Preservation of the anal sphincter is a goal of all surgeons treating colorectal malignancies. In the hands of many, transsacral and low anterior resections with end-to-end anastomosis have been associated with high morbidity and leakage. The EEA stapler represents an alternative in re-establishing bowel continuity. Fifty-eight patients with adenocarcinoma of the rectum were treated over a three-year period at Roswell Park Memorial Institute. Forty-nine procedures judged to be curative and nine, palliative. Nitroglycerin administered orally undergoes substantial presystemic elimination. It was shown recently that first-pass hepatic metabolism of high clearance drugs can be substantially avoided via rectal administration. In applying this concept to nitroglycerin in rats, it was found that unrestricted rectal instillation of nitroglycerin (at 3.5-mg/kg dose) gave a mean +/-SD bioavailability of 26.7 +/- 7.0% (n=6) compared to 1.8 +/- 0.9% (n=5) from oral dosing. The authors have reviewed their cases of neonatal obstructive malformations of the intestinal tract and analyzed the incidence of association with indirect hyperbilirubinemia. 39 patients in four years are considered. In 23 of them there is a bilirubin level greater than 5 mg/100 ml. In the 18-year period between 1960 and 1978, only 20 cases of carcinoma of the anus were recorded in the Cancer Registry of the University College Hospital, Ibadan, Nigeria; of these 18 were treated in the hospital. Textbooks do not provide sufficient descriptions of the proctodeal anatomy and its clinical significance. For example, operation on anal or anorectal fistulas demands detailed knowledge of the continence organ. Obliterating arteriosclerotic lesions of pelvic vessels may induce inflammation and fibrosis of the rectum and subsequently ischemic proctitis. This may lead to damage of the sphincter muscle and to complete incontinence. Clinical symptoms are pain and bloody stools, as in ischemic colitis. The combination of periproctitis and perianal necrosis may be misinterpreted as anal fistulas. An anus praeter sigmoideus is to be recommended for palliation. A case report of this particular disease is given including clinical, angiographic and morphological data.

Of 77 patients treated by sphincter-saving procedures (anterior or pull-through resection) for cancer of the rectum, 17 (22%) developed a local recurrence; 10 of them were submitted to abdominoperineal resection. Forty-three postoperative anorectal bleeding episodes were encountered in the treatment of 4,059 anorectal operations, an incidence of 1.06 per cent.
