Recurrence After Rectopexy: Insights From Magnetic Resonance Defecography.

BACKGROUND: Rectal prolapse recurrence after surgical repair remains a major limitation. Prior research surrounding recurrence risk has yielded inconsistent results and assessed a limited range of factors. OBJECTIVE: To evaluate whether preoperative magnetic resonance defecography, anorectal manometry, or balloon expulsion testing could identify biomarkers associated with rectal prolapse recurrence after rectopexy. DESIGN: Retrospective cohort study from a prospectively maintained rectal prolapse registry. SETTINGS: Single tertiary institution analysis. PATIENTS: A total of 155 patients (1

Biomechanical characterization of the female rectum in vivo through MRI-based inverse finite element analysis.

Rectal prolapse is a pelvic floor disorder characterized by pathological descent of the rectum resulting from impaired pelvic support. Understanding the rectum's in vivo biomechanical behavior is essential for improving diagnosis and treatment planning. This study aimed to estimate the in vivo mechanical properties of the rectum using inverse finite element analysis (FEA) driven by magnetic resonance imaging (MRI) data. MRI scans from six women (three with rectal prolapse and three asymptomatic controls) were analyzed to quantify supero-inferior (SI) and antero-posterior (AP) rectal displaceme

The tack technique for robotic ventral mesh rectopexy: An useful, safe and effective trick using the Hugo RAS™ system.

TECHNIQUE: We describe our standardized technique for mesh fixation during robotic ventral rectopexy performed with the Hugo RAS™ system. A non-absorbable helical tack device, introduced by the assistant through a robotic trocar, is used to secure the mesh to the anterior surface of the S1 vertebral body. Particular attention is paid to the preservation of critical anatomical structures, including the hypogastric nerves and sacral foramina. This approach, widely adopted in conventional laparoscopy, is reproducible and feasible in the robotic setting with the Hugo RAS™ platform. The system's op

Advanced Colorectal Surgery with a Newly Approved Japanese Robotic Surgical Platform in Singapore.

BACKGROUND: Robotic-assisted colorectal surgery has matured over the past two decades, driven predominantly by the well-known robotic technology developed in the United States. New evidence also demonstrates improved postoperative recovery, lower conversion rates, and improved oncological resection margins over laparoscopy. However, high acquisition and maintenance costs and a bulky infrastructure have limited global adoption. The recent expiry of a significant number of core foundational intellectual property patents related to USA technology marked a pivotal shift especially in 2019 and redu

Cost-effectiveness and patient-reported outcomes of combined versus isolated robotic rectal and pelvic organ prolapse repair: A multicentre prospective cohort study.

AIM: Rectal prolapse (RP) and pelvic organ prolapse (POP) frequently coexist, yet are often evaluated and treated separately. This study aimed to compare patient-reported outcomes and cost-effectiveness of combined versus isolated robotic prolapse repair. METHODS: We conducted a prospective multicentre cohort study across three tertiary pelvic floor centres. Women undergoing robotic rectal prolapse repair (rRP), robotic pelvic organ prolapse repair (rPOP) or combined robotic repair (rRP + rPOP) were included. Validated patient-reported outcome measures were collected at baseline and 12 months

Transvaginal single-pathway composite reconstruction for pelvic organ prolapse combined with rectal prolapse: a case report.

Concomitant pelvic organ prolapse (POP) and rectal prolapse (RP) is a relatively rare and often underrecognized clinical condition, typically managed as two separate entities within a single-disciplinary framework. Historically, transvaginal mesh (TVM), although widely used in pelvic floor reconstruction, has not been applied to treat RP. We report a 76-year-old female with severe POP and concomitant RP. Guided by the integral theory of pelvic floor, we innovatively cut and assembled TiLOOP Total 4 mesh into Total 6 configuration, connected it to a trimmed TiLOOP mesh (10 × 15 cm), and perform

Bacteroides-associated NAD⁺ depletion correlates with exacerbated radiation-induced colorectal injury and impaired mucosal proliferative capacity.

Radiation proctitis (RP) is a frequent complication of pelvic radiotherapy that compromises treatment delivery and patient quality of life, yet the factors shaping injury severity remain incompletely defined. We prospectively profiled pretreatment fecal microbiomes and metabolomes from 55 patients and stratified them by outcome into mild versus severe RP. Baseline microbial composition showed Bacteroidales enriched in severe RP and Firmicutes enriched in mild cases. Multi-omics integration highlighted nicotinate/nicotinamide pathways; severe RP was characterized by concomitant reductions in bo

Case Report: Fascia lata transposition for chronic recurrent perineal hernia followed by laparoscopic colopexy for subsequent rectal prolapse in a dog.

This case report describes a surgical management of chronic, recurrent, complicated perineal hernia following bilateral herniorrhaphy in a 6-kg castrated male mixed-breed dog, estimated to be over 10 years old. The patient presented with recurrent right-sided perineal hernia and constipation associated with rectal sacculation. During the herniorrhaphy attempted, severe atrophy of pelvic musculature was identified; therefore, a fascia lata transposition was performed. Postoperatively, the dog developed a rectal prolapse, which was successfully addressed through laparoscopic colopexy. The dog re
