Retroperitoneal Dedifferentiated Liposarcoma With Penetrating Abscess Formation Secondary to Ileal Invasion: A Case Report.

Liposarcoma is a relatively common malignant soft tissue tumor in adults. Dedifferentiated liposarcoma (DLS) is a particularly invasive subtype characterized by high recurrence rates and a poor prognosis. We describe a case of a retroperitoneal liposarcoma with poor differentiation that presented with peritonitis accompanied by ileal infiltration and perforating abscess formation. A 77-year-old male presented to our hospital with abdominal pain and fever. Computed tomography revealed a 13 cm tumor in the right lower abdomen with internal air density. An ileocecal resection was performed. Histo

Clinical, Endoscopic, and Histological Characteristics of Severe Immune Checkpoint Inhibitor-Induced Colitis.

Background: Immune checkpoint inhibitors (ICIs) have revolutionized cancer therapy. They can cause immune-mediated colitis (IMC), a potentially severe adverse event. Current data on severe IMC (grade 3-4) are limited, particularly regarding clinical, endoscopic, and histological features. Methods: We conducted a multicenter, retrospective observational study promoted by GETECCU, including adults with solid tumors who developed grade 3-4 IMC requiring hospitalization and systemic therapy. Clinical symptoms, endoscopic findings (Mayo and UCEIS indices), and histological features were systematica

Use of barbed suture in surgery and the risk of intestinal obstruction: analysis of a large institutional cohort.

BACKGROUND: Barbed sutures have become increasingly popular across multiple surgical specialties due to their self-anchoring design, which eliminates the need for knot tying and reduces operative time. However, concerns persist that exposed barbs may cause postoperative intestinal obstruction by entangling adjacent bowel loops or mesentery. The true incidence of such events in large surgical populations remains unclear. METHODS: A retrospective analysis was conducted using an institutional database encompassing all intra-abdominal surgeries performed with barbed sutures from August 2017 to Ap

Increased Prevalence of Pulmonary Diseases in Patients With Inflammatory Bowel Disease: A Retrospective Multicenter Study.

BACKGROUND: Inflammatory bowel disease (IBD) is a multisystem illness with intestinal and extraintestinal manifestations. We aimed to analyze the association of developing pulmonary diseases (PDs) in patients with IBD. METHODS: In this retrospective cohort study, TriNetX was used to identify patients with IBD who were prescribed at least one IBD-specific medication or advanced therapy. Patients with an inpatient visit and no IBD comprised the control group. Propensity score matching (PSM) was used to balance cohorts. The odds ratio (OR) of developing PDs all-cause mortality were determined.

Effect of Ileal Feces Transplantation in Preventing Diversion Colitis and Its Complications Due to Protective Loop Ileostomy in Rectal Cancer Cases.

To evaluate the effectiveness of fecal transplantation (FT) in preventing the development of diversion colitis (DC) and reducing its complications in patients who underwent protective loop ileostomy (PLI) following low anterior resection for rectal cancer. The study was prospectively conducted at Tertiary Hospital. Seventeen patients who underwent PLI were assigned to the FT group (FT+), and 19 patients served as the control group (FT-). In the FT+ group, ileal content was delivered to the efferent loop via a catheter placed during surgery. Both groups were evaluated postoperatively through bi

Predictors of Recurrence in Pediatric Perianal Abscess and Fistula-In-Ano: Insights From a 5-Year Surgical Cohort Study.

BACKGROUND Perianal abscess (PA) and fistula-in-ano (FIA) are common in children, particularly infants. Despite their frequency, their pathophysiology, diagnostic accuracy, and optimal treatment remain debated. This study aimed to evaluate recurrence rates after surgical treatment of PA and FIA and identify clinical factors associated with fistula formation, including ultrasound findings, antibiotic therapy, abscess size, and symptom duration in otherwise healthy pediatric patients. MATERIAL AND METHODS We retrospectively reviewed 108 pediatric patients (0-17 years) treated surgically for PA a

Exploring genotype-phenotype correlations in three pediatric patients with IL10RA variants and very early-onset inflammatory bowel disease.

