Norovirus gastroenteritis complicated by polymicrobial bacteremia and liver abscess.

This report describes a patient with norovirus gastroenteritis complicated by a polymicrobial liver abscess involving Klebsiella pneumoniae and Clostridium perfringens. The patient presented with viral gastroenteritis and subsequently developed bacteremia and multiple hepatic abscesses within six days, likely due to norovirus-associated disruption of the intestinal mucosa facilitating bacterial translocation. Blood cultures identified both bacterial organisms, and stool polymerase chain reaction (PCR) detected norovirus. Despite the high mortality associated with C. perfringens bacteremia, inc

Perforated Appendicitis in a 52-Year-Old Male With Previously Undiagnosed Intestinal Malrotation: A Case Report.

Intestinal malrotation is a congenital anomaly resulting from abnormal midgut rotation during embryologic development. Although commonly diagnosed in infancy, it may remain asymptomatic until adulthood and is often discovered incidentally. Acute appendicitis in the setting of malrotation may present atypically due to abnormal positioning of the cecum and appendix, potentially leading to delayed diagnosis and increased morbidity. We report a case of a 52-year-old male who presented with left lower quadrant (LLQ) abdominal pain and was found to have perforated appendicitis in the context of prev

Breast Abscess Caused by Salmonella paratyphi A in an Immunocompetent Non-Lactating Female: A Case Report.

BACKGROUND: Breast abscesses are most commonly caused by Staphylococcus aureus, whereas Salmonella enterica Paratyphi A is an exceptionally rare pathogen, even in regions where enteric fever is endemic. Extra-intestinal infection due to Salmonella enterica Paratyphi A is uncommon, and involvement of the breast is particularly rare. CASE PRESENTATION: We report a case of a 36-year-old immunocompetent, non-lactating female who presented with a four week history of pain and swelling in the right breast. Ultrasound revealed a localized abscess, which was aspirated yielding purulent material. Cult

A Rare Case of Sterile Retrostyloid Abscess in a Patient With Crohn's Disease: Diagnostic and Therapeutic Insights.

INTRODUCTION: Aseptic abscess syndrome is a rare inflammatory disorder of unclear etiology, characterized by sterile lesions densely infiltrated by polymorphonuclear neutrophils. Its frequently associated with inflammatory bowel disease, which includes chronic gastrointestinal disorders such as Crohn's disease and ulcerative colitis, often accompanied by both intestinal and extraintestinal manifestations. Extraintestinal involvement may include musculoskeletal, cutaneous, ocular, oral, and hepatobiliary systems. Although uncommon, aseptic abscess syndrome can precede the onset of inflammatory

Complicated Amyand's hernia.

Amyand's hernia is a rare condition characterised by the presence of the appendix within an inguinal hernia sac. We describe a case of a complicated right-sided Amyand's hernia, where imaging demonstrated a ruptured hernia containing a perforated appendix with an appendicolith and a rim-enhancing scrotal abscess, necessitating urgent surgical intervention. This case highlights the importance of computed tomography (CT) imaging in evaluating atypical groin sepsis, the value of multidisciplinary management, and the critical role of timely surgery. It further emphasises the need for individualise

Intestinal ultrasound to detect postoperative recurrence in patients with Crohn's disease: an international, multidisciplinary, RAND/UCLA appropriateness method study.

BACKGROUND: Intestinal ultrasound is a non-invasive method increasingly used to diagnose and monitor disease activity in patients with inflammatory bowel disease; however, there is no consensus on the appropriate implementation of intestinal ultrasound to assess postoperative recurrence in Crohn's disease. We aimed to develop expert-based recommendations for standardising intestinal ultrasound use for the detection and evaluation of postoperative Crohn's disease recurrence. METHODS: An international, multidisciplinary, modified RAND/University of California Los Angeles appropriateness method

10 Strategies for prevention of perineal wound dehiscence after intersphincteric proctectomy for perianal Crohn's disease.

Non-healing perineal wounds (NHPWs), including persistent presacral sinuses, remain a common and morbid complication after elective intersphincteric proctectomy (ISP) for fistulizing perianal Crohn's disease (pCD), with reported healing rates of approximately 70% at 1 year. These complications significantly impair quality of life and increase healthcare utilization, highlighting the importance of prevention. This narrative review summarizes contemporary evidence and expert practice to present ten pragmatic strategies to reduce perineal wound dehiscence after ISP for pCD. Prevention begins with

Transperineal contrast-enhanced ultrasound: a rapid and reliable tool for differentiating abscesses from inflammatory masses in perianal Crohn's disease.

