The Marburg virus (MARV), responsible for severe hemorrhagic fevers with mortality rates as high as 90%, remains a significant public health threat. This study employs machine learning to identify inhibitors targeting the MARV Gene 4 Small ORF protein, crucial for the virus's replication and immune evasion. The Gene 4 Small ORF protein is pivotal in taking over the host's cellular mechanisms, facilitating unchecked viral replication and significant immune system disruption. Effective targeting of this protein holds promise for mitigating the viral lifecycle and entry, potentially curbing the s

BACKGROUND: Diagnostics and laboratory testing are critical components of facilities’ systems for emergency response to infectious diseases. Yet, critical gaps exist in the testing and diagnostic capacities of faith-based health providers, particularly those in low and middle-income countries, limiting their response to health emergencies. Accordingly, the Coronavirus Disease 2019 (COVID-19) Strategic Preparedness and Response Plan for the World Health Organization African Region (1 February 2021–31 January 2022) was used to examine the capacity for laboratory and diagnostic services in a Chri

INTRODUCTION: Marburg Virus Disease (MVD) is a fatal zoonotic disease of humans and nonhuman primates caused by the Marburg virus (MARV) of the Filoviridae family, and presenting as hemorrhagic fever with a high fatality rate. Egyptian fruit bats, Rousettus aegyptiacus, are the principal natural reservoir, with evidence linking them to most human outbreaks.
METHODS: This systematic review evaluated the prevalence of MARV in bats, domestic animals, and rodents, as well as the duration of antibodies and potential routes of viral shedding. A comprehensive search of six (6) scientific databases i

Bats, often silent carriers of lethal pathogens, serve as natural reservoirs for numerous viruses capable of crossing species barriers with devastating effects. Among them, Rousettus aegyptiacus (R. aegyptiacus) has emerged as a pivotal species in understanding viral spillovers. This fruit bat hosts the Marburg virus (MARV), supporting its replication and occasional transmission to humans, where it causes fatal hemorrhagic fever. However, a striking paradox remains while MARV infection is deadly in humans, R. aegyptiacus remains asymptomatic. Driven by this contrast, our study explores the cod

OBJECTIVE: This scoping review aims to map and synthesize the evidence on key challenges, facilitators, and recommendations related to the implementation and community acceptance of safe and dignified burials (SDBs) during outbreaks of Ebola disease (EBOD) or Marburg virus disease (MVD).
INTRODUCTION: SDBs are a critical public health intervention designed to reduce post-mortem transmission of EBOD and MVD-two contagious and often fatal diseases-while upholding cultural and religious practices. Despite their importance, challenges to implementation and acceptance have been documented, which m

BACKGROUND: To date, 18 Marburg virus disease (MVD) outbreaks have been reported, most recently in Tanzania and Ethiopia. From 1998 to 2000, a large MVD outbreak in Watsa/Durba region, Democratic Republic of the Congo (DRC), resulted in 154 cases and 128 deaths. Given the public health risks posed by MARV, the lack of approved vaccines and therapeutics, and unpredictability of future outbreaks, this study aimed to assess the anti-MARV seroprevalence in Watsa/Durba.
METHODS: Samples collected in May 2023 (n = 313) included known MVD survivors (n = 5), close contacts (n = 22), and other residen

In November 2025, Ethiopia confirmed its first outbreak of Marburg virus disease in South Omo Zone, marking a critical public health emergency in a previously unaffected country. The outbreak was detected following reports of suspected viral hemorrhagic fever in Jinka town, with laboratory confirmation achieved within days through national reference laboratory testing. By mid-December 2025, 14 laboratory-confirmed cases had been identified, including 9 deaths and 5 recoveries, following nearly 2 thousand investigations nationwide. This report describes the detection of the outbreak, early epid

Africa remains the epicenter of filovirus outbreaks, including Ebola virus disease (EVD) and Marburg virus disease (MVD), which continue to expose systemic weaknesses in epidemic preparedness and response systems. Despite the existence of national and regional policies, these deadly diseases continue to emerge across the continent, underscoring the urgent need for integrated strategies that align governance, policy, and health security innovation to strengthen resilience and mitigate the impact of future outbreaks. We reviewed the trajectory of EVD and MVD outbreaks in Africa from 1975 and 202

BACKGROUND: Marburg virus disease (MVD) is a rare but highly fatal zoonotic illness. Current serological tools are limited by their reliance on glycoprotein (GP)-based antigens or Marburg virus infected cell lysates, which cannot distinguish natural infection induced immune responses from GP-based vaccines. This diagnostic gap complicates the detection of missed cases particularly in areas of high vaccination coverage.
METHODS: We developed and validated two complementary assays: a nucleoprotein C-terminal tail (NPct) Mix-and-Read (MR) assay and a recombinant GP1,2ΔTM ELISA. Serum samples use

