Hantavirus: A Comprehensive Narrative Review of Virology, Epidemiology, Pathogenesis, Clinical Manifestations, Diagnostics, and Emerging Therapeutic Strategies.

Hantaviruses are a genus of negative-sense, single-stranded, tri-segmented RNA viruses that are distributed worldwide. They cause two severe zoonotic diseases in humans: haemorrhagic fever with renal syndrome (HFRS) and hantavirus pulmonary syndrome (HPS), also known as hantavirus cardiopulmonary syndrome (HCPS). Hantaviruses are primarily spread by inhaling aerosolised excreta from infected rodent reservoirs. Over 50 rodent species on six continents have been found to carry hantaviruses, with only one reported case of human-to-human transmission, involving the Andes virus (ANDV) in South Amer

Hantavirus preparedness at human-rodent interfaces: lessons from China's HFRS control and Andes virus transmission in mobile populations.

Hantaviruses are globally distributed reservoir-associated viruses that cause haemorrhagic fever with renal syndrome (HFRS), mainly in Eurasia, and hantavirus cardiopulmonary syndrome (HCPS; also termed hantavirus pulmonary syndrome, HPS), mainly in the Americas. Most human infections arise from exposure to rodent-contaminated environments, so clustered disease may reflect shared environmental exposure rather than interpersonal transmission. This Review uses China's long-term HFRS experience as a central case study for interface-centred preparedness. China demonstrates that sustained surveilla

Access to licenced vaccines for high-threat infectious diseases: a scoping review and interpretive analysis of Ebola virus disease.

BACKGROUND: Outbreaks of high-threat zoonotic diseases like Ebola virus disease (EVD) occur more frequently, causing severe health and socioeconomic impacts. With an average case fatality rate of 67%, EVD outcomes can significantly improve when timely care and vaccination are provided. Despite the WHO’s prequalification of EVD vaccines (Ervebo and Zabdeno/Mvabea), their long-term access and use remain uncertain, with 40% of the global stockpile volume expired in 2024. With the launch of a new preventive vaccination program, understanding the evolving EVD immunization system to inform effective

Understanding perceptions towards COVID-19 vaccination and strategies to increase vaccine uptake among Ebola- affected communities in North Kivu, Democratic Republic of the Congo: a qualitative study.

BACKGROUND: North Kivu, in eastern Democratic Republic of the Congo (DRC), was the epicenter of the second largest Ebola Virus Disease (EVD) outbreak in history and has been an active conflict zone for decades. The COVID-19 pandemic further exacerbated an already complex situation. This qualitative study, conducted as part of a broader investigation into Ebola vaccine hesitancy and at the onset of COVID-19 vaccination rollout in the DRC, assessed perceptions, beliefs, and attitudes toward the COVID-19 virus and vaccines among community members and healthcare workers who had experienced the 201

Metabolic Basis of Post-Infectious Sequelae After Ebola Virus Disease.

Ebola virus disease (EVD) survivors often present with clinical sequelae after acute disease resolution, called post-Ebola syndrome (PES). Why some survivors develop these sequelae and others do not is poorly defined. Altered metabolism has been noted in acute EVD but not studied in PES. We identified differential expression of metabolites involved in multiple metabolic pathways in EVD survivors with PES. This included the tricarboxylic acid cycle, amino acid, nucleotide, and short chain fatty acid metabolism. Several of these pathways are associated with immune dysfunction. The identified met

Molecular characterization of Ebola virus glycoprotein V75A substitution in the 2018-2020 epidemic.

The 2018-2020 Ebola virus disease (EVD) epidemic facilitated the emergence of viral mutations, enhancing the potential for host adaptation during sustained human transmission. Here, we identified the Ebola virus (EBOV) glycoprotein V75A (GP-V75A) substitution as a dominant variant during the epidemic. This substitution, located within the receptor-binding domain, emerged early in the outbreak and rapidly reached high prevalence. GP-V75A demonstrated enhanced infectivity in multiple cell lines and murine models. Mechanistically, GP-V75A increased viral GP binding affinity to the host receptor N

Long-term psychosocial sequelae of Ebola virus disease among survivors compared with contacts following the 2013-2016 epidemic in Liberia.

