Gracilis Muscle Transposition for Recurrent Rectovaginal Fistula.

STUDY OBJECTIVE: To demonstrate a gracilis muscle transposition technique for repair of recurrent RVF in collaboration with plastic surgery. DESIGN: Stepwise demonstration with narrated video footage of the above technique. SETTING: Primary rectovaginal fistula (RVF) repair can fail. Several options exist to facilitate repair of recurrent RVF, including Martius flap, endorectal advancement flap, and gracilis muscle transposition, which gynecologists may perceive as prohibitively complex [1-4]. Collaboration with plastic surgery for gracilis muscle transposition is a key tool for unlocking th

Rare submucosal abscess following endoscopic mucosal resection for rectal polyps: a case report and literature review.

Colonoscopy combined with polypectomy is a well-established strategy for the prevention of colorectal cancer, and endoscopic mucosal resection (EMR) serves as one of the primary therapeutic modalities for colorectal neoplastic lesions. While EMR is generally safe, severe complications may occasionally occur. Herein, we report a rare case of rectal submucosal abscess in a 74-year-old female patient who underwent EMR for rectal polyps. Initial endoscopic fenestration and drainage of the abscess failed to achieve a satisfactory therapeutic effect, and the patient ultimately underwent sigmoidostom

Comparing Preoperative Healthcare Utilization Between Immigrants and Non-Immigrants Undergoing Emergency General Surgery in British Columbia.

Immigrants to Canada face unique barriers that result in them accessing publicly funded healthcare services differently than non-immigrants. These barriers are particularly critical in the context of Emergency General Surgery (EGS) where timely access to healthcare is essential for better health outcomes. This study compares preoperative healthcare utilization between immigrants undergoing EGS with non-immigrants. EGS procedures in British Columbia between 2013 and 2021 were analyzed using population-based linked immigration, physician billing, and hospital discharge summary data. Preoperative

Anorectal Tuberculosis Presenting With Multiple Abscesses and Fistulas in a Young Adult Male Patient With Alcohol Use Disorder: A Case Report.

Gastrointestinal tuberculosis (TB) is an uncommon form of extrapulmonary TB, with anorectal involvement representing an exceedingly rare subset. We report a fatal case of disseminated anorectal TB in a 28-year-old male immigrant from Kenya with a history of alcohol use disorder, a recognized risk factor for TB reactivation and spread. The patient initially presented with rectal pain and perirectal purulent discharge. On evaluation, he was found to have a left perirectal abscess, a right ischioanal abscess, and multiple fistulas. Acid-fast bacillus staining of debrided anorectal abscess fluid r

Recurrent vaginal squamous cell carcinoma mimicking peri-rectal abscess: The role of endoscopic ultrasound.

Recurrence of vaginal squamous cell carcinoma (SCC) involving the rectum is extremely rare and usually results from direct spread of the primary tumor. Such cases often present with pelvic metastases and can mimic infectious or inflammatory conditions, complicating diagnosis and delaying treatment. While computed tomography scans are commonly used for diagnosis, they may be misleading. Endoscopic ultrasound (EUS) is crucial for accurately assessing rectal and perirectal lesions. We present the case of a 60-year-old female with a history of vaginal SCC who presented with refractory constipation

Case Report: A case of perirectal abscess complicated with rectal necrosis.

Rectal necrosis represents the extreme progression of ischemic proctitis. Given the unique blood supply of the rectum, which features an extensive collateral circulation, cases of rectal necrosis are extremely rare, accounting for 2%-5% of the ischemic colitis cases. The etiology and pathogenesis of rectal necrosis encompass acute vascular occlusion, severe vascular diseases, low blood flow states, and factors such as radiotherapy, vasculitis, and mesenteric venous myointimal hyperplasia. Herein, we report a rare case of perirectal abscess complicated by rectal necrosis at the Anorectal Surger

Perianal Injury Prevention in Patients With Leukemia.

BACKGROUND: Perianal injuries in adults with hematologic malignancies can result in sepsis or death. Patients on a 36-bed acute care oncology unit experienced increased perianal injuries from 2018 to 2021 based on a chart review, which revealed that 24 patients with leukemia, all of whom had received cytarabine, developed perianal injuries. OBJECTIVES: This study examined whether a nurse-led educational intervention would decrease perianal injuries. METHODS: A survey examined baseline nurse knowledge on perianal injuries. Nurses received a 10-minute in-service education session about periana

Anorectal Infections in Neutropenic Leukemia Patients: A Common Clinical Challenge.

