An electronic measuring instrument, which automatically calculates the rectal potential difference, has been developed to determine mineralocorticoid activity in patients with primaroid disorders. A special probe with a disposable tip enables measurements to be made on successive patients without prior cleaning of the probe. The technique, results and their clinical implications are discussed. A highly significant correlation between plasma aldosterone and rectal potential difference is demonstrated.

We have described the clinicopathologic findings in two cases of anorectal melanoma, and extracted the salient features from the medical literature. The disease is rare. Melanoma arises from the anal squamous membrane and very often spreads upward through submucosal planes, producing secondary satelites in the rectum. Of the benign colonic epithelial neoplasms the villous papilloma, although rare, is reported to become malignant in 30--50%. This paper reports a retrospective study of 213 patients with a colorectal villous papilloma. The tumours were subdivided histologically according to Wheat and Ackerman (1958) into four different groups. 20% of the tumours contained either focal or invasive carcinoma. A prospective study as been carried out on fifty patients with haemorrhoids treated by the Lord method who were followed up for at least four years. All patients were interviewed and examined regularly during the observation period. 75% of patients were rendered symptom-free or considerably improved. Apparently the long-term results of this method of treatment can be predicted from the results at eighteen months after operation.

Urologic complications arose in 23.7 per cent of 569 patients who underwent abdominoperineal or low anterior resection for anorectal cancer. No radomization of the two operations was attempted, low anterior resection being performed whenever resection 5 cm below the tumor was possible. 81 patients with tumors of the anal canal and anal margin treated at the Cantonal Hospital, Lausanne, during the period January 1942 to June 1974 have been studied. The morbid anatomy of transitional zone and cloacogenic carcinomas, subdivided into basaloid types, is related to the anatomy and histology of the normal anal canal. An anatomical and clinical study aimed at uncovering factors likely to be helpful in understanding the true nature of haemorrhoids is described. We have endeavored to show the importance of iron deficiency and to cite a few examples of the systemic effects of such deficiency without anemia. We have shown the need for large-scale screening of patients for iron deficiency and suggest that serum iron and total iron-binding capacity determinations will adequately furnish the vehicle for this purpose. The modes of iron therapy are discussed with the belief that for the surgeon parenteral iron is the best vehicle to replace the deficiency with the least risk.
