[Perianal surgery in a non-sterile setting: risks, cost-saving ad climate benefits].

INTRODUCTION: Sustainable healthcare is essential to reduce environmental impact without compromising patient safety. This study investigates whether minor perianal surgery can be safely performed under a non-sterile condition, aiming to reduce material use, costs, and CO2-emissions, and whether these findings can be incorporated into a widely supported surgical protocol. METHODS: A retrospective cohort study was conducted among patients who underwent minor perianal surgery under sterile or non-sterile conditions. Primary outcomes were postoperative wound infections (POWIs) within 30 days, co

Revised Criterion for Identifying Small-Bowel Stricture in Crohn Disease at CT Enterography.

Background Recent guidelines lowered the prestenotic dilatation threshold from >3 cm to ≥2.5 cm for diagnosing small-bowel stricture at CT enterography (CTE) in Crohn disease (CD). Its impact on stricture prevalence and risk stratification is unknown. Purpose To quantify the increase in stricture prevalence when applying a threshold of ≥2.5 cm and to assess whether risks for adverse outcomes are stratified according to the new threshold. Materials and Methods This retrospective study included patients with CD without acute obstructive symptoms who underwent CTE from 2017 to 2018 for routine fo

Gossypiboma Following Cesarean Section Presenting as Bilateral Abdominal Abscesses: A Case Report.

Background: Gossypiboma is an uncommon postoperative complication caused by the retention of surgical materials, most frequently sponges, and is associated with substantial morbidity and medicolegal consequences. Despite a reduction in reported incidence, diagnosis remains challenging due to its nonspecific clinical presentation. Case Presentation: We present the case of a 36-year-old woman who presented with a one-week history of throbbing abdominal pain in the umbilical and left lumbar regions, associated with fetid leukorrhea. Her medical history included an appendectomy, a recent cesarean

A Case of Abdominal Closure Using Endoscopic Anterior Component Separation Following Open Abdomen Management for Colon Perforation.

INTRODUCTION: In open abdomen management (OAM), consistent early closure is crucial to avoid complications and improve short- and long-term outcomes. Open anterior component separation (OCS) has been reported to be a method for early definitive closure after OAM.Originally, OCS is a common treatment option for incisional hernias. However, a major drawback is the high incidence of surgical site occurrences (SSOs) owing to impaired blood flow. This is caused by damage to the perforating branches supplying the skin from the rectus abdominis muscle, necessitating extensive subcutaneous dissection.

FAERS based pharmacovigilance study and network pharmacology analysis of Lutathera and Pluvicto.

BACKGROUND: Lutetium Lu 177-based agents Lutathera (targeting SSTR in GEP-NETs) and Pluvicto (targeting PSMA in mCRPC) exhibited significant efficacy in clinical trials, yet real-world safety validation remains imperative. METHODS: We performed disproportionality analysis on FAERS reports and applied network pharmacology to identify drug targets, construct protein-protein interaction networks, and elucidate adverse event mechanisms through KEGG pathway enrichment. RESULTS: Shared adverse reactions for both agents include pleural effusion (ROR 1.68, 95%CI: 1.03-2.74) and pulmonary embolism (R

Emergency surgical approach for abdominal trauma in a patient with Crohn's disease: case report and literature review.

INTRODUCTION: Crohn's disease (CD) is a chronic, relapsing inflammatory condition that affects the gastrointestinal tract, associated with abscesses, fistulas, and perforations. Owing to impaired integrity and proximal dilatation secondary to strictures, CD patients may be predisposed to perforations after blunt abdominal trauma. CASE PRESENTATION: A 29-year-old male was admitted to a tertiary hospital after a motor vehicle accident. He underwent chest drainage for pneumothorax and remained stable. Abdominal computed tomography scan revealed pneumoperitoneum and thickening of small bowel loop

Appendiceal abscess complicated by mechanical small-bowel obstruction: a diagnostic and therapeutic challenge: A case report.