BACKGROUND: Very early onset inflammatory bowel disease (VEO-IBD) is a severe chronic intestinal inflammatory disorder with onset in infancy. Mutations in the interleukin-10 (IL-10) receptor A (IL10RA) gene represent a key pathogenic mechanism of VEO-IBD; however, the genotype–phenotype correlations remain unclear. OBJECTIVE: This study aims to describe and compare the clinical features, genotypes, and family segregation data of three pediatric patients with IL10RA mutation-associated VEO-IBD to explore potential associations between different IL10RA mutation sites and types (compound heterozy

Perforated solitary mid-rectal diverticulum diagnosed with colonoscopy: a case report and literature review.

Rectal diverticulum is an exceedingly rare clinical entity, typically occurring as a solitary lesion or, less frequently, as up to three diverticula. Its development is often associated with underlying conditions such as obstructive defecation syndrome, rectal malignancy, pelvic floor weakness, chronic rectal impaction, obesity, or infection-related muscular atrophy. Although uncommon, rectal diverticulitis represents a surgical emergency requiring timely recognition and management. In this case, a female patient presented with rectal bleeding and secondary anemia. During the clinical course,

Evidence Suggests Prophylactic Antibiotics May Be Unnecessary in Anorectal Surgery-A Systematic Review and Meta-Analysis.

BACKGROUND: Amid increasing global concerns regarding antimicrobial resistance, the routine use of prophylactic antibiotics in anorectal surgery has been questioned. In practice, prescribing practices vary widely among surgeons, highlighting the need for stronger evidence-based guidance. The aim of this study is to perform a systematic, critical assessment of the current literature to determine the role of prophylactic antibiotics in elective anorectal surgery. METHODS: A comprehensive search of studies published between January 1980 and June 2025 was performed using PubMed, Embase, and Cochr

An unusual aspect of Pott's disease: small bowel obstruction from a tuberculous psoas abscess.

Pott's disease typically presents with back pain and systemic symptoms, but atypical presentations can obscure diagnosis. We describe a rare manifestation where the initial presentation was acute small bowel obstruction. A 20-year-old female presented with acute abdominal pain, distension, and vomiting, with a 3-month history of intermittent lower back pain. Computed tomography scan revealed distal ileal obstruction due to wall thickening adjacent to a left psoas abscess and a large paravertebral abscess. Aspirated pus confirmed Mycobacterium tuberculosis. The diagnosis was small bowel obstruc

Diverticular Perforation with Normal C-reactive Protein in a Patient with Neuromyelitis Optica Spectrum Disorder Receiving Satralizumab.

Interleukin-6 blockade can blunt inflammatory biomarkers and mask serious infection. A 49-year-old woman with neuromyelitis optica spectrum disorder receiving satralizumab presented with mild abdominal pain; C-reactive protein levels were normal at the time of perforation despite a transient elevation early during hospitalization. Computed tomography revealed colonic diverticular perforation with a localized abscess. She improved with percutaneous drainage, targeted antimicrobials, and withdrawal of satralizumab, followed by transition to ravulizumab. This case suggests that satralizumab may o

Giant pyogenic ruptured hepatic abscess causing intestinal occlusion resolved by a laparoscopic approach: a case report.

Pyogenic liver abscesses (PLAs) are rare, Klebsiella pneumoniae, and Escherichia coli are the most common pathogen of liver abscess. Clinical presentation of PLAs is non-specific, the most common presentation includes fever and rigors, nausea and vomiting, right upper quadrant pain, anorexia, weight loss, weakness and malaise. Untreated, (PLA) is uniformly fatal due to ensuing complications. The peritoneal cavity is the most common location of ruptured liver abscess, a serious surgical emergency. Systemic antimicrobial therapy remains the mainstay primary treatment. We present a rare case of 6

Conservative Management of Perforated Diverticulitis in Adults: An Updated Systematic Review of Outcomes.

BACKGROUND: The management of perforated diverticulitis (PD) has shifted over the last decade, moving from aggressive surgery toward non-operative strategies for hemodynamically stable patients. However, the safety of non-operative treatment in cases with extraluminal air, particularly Hinchey Ia-II disease, remains debated. This systematic review evaluates the efficacy, safety, and long-term outcomes of non-operative management for PD and identifies radiologic predictors of treatment failure. METHODS: A systematic review was conducted per PRISMA 2020 guidelines. PubMed/MEDLINE, Europe PMC, S