BACKGROUND AND AIM: Perianal abscesses are serious complications of Crohn's disease (CD) requiring timely and accurate diagnosis. Contrast-enhanced ultrasound (CEUS), which in abdominal intestinal ultrasound (IUS) has shown diagnostic performance comparable to CT/MRI in distinguishing intra-abdominal abscesses from inflammatory masses, has not been systematically evaluated in the transperineal assessment of perianal Crohn's disease. This study aimed to evaluate the diagnostic performance, inter- and intra-observer agreement, and diagnostic confidence of transperineal CEUS versus conventional B

Cross-kingdom signals: microbial, metabolites, and immune cells on the gut-breast axis.

The mammary gland is a dynamic mucosal organ whose structure and function are continually reshaped across pregnancy, lactation, and involution. Emerging evidence places it within a broader gut-breast axis in which intestinal microbes and their products influence mammary immunity, epithelial programs, and the composition of human milk. Three mechanistic lanes organize this crosstalk. First, microbial signals-ranging from structural ligands to extracellular vesicles-can reach the breast and tune innate sensing. Second, gut-derived metabolites-notably short-chain fatty acids (SCFA), secondary bil

Unusual presentation of Currarino syndrome in an infant with right gluteal abscess.

Currarino syndrome is a rare condition with an incidence of approximately 1 in 100,000. It is characterised by the triad of a sacral defect, a presacral mass and anal atresia or stenosis, and it is commonly associated with genetic mutations in the MNX1 gene.We report a case of an infant presenting with recurrent right gluteal abscess secondary to an infected sacrococcygeal teratoma, associated with anal stenosis. Extensive workup was performed to investigate for the source of her recurrent gluteal abscess and anal stenosis, leading to the diagnosis of Currarino syndrome. She underwent multiple

The role of the Alistipes genus in intestinal inflammation, cancer, and aging: a narrative review.

Alistipes, a bacterial genus under phylum Bacteroidetes, widely colonizes the gut. Since the first report of Alistipes in 2003, 13 species and 3 subspecies have been successfully isolated and identified from human feces, urine, appendiceal lesions, and rectal abscesses. Alistipes was reported to enhance the efficacy of immunotherapies in cancer treatment, while the detailed descriptions of other biological functions of this bacterium are lacking. This review summarizes the roles and potential mechanisms of Alistipes and its metabolites in inflammatory bowel diseases, digestive cancer, lung can

Possible Gastroenterological Causes of FUO (Fever of Unknown Origin).

Fever of unknown origin (FUO) remains a persistent diagnostic challenge in clinical medicine despite significant advances in laboratory testing and imaging techniques. The definition of FUO has evolved since the original criteria proposed in 1961 and currently refers to persistent fever exceeding approximately 38.2-38.3 °C without a definitive diagnosis after an adequate diagnostic evaluation. Gastrointestinal diseases represent an important but often underrecognized group of conditions associated with FUO. The aim of this review is to synthesize current evidence on the gastroenterological cau

Incidental Diagnosis of Serrated Polyposis Syndrome of the Small and Large Bowel Treated Endoscopically and Surgically: A Case Report.

Colorectal serrated polyps are now recognized as an important pathway in colorectal carcinogenesis. An association between serrated polyposis syndrome (SPS) and duodenal serrated lesions sharing a similar immunophenotype in both the colon and duodenum has been reported in the literature. We present a case of a 38-year-old male who presented in June 2023 with abdominal pain and fever. Initial investigations, including multiple computed tomography scans of the abdomen and pelvis, revealed an appendiceal abscess and small bowel intussusception involving two segments of the jejunum without evidenc

False Bilothorax Secondary to Incarcerated Left Diaphragmatic Hernia With Intestinal Obstruction: A Case Report and Literature Review.

Bilothorax, or cholethorax, is defined as the presence of bile within the pleural cavity and represents a very rare entity, classically associated with hepatobiliary injuries, trauma, surgery, or subphrenic abscesses. A pleural-to-serum bilirubin ratio greater than 1 is generally considered suggestive of the diagnosis. Most reported cases occur in the right hemithorax due to the anatomical proximity of the liver, whereas left-sided or bilateral cases are exceptional. We report the case of a 39-year-old male with a history of prior penetrating thoracic trauma who presented with abdominal pain,
