The management of anorectal abscess: An inexpensive and simple alternative technique to incision and "deroofing".

All patients with anorectal abscess who were referred to the University Department of Surgery in Wellington have been managed by simple "de Pezzer" drainage since 1981. The catheter used was between 3 to 5 mm in diameted and was usually inserted under a local anesthetic. The method is described in detail. One hundred and fifty-seven patients have presented with anorectal abscess, and after exclusion of those patients with intersphincteric abscess, 140 have been managed using de Pezzer drainage. The male to female ratio was 2.7:1. A perianal abscess was present in 120 patients. General anesthes

Surgical management of anorectal infection in the leukemic patient.

Anorectal infections are a common complication in immunocompromised patients. Such infections are multifactorial in etiology, often promoted by breaks in the anorectal mucosa and depressed leukocyte counts, both secondary to chemotherapy. Because of altered immune response to infection, diagnosis of anorectal infections in leukemic patients is often delayed, with a resultant mortality of up to 78%. Management is typically supportive for fears of extension of infection or non-healing of wounds with surgical intervention. We recently encountered a 17-year-old woman who presented with an anorecta

[Abscess excision and primary fistulectomy as initial therapy of peri-proctal abscess. A prospective analysis of 122 patients].

A prospective study of 122 patients with anorectal abscesses was carried out to elucidate the value of abscess drainage with primary fistulectomy, when fistulas were identified. Patients were followed from 2 1/2 to 3 1/2 years with a mean follow-up of three years. In 52 patients (Group A) a primary fistula was present (42.6%), in 47 patients simultaneous excision of abscess and drainage of the intersphincteric space combined with partial sphincterotomy was performed. Recidives occurred in 17.3%. In 5 patients with a high transsphincteric or suprasphincteric fistula, a two-stage fistulectomy wa

Management of anorectal abscesses: comparison between traditional incision and packing and de Pezzer catheter drainage.

This study compares the results of de Pezzer catheter drainage of anorectal abscess with the more traditional technique of incision and packing. A prospective study from this department in 1987 detailed the outcome of patients with anorectal abscess managed routinely by de Pezzer catheter drainage. Ninety-one patients were treated, 18 required general anaesthesia, the mean hospital stay was 1.4 days, and 24% subsequently required fistula surgery. A retrospective study has now been undertaken of all patients admitted to Wellington Hospital with anorectal abscess during the same period who were

[Experience on the treatment of acute anorectal abscess with primary fistulotomy].

From August 1985 to July 1989, 72 cases with initial attacks of acute cryptoglandular anorectal abscesses were hospitalized and treated in Kuang Tien General Hospital. Among them were 52 cases of the perianal type, 8 of the intersphincteric type, 11 of the ischiorectal type and one case involving the supralevator space. Included were 62 males and 10 females (sex ratio 6:1) with a mean age of 39 years (range, 18 to 63). The mean duration of the follow-up was 24 months. Most anorectal abscesses originate from a cryptoglandular infection and are usually treated initially by incision and drainage.

Anorectal complications of vaginal delivery.

The incidence of anorectal complications following vaginal delivery was studied in 20,500 women. One thousand forty (5 percent) of all normal vaginal deliveries resulted in episiotomy with third- and fourth-degree extension or a fourth-degree perineal tear. Of these fourth-degree lacerations, 101 patients (10 percent) experienced wound disruption after primary repair. Sixty-seven patients (66 percent) experienced wound disruption that required surgical correction. Anorectal complications were anal ulcer, anorectal abscess, sphincteric disruption, and rectovaginal fistula. Surgical correction o

Corynebacterium group JK in a hematological ward: infections, colonization and environmental contamination.

Because 2 patients with acute leukemia expired in septicemia with multiresistant Corynebacterium group JK (JK) the occurrence and significance of these bacteria in a hematological ward was analysed. During the following year JK was isolated in 6 other patients with acute leukemia, in 5 as a colonizing agent and in 1 as cause of an anorectal abscess. The environmental investigation with cultures from all patients, personnel and rooms in the ward disclosed heavy contamination with JK in 2 isolation rooms housing JK-colonized patients. Contamination with JK in other rooms was very sparse and ther

Anorectal abscess and fistula-in-ano.

