Recurrent Pregnancy Loss: Novel Insights and Future Directions.

In the last 15 years, the US and UK have fought two major wars in the Persian Gulf region. Controversy has arisen over the nature and causes of health problems among military veterans of these two wars. Toxic exposures have been hypothesised to cause the majority of the long-term health problems experienced by veterans of the 1991 Gulf War. The assessment of these toxic exposures and the resolution of controversy about their health effects provide a unique case study for understanding how toxicological disputes are settled in the US. Neither clinical examination of ill war veterans nor scienti

Macro-anatomical, histological, and ultrastructural investigation of glandulae anales in Anatolian ground squirrels.

In this study, the anal glands of 10 adult Anatolian ground squirrels (Spermophilus xanthophrymnus) were examined through dissection, scanning electron microscopy (SEM), and light microscopy. The anal glands consist of three independent complexes positioned ventrolaterally to the anus. These glandular complexes were connected with the anal canal, the rectum, and with each other through muscular and connective tissue layers coursing in multiple directions. Each anal gland complex consisted of a glandular component and a sac-like structure. The sac structures of the anal gland complexes, which c

Life-threatening lower gastrointestinal bleeding after routine anorectal procedures: a case series and management implications.

BACKGROUND: This case series characterizes enema- and catheter-induced rectal injuries as an underrecognized iatrogenic cause of life-threatening lower gastrointestinal bleeding (LGIB). By focusing on elderly, high-risk patients, we highlight the diagnostic complexities and management strategies associated with these routine yet potentially catastrophic anorectal procedures. METHODS: We analyzed three cases of high-risk elderly patients who experienced acute, massive hemorrhage following routine anorectal procedures, specifically glycerin enemas and anorectal manometry. To contextualize these

Integrated intraoperative strategies to prevent anastomotic leakage after laparoscopic low anterior resection: The roles of left colic artery preservation, reinforcement suturing, and transanal tube placement.

Anastomotic leakage is a major complication following laparoscopic low anterior resection in patients with low rectal cancer. This study investigated the preventive effects of three intraoperative techniques, including the preservation of the left colic artery, reinforcement suturing, and transanal decompression tube placement. A total of 88 patients undergoing laparoscopic rectal cancer surgery were retrospectively analyzed. Univariate and multivariate logistic regression analyses were performed to assess the associations between these interventions and the occurrence of anastomotic leakage.

Visceral technetium-99 m-labeled pyrophosphate uptake in patients with wild-type transthyretin cardiac amyloidosis.

OBJECTIVES: Technetium-99 m-labeled pyrophosphate (Tc-99 m-PYP) scintigraphy can image tracer uptake in the myocardium, subcutaneous fat, and skeletal muscles of patients with wild-type amyloid transthyretin (ATTRwt) cardiac amyloidosis (CA), which indicates ATTR deposition. However, there are few reports of visceral Tc-99 m-PYP uptake in patients with ATTRwt-CA. This study was conducted to determine whether Tc-99 m-PYP scintigraphy can image ATTR deposition in the viscera, particularly the liver, spleen, and gastrointestinal tract. METHODS: Chest- and abdomen-centered Tc-99 m-PYP scintigraphy

Detection and surgical intervention in anorectal malignant melanoma: a case report.

BACKGROUND: Anorectal malignant melanoma (ARMM), a rare and highly aggressive subtype of mucosal melanoma, accounts for < 0.1% of all colorectal malignancies. Characterized by nonspecific clinical manifestations and the absence of early diagnostic biomarkers, ARMM is frequently diagnosed at advanced stages, resulting in a dismal prognosis with a 5-year survival rate below 20%. Despite recent advances in immunotherapy and targeted therapies, critical knowledge gaps persist regarding its pathogenesis, standardized therapeutic protocols, and prognostic prediction models. CASE PRESENTATION: We pr

Bridging the gap: a systematic literature review and meta-analysis on the management of rectal wall defect after transanal excision.