BACKGROUND: The frequency and scale of viral disease outbreaks are increasing globally, with communities in sub-Saharan Africa disproportionately affected due to structural, social, and health-system vulnerabilities. Community knowledge, attitudes, and perceptions (KAP) play a critical role in outbreak prevention, control, and response. This study assessed KAP related to viral disease outbreaks among adults in Dar es Salaam, Tanzania, following recent outbreaks, including COVID-19, Mpox, and Marburg virus disease.
METHODS: A cross-sectional study was conducted among 104 adult visitors attendi

The recent outbreak of the Marburg virus (MARV) in Rwanda was effectively controlled, but concerns remain regarding the virus's persistence and its long-term effects. MARV can remain in the testes of nonhuman primates, where it harms Sertoli cells and compromises the blood-testis barrier, which may have implications for male infertility. This persistence is aided by immunosuppressive regulatory T cells (Tregs), which allow the virus to evade the immune system. Drawing comparisons with Ebola and Zika viruses, we propose that MARV may contribute to male infertility and recommend further studies

Understanding zoonotic spillover requires observation of reservoir-host interfaces, yet direct multi-species behavioral data remain rare. During long-term carnivore monitoring in Queen Elizabeth National Park, Uganda, we deployed camera traps at Python Cave, a roost for Egyptian fruit bats (Rousettus aegyptiacus) and known Marburg-virus reservoir. Over the course of 368 trap nights, we recorded > 14 vertebrate species engaging in predation, scavenging, guano foraging, or exploration across 321 events, alongside 214 human visitors. The shallow cave structure collapses spatial separation between

Marburg virus (MARV) causes Marburg virus disease (MVD), a severe hemorrhagic fever associated with high morbidity and mortality. No specific antiviral therapies are currently approved for MVD, and viral pathogenesis, particularly the viral pathogenic factor(s) involved, remains poorly defined. Here, we demonstrated that MARV glycoprotein (GP) can induce rounding and detachment of adherent cells, likely by shielding and downregulating cell surface molecules, mainly via its mucin-like domain (MLD), disrupting monolayer barrier function. In an ex vivo rat vascular model, GP expression caused sig

The 2023 Marburg virus disease (MVD) outbreak in Tanzania, which resulted in nine confirmed cases and six deaths (case fatality rate [CFR]: 66.7%), served as a critical stress test of the national health system, revealing significant gaps in outbreak preparedness across surveillance, laboratory capacity, and infection prevention and control (IPC). This narrative review examines how Tanzania's public health response evolved between the 2023 and subsequent 2025 outbreak and assesses how response strategies changed between the two outbreaks. The 2025 outbreak was limited to two confirmed cases wi

Marburg virus disease remains a major threat to global health security in Africa, where recurrent filovirus outbreaks intersect with fragile health systems. In 2024, Rwanda faced its first Marburg virus disease outbreak, yet achieved an unusually low case-fatality rate of 23%, in contrast to the high mortality reported in the Democratic Republic of the Congo (83%), Angola (90%), Uganda (60%), and Tanzania (100%). In this Health Policy paper, we summarise the key lessons from Rwanda that can inform the responses to current and future Marburg virus disease outbreaks in Africa. Using a narrative

Marburg Virus Disease (MVD) is a highly lethal viral hemorrhagic fever caused by the Marburg virus, a member of the filovirus family closely related to Ebola. Characterized by sudden onset of fever, hemorrhage, and multi-organ failure, MVD outbreaks pose significant public health challenges due to high mortality rates and limited medical countermeasures. The 2024 outbreak in Rwanda marked a critical point, with 66 confirmed cases and 15 fatalities, highlighting the urgent need for effective management strategies. This narrative review, reviews key lessons from Rwanda's response, emphasizing th

BACKGROUND: An outbreak of Marburg virus in Jinka Town, Southern Ethiopia, has raised significant concern regarding the potential spreading of disease throughout the country. Healthcare workers play a crucial role in early prevention and control of such an outbreak. However, the knowledge, attitudes, and preparedness of healthcare workers regarding Marburg virus disease have not been assessed yet, despite these factors being critical for early prevention and control of an outbreak. This study aimed to assess the knowledge, attitude, and preparedness regarding Marburg virus disease among health

BACKGROUND: Marburg virus disease (MVD) is a rare but highly lethal viral hemorrhagic fever (VHF) caused by Marburg virus (MARV) and Ravn virus (RAVV). While MVD has historically been limited to specific areas of sub-Saharan Africa, recent outbreaks in previously unaffected countries indicate an expanding ecological and epidemiological risk. In November 2025, Ethiopia confirmed its first-ever MVD outbreak, constituting a significant national and regional public health emergency.
METHODS: This narrative review synthesizes publicly available epidemiological data, government situation reports, a
