Scoping review of indications for robotic ventral mesh rectopexy: addressing variability in patient selection.

INTRODUCTION: Ventral mesh rectopexy (VMR) has gained popularity as a surgical solution for rectal prolapse. However, significant variability exists in patient selection criteria, preoperative evaluation, and reporting standards. This scoping review analyzes indications for robotic VMR (RVMR) and highlights areas requiring further standardization. METHODS: The review was conducted according to PRISMA-ScR guidelines. Comprehensive searches of PubMed, Scopus, and Web of Science were completed through December 4, 2024. Studies reporting on RVMR were screened in a three-step process, with disagre

Unveiling the Hidden: A Case Report on Altemeier's Surgical Triumph in Rectal Prolapse With Involvement of the Small Bowel.

Full-thickness rectal prolapse is a condition where the rectum or rectosigmoid protrudes through the anus. This condition can severely impact quality of life, requiring accurate diagnosis and treatment. Small bowel involvement is rare. This case report presents a 26-year-old male patient with complete rectal prolapse that could not be reduced through conservative management. Due to the irreducibility of the prolapse, even under spinal anesthesia, Altemeier's repair was ultimately performed. During surgery, loops of the small intestine were found along with the prolapsed segment of the rectum a

The effect of total colectomy with posterior suture rectopexy in patients with internal prolapse and colonic inertia; A case series on 9 women.

INTRODUCTION AND IMPORTANCE: The benefits of ventral mesh rectopexy, performed by laparoscopic method, simultaneous with total laparoscopic colectomy, for patients with internal rectal prolapse (IRP), have been shown previously. Considering the advantages of posterior suture rectopexy in patients with colonic inertia, we hypothesized that patients with IRP and colonic inertia may benefit from combination of posterior suture rectopexy with total laparoscopic colectomy, while this has not been evaluated, to date. CASE PRESENTATION: In the present study, we performed laparoscopic posterior recto

Comparative Outcomes of Transabdominal and Transperineal Approaches for Full-Thickness Rectal Prolapse Repair: A Fourteen-Year Retrospective Study.

BACKGROUND: The choice between transabdominal and transperineal approaches for full-thickness rectal prolapse repair remains controversial. This study compared the outcomes of these two approaches over a 14-year period in a real-world setting. METHODS: This retrospective cohort study was conducted at a tertiary hospital in Thailand and included data from surgeries performed between January 2010 and December 2023. All patients who underwent surgical repair were included, except those with rectal prolapse secondary to colorectal cancer or those who did not receive surgical treatment. Surgical a

The influence of sacrocolporectopexy on pelvic anatomy assessed in an upright position using MRI.

AIM: Rectopexy with concomitant sacrocolpopexy (sacrocolporectopexy) is the favoured technique for treating combined pelvic organ prolapse and internal or external rectal prolapse, despite limited functional improvement. Previous studies have assessed anatomical change after standalone rectopexy or sacrocolpopexy, based on supine MRI defaecography. Since a supine position can underestimate the extent of pelvic organ prolapse, it might also incorrectly assess the anatomical effect of sacrocolporectopexy. The aim of this study was to assess the effect of sacrocolporectopexy on the pelvic anatomy

Backtracking to the Brain: A Journey From Cirrhosis to Hypothalamic Insight.

We present a 31-year-old female patient with cryptogenic cirrhosis admitted for rectal bleeding secondary to rectal prolapse. During her hospital course, she was found to have severe multi-endocrine dysfunction as evidenced by persistent hypotension, bradycardia, and intermittent hypothermia with hypothyroidism, adrenal insufficiency, and diabetes insipidus, which were confirmed by laboratory testing. Due to the patient's hypothalamic-pituitary dysfunction along with biopsy-confirmed cirrhosis of unknown etiology, magnetic resonance imaging (MRI) of the brain was ordered. The MRI demonstrated

Sigmoid cancer intussusception mimicking strangulated rectal prolapse.

This case report highlights a rare instance of colonic intussusception mimicking strangulated rectal prolapse in a woman. Initially misdiagnosed with strangulated rectal prolapse, the patient presented with a painful, irreducible rectal mass. Unexpected clinical signs led to further imaging, revealing colonic intussusception with a malignant mass. This underscores the diagnostic challenges of overlapping symptoms and the necessity of thorough examinations to detect subtle anatomical differences. An emergency CT scan showed a prolapsed sigmoid colon prompting a total mesorectal excision and end

Incidence and Risk Factors for Venous Thromboembolism Events After Rectal Prolapse Surgery.