BACKGROUND: Transanal resection techniques have gained considerable importance in the treatment of benign and malignant rectal neoplasms. However, there is no definitive consensus on whether the rectal defect should be closed or left open after excision. We sought to provide an updated pooled analysis of the management of rectal wall defect after transanal excision. METHODS: In accordance with PRISMA and Cochrane guidelines, this meta-analysis was performed using the PubMed (MEDLINE), Cochrane Central Register of Controlled Trials, and Google Scholar databases to identify studies comparing pe

Primary anorectal melanotic melanoma: a case report.

BACKGROUND: Anorectal melanoma (AM) is a rare and aggressive malignancy that is often advanced at the time of diagnosis. This case demonstrates the importance of a prompt colonoscopic evaluation, especially in older patients presenting with new gastrointestinal symptoms, and underscores the need for further research establishing a standardized treatment protocol. CASE PRESENTATION: We report a case of a 71-year-old white male who was referred to gastroenterology after noticing increased frequency of bright red blood per rectum following bowel movements. A subsequent colonoscopy revealed a 1.5

Bowel perforation and transrectal migration, an extremely rare complication of a peritoneal dialysis catheter: a case report.

Peritoneal dialysis catheters are an excellent and highly effective treatment for end-stage renal failure. This procedure is relatively easy and straightforward, but as with any other medical procedure, it can lead to complications, which include infection, hematoma, leakage, and peritonitis. However, in a few selected cases, the catheter can perforate the bowel, and in even rarer cases, it can migrate through it. We report the case of an 18-year-old who needed renal replacement therapy through a Tenckhoff catheter. Two years after its placement, he suddenly noticed that the tip of the cathete

Clinical Outcomes and Recurrence Prevention in Laparoscopic Wells Rectopexy for Rectal Prolapse through Mesh Trimming and Fixation Techniques.

OBJECTIVES: Laparoscopic Wells rectopexy (LWR) is a standard procedure for rectal prolapse; however, recurrence and mesh-related complications remain clinical challenges, particularly in elderly patients with small pelvic anatomy. This study aimed to evaluate the clinical outcomes of a modified LWR technique featuring intraoperative mesh tailoring and a hybrid fixation method. METHODS: We retrospectively analyzed 29 patients who underwent modified LWR between October 2014 and June 2025. Technical modifications included intraoperative trimming of the polypropylene mesh to fit each patient's pe

Preoperative Radiotherapy as a Major Risk Factor for Pelvic Abscess after Abdominoperineal Resection and Total Pelvic Exenteration: A Retrospective Study.

OBJECTIVES: Pelvic abscess, alongside perineal wound infection, is a major postoperative complication after abdominoperineal resection (APR) and total pelvic exenteration (TPE). Because the underlying mechanisms of these two complications differ, separate evaluation is warranted. This study aimed to identify risk factors specifically associated with pelvic abscess. METHODS: We retrospectively analyzed patients who underwent APR or TPE at our institution between December 2004 and August 2025. Patients were divided into abscess and non-abscess groups, and patient characteristics and surgical va

Intraoperative Doppler Ultrasonography with Superb Microvascular Imaging for Intestinal Perfusion Assessment in Colorectal Cancer Surgery: A Prospective Pilot Study.

OBJECTIVES: Intraoperative indocyanine green fluorescence imaging (ICG-FI) reduces anastomotic leakage, but has limitations related to contrast agent use, repeatability, and timing-dependent interpretation. We conducted an exploratory trial of a Doppler ultrasonography method using a high-frequency probe with superb microvascular imaging (SMI) to assess intestinal perfusion during colorectal cancer surgery. METHODS: In this single-center prospective pilot study, we enrolled patients undergoing colorectal cancer resection with anastomosis. Intraoperatively, SMI assessed intramural microvascula

Aggressive Angiomyxoma in the Anus of Elderly Male.

Aggressive angiomyxoma (AAM) is a rare mesenchymal tumor characterized by local invasion and a high recurrence rate, primarily affecting the pelvic-perineal region in women. It is extremely rare in men. Although male-onset cases are typically reported in the scrotum or buttocks, there have been no previously reported cases involving the anus. We report the first such case in a 71-year-old man initially diagnosed with prolapsed internal hemorrhoids (Goligher grade IV). The patient underwent surgical treatment; however, histological analysis of the specimen unexpectedly revealed AAM, with positi