RATIONALE: Appendiceal abscesses are typically managed conservatively. Mechanical small-bowel obstruction (MSBO) is a rare complication that poses diagnostic and therapeutic dilemmas. PATIENT CONCERNS: A 52-year-old woman with appendiceal abscess, which also manifested as symptoms of intestinal obstruction, was finally confirmed to have MSBO intraoperatively. DIAGNOSES: Appendiceal abscess, MSBO. INTERVENTIONS: The patient presented with 5-day migratory right lower quadrant pain. Initial computed tomography suggested appendicitis with possible small-bowel obstruction. Despite the use of agg

Atypical Presentation of an Odontogenic Brain Abscess Due to Eggerthella lenta and Bilophila wadsworthia Coinfection.

Odontogenic brain abscesses are uncommon. Eggerthella lenta and Bilophila wadsworthia are anaerobes that are normal components of the human intestinal microbiota. Both can also be found in the oral cavity. We report the case of an 81-year-old man with a brain abscess caused by coinfection of E. lenta and B. wadsworthia associated with periodontitis. The patient underwent a right frontal craniotomy with excision of the abscess. Ceftriaxone and metronidazole were then started, along with levetiracetam. Ceftriaxone and metronidazole were maintained for four weeks, and the antibiotic treatment was

Implementation of Colectomy Guidelines: Increasing Rate of Safe Anastomosis in Emergency General Surgery Patients.

INTRODUCTION: Creation of a colostomy versus anastomosis remains a controversial topic in emergency general surgery (EGS) patients requiring emergent or urgent colon resection. In 2017, our institution standardized the indications for colostomy creation in EGS patients, which included the presence of septic shock, poor tissue condition at the anastomotic site, and conditions associated with poor wound healing. In the absence of these conditions, the guideline recommended anastomosis. The goal of this study is to examine rates of colostomy, anastomosis, and abdominal complications before (PRE)

Appendiceal stump length as a predictor of operative outcomes in complicated appendicitis.

BACKGROUND: Optimal management of patients with acute complicated appendicitis remains debated. Traditional management often emphasized nonoperative treatment using antibiotics and, when necessary, percutaneous drainage. Growing evidence indicates that operative management may be safe in select patients. This study aimed to evaluate whether appendiceal stump size ≥2 cm on preoperative CT scan is predictive of intraoperative complexity and postoperative outcomes. METHODS: A multicenter retrospective review evaluated operative outcomes in patients with acute complicated appendicitis identified

Risk factors for stricture-related surgery in stricturing Crohn's disease.

BACKGROUND: Crohn's disease (CD) is a chronic inflammatory condition of the gastrointestinal tract, frequently complicated by stricture, fistula, and abscess. Strictures affect nearly 50% of patients, often requiring medical, endoscopic, or surgical management. This study aimed to identify surgical risk factors and postoperative complications in CD patients with strictures. METHOD: Patients with stricturing CD followed at our inflammatory bowel disease (IBD)- specific outpatient clinic between 1989 and 2024 were retrospectively analyzed. Clinical data were collected from hospital records. Pat

Baseline stool TIMP-2 predicts strictures and penetrating disease progression in Crohn's patients.

OBJECTIVE: Longstanding subclinical Crohn's Disease (CD) can progress to complications such as intestinal strictures and penetrating (intestinal fistulae, abscesses, or perforations) disease. Despite the advances in non-invasive diagnostics for IBD, there is a dearth of biomarkers for predicting disease progression and complications in CD. Here, we investigate four baseline stool biomarkers, BDNF, MMP-8, TIMP-2, and Calprotectin (S100A8/A9), for their ability to predict disease progression in pediatric CD. METHODS: Baseline stool samples from forty-seven patients recruited into the Pediatric

Intestinal amoebiasis: a cause of colonic ulceration.

We present two clinical cases of intestinal amoebiasis. The first case was asymptomatic, with colonic ulcers incidentally detected during colonoscopy. The second case presented with diarrhea and a hepatic abscess, also associated with colonic ulcers. Both patients showed a favorable clinical outcome following treatment. Amoebiasis is caused by the parasite Entamoeba histolytica. It is transmitted via the fecal-oral route and is more common among travelers, immunocompromised individuals, and men who have sex with men. Clinically, it can range from asymptomatic infection to severe colitis and he