Anorectal infections in neutropenic leukemia patients are a significant and potentially life-threatening complication. The pathogenesis of this condition is not entirely understood and believed to be multifactorial, including mucosal injury as a result of cytotoxic drugs, profound neutropenia and impaired host defense. Establishing an early diagnosis is key and often made clinically on the basis of signs and symptoms, but also from imaging studies demonstrating perianal inflammation or fluid collection. The management of anorectal infections in neutropenic leukemia patients is not straightforw

Complex Tunneling Perirectal Abscess: Intra-abdominal and Extraperitoneal Extension of a Persistent Perirectal Abscess.

Deep-tissue extension of perianal and perirectal abscesses, while rare, requires timely diagnosis and emergent surgical intervention to prevent serious secondary complications. This report evaluates a case of intra-abdominal and extraperitoneal extension of a persistent perirectal abscess that required comprehensive irrigation, drainage, and debridement of multiple abscess-associated cavities. This report follows the case of a 24-year-old African-American female presenting to the ED with mild fevers, nausea, abdominal distension, and lower abdominal pain following a persistent perirectal absce

Endoluminal Vacuum Therapy (EVT) for the Treatment of Rectal Perforation Following Cleansing Enema Application.

Traumatic rectal injuries (TRIs) are challenging for surgeons because of their high morbidity and mortality. Considering the well-known predisposing factors, enema-associated rectal perforation seems to be the most overlooked entity that leads to devastating rectal injuries. A 61-year-old man with a three-day history of painful swelling around his perirectal area after enema application was referred to the outpatient clinic. CT demonstrated the presence of a left posterolateral rectal abscess compatible with an extraperitoneal rectal injury. Sigmoidoscopy revealed the perforation started 2 cm

Pneumorrhachis Secondary to a Locally Advanced Rectal Cancer with Pre-Sacral Abscess-Case Report and Review.

The occurrence of pneumorrhachis (PR), defined as the presence of air within the spinal canal, presents a complex clinical picture with diverse etiological factors. We report an exceedingly rare case of PR arising from locally advanced rectal cancer accompanied by a pre-sacral abscess. This report aims to enhance awareness and understanding of rare causes of PR within the medical community, particularly among surgeons engaged in emergency procedures. The patient survived the acute phase of the disease through multiple surgical interventions and admission to the intensive care unit, but succumb

Antimicrobial neurotoxicity: an under-recognised cause of delirium.

Antimicrobial associated encephalopathy (AAE) is a well-documented, though under recognised, adverse event associated with antimicrobial use. Clinical manifestations of AAE are varied, ranging from myoclonus and seizure to an encephalopathy with cerebellar signs. The phenotypic presentation of the encephalopathy syndrome is, in general, governed by the antimicrobial in question. Given its apparent rarity in everyday clinical practice, awareness of AAE is crucial for physicians. We describe a reversible encephalopathy characterised by confusion, myoclonus and stupor in a 76 year old gentleman o

Anesthetic Considerations in Patients With Achondroplasia.

Anesthetic management of achondroplastic patients warrants special anatomical and physiological considerations due to significant variations in the airway as well as the spine in regional techniques. In this report, we present the case of a 30-year-old morbidly obese male with achondroplasia, end-stage renal disease (ESRD) on hemodialysis, and renal osteodystrophy, who was scheduled for incision and drainage of a rectal abscess. Preoperative evaluation revealed Mallampati IV airway with a short neck and a scoliotic spine with possible atlantoaxial instability.

Extraintestinal Clostridioides difficile Infections: Epidemiology in a University Hospital in Hungary and Review of the Literature.

Extraintestinal manifestations of Clostridioides difficile infections (CDIs) are very uncommon, and according to the literature, poor outcomes and a high mortality have been observed among affected individuals. The objective of this study was to investigate the incidence rate of extraintestinal infections caused by C. difficile (ECD) in a tertiary-care university hospital in Hungary. During a 10-year study period, the microbiology laboratory isolated 4129 individual strains of C. difficile; among these, the majority were either from diarrheal fecal samples or from colonic material and only n =

Inadequate Antibiotic Therapy Results in Higher Recurrence Rate after Drainage of Complicated Peri-Rectal Abscess.

Background: Recommended treatment for complicated peri-rectal abscess is incision and drainage (I&D) in conjunction with antibiotics. However, there is no standard antibiotic regimen for post-operative therapy described in the published literature. Our hypothesis was that appropriate post-operative antibiotic therapy after emergency I&D of complicated peri-rectal abscess will improve patient outcomes. Methods: Data from 58 patients with complicated peri-rectal abscess who underwent emergency I&D were analyzed retrospectively. Demographic, microbiologic, and antibiotic data were abstracted. Ade