BACKGROUND: The most common cause of preventable death in hospitalized patients in the United States is venous thromboembolism. Although pelvic organ prolapse surgery in urogynecology literature reports rates less than 1%, the rate of venous thromboembolism for rectal prolapse surgery remains undescribed. We hypothesized that the rate of venous thromboembolism with rectal prolapse surgery would be low and that risk factors would include surgical approach, BMI, longer operative time, older age, renal failure, and functional status. OBJECTIVE: Define the incidence and identify risk factors asso

Sudden rupture of recurrent rectal prolapse complicated by small intestine evisceration from anus: a case study.

Evisceration of the small intestine from the anus is a rare complication following rectal injury. In patients with recurrent rectal prolapse, delayed surgical intervention can lead to rectal wall thinning due to chronic ischemia, increasing the risk of rectal rupture and subsequent intrusion of the small bowel followed by antegrade intussusception. Since Bodie first described this condition in 1827, fewer than one hundred cases have been reported globally. This case involved a 78-year-old male with a 60-year history of untreated rectal prolapse. After a fall, his abdominal pressure increased a

Evaluation of ProBNP and Troponin I in Cases of Placenta Accreta Spectrum Disorders and Placenta Praevia.

PURPOSE: Pulmonary Hypertension (PH) and Diabetes mellitus (DM) have been extensively studied independently; data on their co-occurrence are limited. This study aims to analyze trends in mortality with PH and DM recorded as contributing causes of death (PH-DM co-mortality) in the U.S from 1999 to 2024. METHOD: The CDC Wonder database was used to examine mortality trends for deaths with PH (I27.0, I27.2, I27.8, I27.9) and DM (E10-E14) recorded as contributing causes of death among adults (≥ 25 years); this age cutoff is consistent with prior CDC WONDER mortality analyses [10]. [MRI Defecography - What is Important for Surgery of the Pelvic Floor?].

MR-defecography plays a key role in imaging complex pelvic floor dysfunction. Simultaneous detection of multiple findings in a complex anatomical setting makes correct analysis and clinical interpretation challenging. Thorough understanding of this imaging technique is essential for pelvic floor surgeons in order to make proper use of its advantages in the diagnosis of pelvic floor pathologies. We have reviewed the scientific literature on MR-defecography from recent years to sum up the most important aspects concerning patients' selection and preparation, examination procedure, standardised r

Encrusted Cystitis in a Child Without Predisposing Factors: A Case Report and Literature Review.

A six-year-old girl was admitted to our hospital with rectal prolapse, urinary frequency, urinary stone drainage, and recurrent febrile urinary tract infections. A urinary culture identified Proteus mirabilis, and an ultrasonography demonstrated mucosal calcification of the bladder wall, leading to the diagnosis of encrusted cystitis. Antibiotic administration and transurethral resection of bladder calcification were performed, and her symptoms gradually disappeared. Encrusted cystitis is extremely rare in childhood and is frequently observed in patients with post-renal transplantation, urolog

Vaginal Ring Pessary Migration and Embedment With Rectal Prolapse: A Rare Complication of a Forgotten Ring Pessary in an Elderly Patient.

The use of a vaginal ring pessary is a common nonsurgical option for the management of pelvic organ prolapse. It may sometimes be considered a first-line option for patients who are not suitable surgical candidates. Pessaries generally have a good safety profile, and serious adverse complications are uncommon. However, long-term use without appropriate follow-up may lead to rare and severe complications. We report a case of vaginal ring pessary migration and embedment with rectal prolapse in an 81-year-old woman. She presented to the Emergency Department complaining of a lump in the perineum.

Management of rectal prolapse in octogenarians: lesson learned in 13 years' experience from a high-volume center.

When treating rectal prolapse, traditionally perineal procedures are recommended for elderly patients, while abdominal approaches are usually preferred in healthier and younger ones. We hypothesize that octogenarian patients can be safely treated with abdominal approaches. Our study aimed to evaluate the safety of abdominal procedures in the treatment of rectal prolapsed and to evaluate the recurrence rate. We conducted a retrospective IRB approved review of all patients ≥ 80 years old who underwent rectal prolapse surgery from 2010 to 2023 in our tertiary referral center. Patients were groupe

Delayed repair for complex genital trauma following sexual assault in a 3-year-old female with late presentation for care: a case report.

BACKGROUND: Genital trauma following child sexual abuse presents a complex and variable clinical picture of pelvic perineal injury, necessitating a comprehensive understanding of the structures involved in effective management. Ideally, these cases are considered urgent, and primary repair should be performed as an emergency. Unfortunately, the window of opportunity for urgent action may be missed as the patient may not present for care until much later. CASE PRESENTATION: We report the case of a 3-year-old African (Congolese) girl who presented to the Panzi Hospital's sexual violence ward in
