Single cells and spatial RNA profiling reveal that inflammatory fibroblasts arise from Edil3 stromal cells in the colon after irradiation.

Patients with cancer who are treated for tumors located in the abdominopelvic region may develop radiation proctitis. This condition is characterized by severe mucosal inflammation that can significantly impair quality of life. In the colon, it's now established that the stroma orchestrates epithelial, endothelial, and immune cell homeostasis and plays a pivotal role following an injury. A more comprehensive understanding of the underlying mechanisms responsible for digestive mucosa injury and regeneration processes would enable therapeutic targets to be identified for limiting or even treatin

Deep learning-based automatic segmentation of rectal tumors in endoscopic images.

PURPOSE: Endoscopy is critical in the identification of rectal tumors, but is prone to observer errors. The aim of this study was to assess the inter- and intra-observer variability in delineating rectal lesions in endoscopic images taken during high-dose-rate (HDR) brachytherapy and develop a deep learning-based automatic tumor segmentation model. MATERIALS AND METHODS: Three expert annotators identified tumors, scaring, ulcers and radiation proctitis in 801 endoscopic images from 24 patients. Inter-observer variability was evaluated at both whole-image and contour levels. Intra-observer var

Incidence and predictors of radiation proctitis or radiation cystitis after radiotherapy in patients with prostate cancer.

INTRODUCTION: This study evaluated the cumulative incidences of radiation cystitis (RC) and radiation proctitis (RP) after radiotherapy (RT) for localised and metastatic prostate cancer (PCa). It also analysed potential risk factors associated with resultant emergency admissions. METHODS: Between January 2001 and January 2023, 762 patients who underwent primary RT for localised PCa and low-volume metastatic hormone-sensitive PCa, as well as salvage or adjuvant RT after prostatectomy, were retrospectively analysed. The severities of RC and RP were assessed using the Common Terminology Criteria

Habitat heterogeneity analysis of planning CT improves the prediction of radiation proctitis in cervical cancer: A multimodal machine learning study.

PURPOSE: Radiation proctitis (RP) is a common complication following radiotherapy for cervical cancer, for which accurate pre-treatment prediction remains challenging. This study aimed to develop and validate a multimodal machine learning model that incorporates habitat heterogeneity analysis from planning CT images to predict RP risk at the individual level. METHODS: In this retrospective study, 100 patients with cervical cancer treated with radiotherapy were enrolled (53 RP-positive, 47 RP-negative). A comprehensive dataset of 2783 multimodal features was collected, encompassing sociodemogr

An injectable Ce-MnCo LDH nanozyme gel with cascade catalytic activity for acute radiation proctitis in rats.

Acute radiation proctitis (ARP) is a common complication of pelvic radiotherapy driven by direct DNA damage and excessive reactive oxygen species (ROS) induced oxidative stress. Currently, effective clinical treatments that can systematically intervene in ARP are lacking. In this study, we developed an injectable, multicenter cascade nanozyme gel (Ce-MnCo LDH gel) for intrarectal administration to combat radiation-induced intestinal injury. By atomically integrating cerium (Ce) into the lattice of manganese-cobalt layered double hydroxides (MnCo LDH), we constructed a complex multicenter elect

Feasibility and safety of laparoscopic versus open repair for rectobulbar fistula in anorectal malformations: A systematic review and meta-analysis.

BACKGROUND: Laparoscopic-assisted Anorectoplasty (LAARP) for male recto-bulbar fistula (RBF) remains debated, with early reports describing approach-specific complications. Prior meta-analyses pooled heterogeneous Anorectal malformation (ARM) subtypes, limiting conclusions for RBF. We conducted an RBF-specific systematic review and meta-analysis comparing LAARP with posterior sagittal anorectoplasty (PSARP). METHODS: PubMed/MEDLINE, Embase, Scopus, and Google Scholar were searched (2000-November 2025) for comparative studies of LAARP versus PSARP in male children with RBF. Risk of bias was as

Innate immune activity in radiation Proctitis.

BACKGROUND AND STUDY AIMS: Radiation proctitis is a common and morbid complication of pelvic radiotherapy, yet its pathophysiological mechanisms remain incompletely understood. Preclinical models suggest that activation of the innate immune system, particularly the NF-kappaB signaling pathway, plays a key role in regulating intestinal responses to ionizing radiation. This study aimed to investigate the activation of innate immune responses in rectal mucosa during prostate radiotherapy and examine correlations with the development of acute proctitis. PATIENTS AND METHODS: A prospective observa

Spontaneous transanal evisceration of the small bowel through perforation of the sigmoid colon: a case report and new surgical approach.

