We assessed the impact and durability of an infection prevention and control (IPC) bundle intervention during the Kivu/Ituri Ebola virus outbreak (2018-2020). IPC scores increased, then declined 6 months postintervention (median 19/36, 30/36, and 28/36, P < .0001). Without sustained IPC, health facilities remain vulnerable to nosocomial transmission in future outbreaks.

The rise of new viral illnesses poses a substantial risk to global public health, as their incidence and impact have increased over the past few decades. This article thoroughly examines the complex connection between environmental degradation and the development of new viral diseases. Human activities, such as deforestation, urbanization, and industrial agriculture, have significantly altered ecosystems, increasing the likelihood of virus transmission from animals to humans. The alteration of natural environments brings wildlife species that serve as hosts for several viruses into closer prox

The accurate measurement of Ebola virus (EBOV)-specific antibody responses is crucial to assessing immunity induced by EBOV infection or vaccination. For this purpose, the Filovirus Animal Nonclinical Group (FANG) anti-EBOV glycoprotein (GP1,2) ELISA is considered the "gold-standard". However, it has limitations such as high repeat-rates and variability, and low throughput. Here, we describe two new alternative assays: a Single-Molecule Assay Planar EBOV GP1,2 ELISA and a multiplexed EBOV GP1,2, EBOV nucleoprotein, and EBOV Viral Protein 40 Luminex assay, and compare these with two versions of

Ancestral state reconstruction is a classical problem of broad relevance in phylogenetics. Likelihood-based methods for reconstructing ancestral states under discrete character models, such as Markov models, have proven extremely useful, but only work so long as the assumed model yields a tractable likelihood function. Unfortunately, extending a simple but tractable phylogenetic model to possess new, but biologically realistic, properties often results in an intractable likelihood, preventing its use in standard modeling tasks, including ancestral state reconstruction. The rapid advancement of

Accurate confirmation of Ebola vaccination (ERVEBO) is essential for interpreting serologic data and assessing vaccine coverage during Ebola virus (EBOV) outbreaks. Current GP1,2-based assays cannot reliably distinguish vaccine-induced immunity from responses generated by natural infection. We developed a multiplex Luminex assay incorporating EBOV GP1,2, secreted glycoprotein (sGP), and a modified vesicular stomatitis virus nucleoprotein (VSV-P-N), a vector antigen encoded by ERVEBO but absent from wild-type EBOV. By using samples from US vaccinees and controls and a small comparison set from

BACKGROUND: Healthcare systems across Africa continue to face a wide range of shocks. Using Ebola and COVID-19 as case studies, this study examines the various resilience strategies implemented in different countries and synthesises them in a way that can serve as a practical guide to make health systems in Africa more resilient for future emergencies. METHODOLOGY: The study is a scoping review that synthesises evidence from peer-reviewed journal articles and grey literature, structured around a conceptual framework developed by the authors. We focused on various aspects of health system resil

BACKGROUND: The 2018-2020 Ebola Virus Disease (EVD) outbreak in Eastern Democratic Republic of the Congo (DRC) occurred amid armed conflict, institutional mistrust, and fragile health systems. The Ebola vaccine was deployed under emergency pre-licensure use, and concerns about it persisted. This study explored community and healthcare worker (HCW) perceptions of the Ebola vaccine to better understand the sociocultural and structural drivers of vaccine acceptance.
METHODS: We conducted a qualitative study in three heavily affected health zones in North Kivu province (Beni, Butembo, and Mabalak

Ebola virus disease remains one of the most serious viral infections with no approved small-molecule treatments. The Ebola virus glycoprotein (EBOV-GP), which enables the virus's entry to host cells, is a promising target for drug discovery. In this study, a multistage computer-aided drug discovery approach was used to identify new specific EBOV-GP inhibitors. A reliable QSAR model was built using 55 terpenoid derivatives. This model was able to predict the activity of newly designed compounds with good accuracy and validated statistical metrics (Rtr2 = 0.70; Rext2 = 0.73). It was subsequently