Long-term psychosocial sequelae of Ebola virus disease (EVD) survivors are poorly understood. We conducted a cross-sectional study of a cohort of EVD survivors and their uninfected contacts in Liberia. Beginning in November 2019, we consecutively sampled eligible participants until the end of the parent study follow up. Enrolled participants completed a questionnaire administered by trained study staff. Outcome measurements were symptoms of depression (PHQ-8) and anxiety (GAD-7) as well as experiences of trauma (PC-PTSD-5), food insecurity (HFIAS), and social support (Duke-UNC SSQ). Excess pre

Readiness of health public facilities to diagnose, manage, and prevent the Ebola epidemic along border districts in Southwestern Uganda.

BACKGROUND: Ebola Virus Disease (EVD) outbreaks typically start from a single case of a probable zoonotic transmission and evolve into an epidemic because of the local and international movement of people, causing a global health security threat. This study aimed to assess the readiness of health facilities in the border districts of Southwestern Uganda towards prevention, diagnosis and management of EVD. METHODS: This was a cross-sectional study conducted between July and September 2024. The study was conducted in the districts that share a border with the Democratic Republic of Congo, which

Commemorating a decade of service: Reflections on Nigeria's deployment of 196 public health professionals to Liberia and Sierra Leone during the 2014-2015 Ebola crisis.

The West African Ebola virus disease (EVD) epidemic of 2013-2016 was the largest on record, severely affecting Guinea, Liberia, and Sierra Leone, and overwhelming public health systems. Following Nigeria's successful containment of its domestic EVD outbreak in 2014, the African Union Support to Ebola Outbreak in West Africa (ASEOWA) mission deployed 196 Nigerian public health professionals, the largest single-nation contingent, to Liberia and Sierra Leone. This commentary reflects on that deployment, highlighting operational contributions, innovations, and the enduring impact of the mission on

Characterization of Atypical Ebola Virus Disease in Ferrets.

Ebola virus (EBOV) typically results in a severe-and often lethal-acute disease. However, increasing evidence suggests that EBOV can persist in certain immune-privileged tissues, which may then serve as reservoirs for the later reemergence of EBOV and disease recrudescence. Here, we report atypical EVD recrudescence in a ferret model inoculated with an otherwise lethal dose of EBOV and treated with low doses of a highly potent monoclonal antibody cocktail. Among 32 antibody-treated ferrets, 14 animals survived, while 8 succumbed to acute EVD within about 5-8 days. The remaining 10 animals succ

Platelet-like particles released from Ebola virus-infected megakaryocytic cells behave like virus-like particles.

Ebola virus (EBOV) is likely a zoonotic and re-emerging virus that causes severe outbreaks of Ebola virus disease. The virus spreads to various tissues during the late stage of infection and has been detected in immune-privileged sites of survivors. However, the mechanism of how EBOV disseminates throughout the body is not completely elucidated. In this study, by using a biologically contained EBOVΔVP30 system, we demonstrate that a megakaryocytic-like MEG-01 cell line that stably expresses VP30 (MEG-01 VP30 cells) is susceptible to EBOVΔVP30 infection and that MEG-01 VP30 cells exposed to EBO

Remdesivir in COVID-19: pros and cons.

BACKGROUND: Beginning in late 2019, the COVID-19 pandemic caused by SARS-CoV-2 rapidly evolved into a global health crisis. High rates of severe illness, hospitalizations, and long-term complications highlighted an urgent need for effective therapeutic agents. This necessity drove unprecedented efforts in drug discovery and repurposing. Remdesivir, developed by Gilead Sciences in 2009, was initially designed as a broad-spectrum antiviral targeting Ebola virus disease. Following observations of broad antiviral activity against coronaviruses, remdesivir was granted Emergency Use Authorization by

Development of VP30-targeted nanoparticles using DPS4 fusion peptides for the inhibition of Ebola virus.

Multiple outbreaks of Ebola virus in West Africa have posed significant threats to global public health owing to its high pathogenicity and fatality rates. Current treatments for Ebola Virus Disease are limited, underscoring the imperative for novel antiviral therapies. VP30, a critical RNA synthesis factor, interacts with nucleoprotein (NP) to facilitate Ebola viral genome transcription and replication. Notably, the host ubiquitin-ligase retinoblastoma-binding protein 6 (RBBP6) binds to VP30 at the same interface as NP, thereby inhibiting VP30-NP interactions and indicating that targeting thi