INTRODUCTION AND IMPORTANCE: Transanal evisceration of the small intestine is a rare surgical emergency that requires immediate intervention. Since its initial description in 1827, only three cases of evisceration secondary to perforation of the sigmoid colon have been documented. This case report describes the fourth case worldwide. PRESENTATION OF CASE: An 85-year-old woman presented with transanal small bowel evisceration after straining during defecation. Examination revealed prolapsed, congested but viable small bowel loops through the anal canal, without uterine or rectal prolapse. Emer

Bilateral Pathological Femoral Fractures Secondary to Long-Term Tenofovir Disoproxil Fumarate-Induced Osteomalacia in a Woman Living with HIV: A Case Report.

BACKGROUND: People living with HIV on long-term antiretroviral therapy (ART) face increased risks of bone loss and fragility fractures, partly due to tenofovir disoproxil fumarate (TDF), a commonly used nucleoside reverse transcriptase inhibitor. TDF is associated with proximal tubular dysfunction, renal phosphate wasting, osteomalacia, and pathological fractures. This case highlights severe, bilateral femoral fractures as a rare but debilitating manifestation of chronic TDF-induced osteomalacia in a postmenopausal woman. CASE PRESENTATION: A 43-year-old African woman with HIV (diagnosed >15

Macro-anatomical, histological, and ultrastructural investigation of glandulae anales in Anatolian ground squirrels.

In this study, the anal glands of 10 adult Anatolian ground squirrels (Spermophilus xanthophrymnus) were examined through dissection, scanning electron microscopy (SEM), and light microscopy. The anal glands consist of three independent complexes positioned ventrolaterally to the anus. These glandular complexes were connected with the anal canal, the rectum, and with each other through muscular and connective tissue layers coursing in multiple directions. Each anal gland complex consisted of a glandular component and a sac-like structure. The sac structures of the anal gland complexes, which c

Life-threatening lower gastrointestinal bleeding after routine anorectal procedures: a case series and management implications.

BACKGROUND: This case series characterizes enema- and catheter-induced rectal injuries as an underrecognized iatrogenic cause of life-threatening lower gastrointestinal bleeding (LGIB). By focusing on elderly, high-risk patients, we highlight the diagnostic complexities and management strategies associated with these routine yet potentially catastrophic anorectal procedures. METHODS: We analyzed three cases of high-risk elderly patients who experienced acute, massive hemorrhage following routine anorectal procedures, specifically glycerin enemas and anorectal manometry. To contextualize these

Integrated intraoperative strategies to prevent anastomotic leakage after laparoscopic low anterior resection: The roles of left colic artery preservation, reinforcement suturing, and transanal tube placement.

Anastomotic leakage is a major complication following laparoscopic low anterior resection in patients with low rectal cancer. This study investigated the preventive effects of three intraoperative techniques, including the preservation of the left colic artery, reinforcement suturing, and transanal decompression tube placement. A total of 88 patients undergoing laparoscopic rectal cancer surgery were retrospectively analyzed. Univariate and multivariate logistic regression analyses were performed to assess the associations between these interventions and the occurrence of anastomotic leakage.

Visceral technetium-99 m-labeled pyrophosphate uptake in patients with wild-type transthyretin cardiac amyloidosis.

OBJECTIVES: Technetium-99 m-labeled pyrophosphate (Tc-99 m-PYP) scintigraphy can image tracer uptake in the myocardium, subcutaneous fat, and skeletal muscles of patients with wild-type amyloid transthyretin (ATTRwt) cardiac amyloidosis (CA), which indicates ATTR deposition. However, there are few reports of visceral Tc-99 m-PYP uptake in patients with ATTRwt-CA. This study was conducted to determine whether Tc-99 m-PYP scintigraphy can image ATTR deposition in the viscera, particularly the liver, spleen, and gastrointestinal tract. METHODS: Chest- and abdomen-centered Tc-99 m-PYP scintigraphy

Detection and surgical intervention in anorectal malignant melanoma: a case report.

BACKGROUND: Anorectal malignant melanoma (ARMM), a rare and highly aggressive subtype of mucosal melanoma, accounts for < 0.1% of all colorectal malignancies. Characterized by nonspecific clinical manifestations and the absence of early diagnostic biomarkers, ARMM is frequently diagnosed at advanced stages, resulting in a dismal prognosis with a 5-year survival rate below 20%. Despite recent advances in immunotherapy and targeted therapies, critical knowledge gaps persist regarding its pathogenesis, standardized therapeutic protocols, and prognostic prediction models.