The etiology of anorectal abscess and fistula-in-ano is discussed. The anatomy, which is vital to the understanding and treatment of the above, is reviewed, with two of the more common classifications of fistula-in-ano presented. The different methods of treating each are discussed, and some of the common complications of the procedure are listed. A true understanding of the disease process and anatomy is needed before treatment of fistulous abscesses is begun, but, with it, successful outcomes will occur in most cases.

Primary partial internal sphincterectomy in the treatment of anorectal abscess.

Anorectal abscess is usually treated by simple incision and drainage. Many patients treated in this way will develop a recurrence of the abscess or a chronic anal fistula. In order to prevent such a recurrence partial internal sphincterectomy has been added to the incision and drainage. The results with this so-called primary partial internal sphincterectomy in 74 patients were excellent, because of absence of recurrent abscess or persistent fistula.

Incision and drainage v. incision, curettage and suture under antibiotic cover in anorectal abscess. A randomized study with 3-year follow-up.

Conventional incision and drainage was compared with incision plus curettage and primary suture of abscess cavity under antibiotic cover in a prospective, randomized trial of 83 patients with acute anorectal abscess with or without low fistula. All the patients were followed up for three years. The time to healing was on average three weeks less after suture than after incision alone. The difference was statistically significant. Primary healing was obtained in 32 of 42 cases after suture. Recurrence of abscess tended to be more frequent after suture, but the time to healing of initial and rec

[Recurrent anorectal abscesses].

A study of 78 patients with recurrent anorectal abscess was carried out to elucidate the cause of recurrence. The inadequate treatment (incision and drainage) and the spontaneous rupture or resolution of the abscess with no further treatment were the most common reasons for recurrence. At surgery, 18 patients were found to have recurrent hidradenitis suppurativa. The authors recommend a careful regional exploration under anesthesia and an aggressive surgical approach to achieve satisfactory results and to ensure minimal, if any recurrence.

Tetanus after anorectal abscess.

As illustrated by this case report, tetanus can occur as a complication of anorectal surgical procedures or abscesses just as it can in other wounds. A synergistic infection of the perineum occurred in a 62-year-old man 8 days after drainage of an anorectal abscess. He was treated with vigorous debridement and antibiotics and was given tetanus prophylaxis. The next day, tetanus developed, presumably from the original abscess. The patient recovered after aggressive therapy, including muscle relaxants and ventilatory support.

Perianal abscesses and fistulas. A study of 1023 patients.

In a five and one-half year period, 1023 patients with anorectal abscesses and fistulas were treated. Under regional anesthesia the abscesses were unroofed and debrided and a primary fistulotomy was performed whenever a low fistula was identified. In 355 (34.7 per cent) an internal fistulous opening was demonstrated at the time of abscess drainage. Thirty-two patients had suprasphincteric fistulas and underwent two-stage fistulotomy using a seton. Perianal abscesses were encountered in 42.7 per cent of the patients, followed by ischiorectal (22.7 per cent), intersphincteric (21.4 per cent), an

[Opening of anorectal abscesses--with or without splitting of the fistula?].

An anorectal abscess has to be deroofed. If, in addition, a fistula is present, it is our policy (in a teaching institution with surgeons in training) to incise it only if easily visualized and situated up to the dentate line. Of 79 anorectal abscesses drainage alone was performed in 55 (group 1) and drainage with fistulatomy in 24 (group 2). One-third of the patients in group 1 needed a secondary fistula operation, but most of the others were free of symptoms after one intervention. In group 2 all but one of the patients were cured by the combined procedure. Incontinence was observed in none.

Anorectal disorders.

Common anorectal disorders can produce one or more of the following symptoms: pain, protrusion, pruritus, bleeding or discharge. The cause may be cryptitis, anal fissure, hemorrhoids, anorectal abscess, fistula in ano, neoplasm, condylomata acuminata, pilonidal cyst or inflammatory bowel disease. Each disorder is suggested by its characteristic history. Adequate examination by proctosigmoidoscopy will usually confirm the presence of the suspected disease.