This study investigates the potential of topological co-indices and their polynomials as computational tools for predicting the physicochemical properties of antiviral compounds targeting the Ebola virus. For an in-depth Quantitative Structure-Property Relationship (QSPR) analysis, we develop and compute topological co-indices using CoM and CoNM polynomials derived from the molecular structures of antiviral drugs, including Galidesivir, Chloroquine, Favipiravir, Amodiaquine, Azithromycin, Brincidofovir, and Clomiphene. These indices were correlated with some important experimental physicochemi

Until relatively recently, treatment for filovirus infections was limited to supportive care measures, but the occurrence of several large outbreaks in past years, including a multinational epidemic in West Africa from 2013 to 2016, has spurred the advancement of several therapeutic approaches into clinical trials, and even led to the recent licensure of two monoclonal antibody-based therapies against the Ebola virus. However, there are still significant gaps in our preparedness, in particular with respect to our ability to treat infections with other filoviruses, such as Marburg virus and Sud

Ebola virus disease (EVD) is a highly pathogenic and lethal disease caused by Ebola virus (EBOV). Endemic to Sub-Saharan Africa, EBOV has been causing global health concerns during infrequent EVD outbreaks since 1976, particularly during the 2013 -2016 epidemic in West Africa. Spread by contact of contaminated bodily fluids with mucous membranes or breaks in the skin, EBOV yields an incredibly high mortality rate and, as a result, has been designated a priority pathogen and a select agent to be studied exclusively in maximum containment laboratories. EBOV has historically been studied in anima

BACKGROUND: Ebola virus disease (EVD) remains a major global public health threat because of its high case fatality rate and recurrent outbreaks, particularly in sub-Saharan Africa. Despite advances in supportive care and targeted therapies, mortality remains substantial and varies widely across epidemics and healthcare settings. A robust quantitative synthesis is therefore needed to estimate overall mortality and identify clinical determinants associated with death among patients with EVD.
METHODS: We conducted a systematic review and meta-analysis in accordance with the PRISMA 2020 guidelin

INTRODUCTION: Patients with a suspected high-consequence infectious disease (HCID), such as Ebola virus disease, may require routine laboratory testing to guide management. We assessed the capabilities of frontline hospitals and the barriers they face performing laboratory testing for patients with a suspected HCID.
METHODS: A one-time confidential REDCap survey querying capabilities to safely perform laboratory tests that the Centers for Disease Control and Prevention considers critical for patients with a suspected HCID was sent to 95 institutions in Baltimore, MD, Boston, MA, New York City

BACKGROUND: Epidemics and disease outbreaks continue to threaten public health security in sub-Saharan Africa (SSA), disproportionately impacting impoverished and hard-to-reach populations. Although many country-specific studies exist, few syntheses have examined short-term health system responses to epidemics in hard-to-reach areas of SSA and their effects on health equity and resilience. This scoping review consolidates regional evidence on structural and policy-relevant lessons for enhancing health system preparedness and epidemic management in resource-limited settings.

INTRODUCTION AND METHODS: While Ebola virus disease (EVD) remains a major threat, recent advances have led to the approval of several medical products, in particular the rVSVΔG-ZEBOV-GP vaccine (Ervebo) and two monoclonal antibody treatments, mAb114 (Ebanga) and REGN-EB3 (Inmazeb). However, their use in post-exposure prophylaxis (PEP) remains poorly documented. Our narrative review of the literature and guidelines aims to assess the potential of the rVSVΔG-ZEBOV-GP vaccine and monoclonal antibodies mAb114 and REGN-EB3 for PEP following high-risk exposure.
RESULTS: Very few studies have been c

Ebolaviruses are known to cause severe hemorrhagic fever in humans. A new ebolavirus species, Bombali orthoebolavirus (BOMV), was recently detected in bats from Sierra Leone, Kenya, Guinea, Mozambique, and Tanzania. No infectious BOMV has been isolated from field samples, but an infectious recombinant BOMV clone (rBOMV) was recently established. To estimate disease potential in humans, we challenged cynomolgus macaques with rBOMV. Macaques exhibit hallmark features of Ebola virus disease, including vascular leak, lymphopenia, neutrophilia, and elevated inflammation markers. Consistent with the
