in a case report. Autologous leukocytes were separated from whole blood in a patient with localized inflammation in the perineal region and labeled with 99mTc, an ideal radionuclide for clinical examination with gamma camera. Scintigraphic investigations demonstrated a deep-lying anal abscess. The procedure may prove useful for detection of occult infections and may provide a new diagnostic approach in fever of unknown origin.

Twenty-two patients with a peri-anal abscess or peri-anal fistula were studied selectively for the presence of HBsAg and anti-HBs. In 14 male and 8 female patients studied, the incidence of anti-HBs was 6 and of HBsAg 1. Positive findings occurred only in European male patients who invariably showed frequent homosexual contacts. It is concluded that, in homosexual patients with peri-anal abscess, the presence of HBsAg and anti-HBs should be determined as a matter of routine and all preventive measures should be taken for possible contamination with hepatitis B virus until proven negative.

Two cases of anal tuberculosis are reported. One presented as an ulceration in the anal canal and the other as an anal abscess and fistula. Both were diagnostic problems, tuberculosis was not suspected at first. Both were cured, one by medical treatment, the other by combined surgical and medical treatment.

Of 111 consecutive patients treated by partial internal sphincterotomy for a haemorrhoidal disease in the course of a prospective study, 101 could be re-examined between 24 and 36 months after operation.
