When defects of the anal sphincter are caused by trauma, surgical correction can be successful even in long-standing cases. At the University of Minnesota, we used overlapping sphincteroplasty in 79 patients with fecal incontinence from 1952 to 1982. There were 62 women and 17 men. Ages ranged from 17 to 68 years. This is a retrospective study of 1753 cases of chronic anal fissures treated by five varying methods over a five-year period from January 1976 to December 1980. Results showed that the incidence of recurrent fissures was higher in those treated by anal fissurectomy with sphincterotomy. After a short survey of the literature concerned, it is pointed out that early findings of rectal carcinoma (T1 or Dukes' disease A orCSI + II according to Mason) can be removed locally in the unaffected tissue under observation of the strictest indication by transanal surgery or by posterior rectomy. In this procedure the surgical mortality is 0 per cent, the ten-year-survival can go up to 90 per cent. The own material described comprises 97 patients with an average age of 64.5 years.

We examined 136 consecutive patients with histories of guaiac positive stool examinations, bright red blood per rectum, or hematochezia to determine the value of the double-contrast barium-enema (DC-BE) examination and colonoscopy/proctoscopy in establishing bleeding sites. If examination findings were analyzed in conjunction with findings of visual examination of the anal area, the difference in the respective sensitivities of the two examinations was not statistically significant. Management of the perineal wound following rectal excision was assessed in 57 patients at the Toronto General Hospital; 40 had ulcerative colitis, 4 had Crohn's disease, 10 had carcinoma of the rectum, 2 had carcinoma of the anus and 1 had anal incontinence. The extent of "first-pass" elimination of lidocaine in relation to the site of absorption in the rectum of rats was investigated. The histology of the rectal mucosa in 57 men with culturally-proven rectal gonorrhoea is described and the results compared with the findings in biopsies from 57 non-infected men. An acute inflammatory cell infiltration of the rectal mucosa was found in three (5.3%) and two (3.5%) infected and non-infected men respectively. Twenty-one (36.8%) men with rectal gonorrhoea and nine (15.8%) non-infected men had an increased number of chronic inflammatory cells in the lamina propria.

The causes of ano-rectal bleeding as the only symptom in 110 patients were studied. A diagnosis was obtained in 77% of patients after clinical examination, sigmoidoscopy and proctoscopy, increasing to 97% after radiology and colonoscopy. Anal causes of bleeding were found in 81% of the patients, while a source in the rectum or colon accounted for only 16%.
