Comprehensive Imaging Evaluation and Staging of Crohn's Disease: When and Why to Use Intestinal Ultrasound, MRE, or CTE: Current Guidelines and Future Directions.

Crohn's disease (CD) is a complex inflammatory bowel disease, defined by chronic transmural inflammation and marked heterogeneity in both anatomical distribution and disease behavior, with potential involvement of any segment of the gastrointestinal tract and multiple phenotypes. Advanced cross-sectional imaging nowadays plays a central role in CD management, reliably assessing both luminal and extraluminal inflammatory manifestations, supporting initial diagnosis, phenotypic characterization, and longitudinal monitoring of disease activity, complications and treatment response. Over the last

Does aspirin worsen Crohn's disease outcomes? A multicenter propensity-matched cohort study.

BACKGROUND: Aspirin is widely used for cardiovascular prevention, but its safety in Crohn's disease (CD) is unclear. Because CD involves chronic epithelial disruption and impaired mucosal repair, patients may be especially susceptible to aspirin-related injury. This study evaluated the association between aspirin use and major clinical outcomes in a large CD cohort. METHODS: Using the TriNetX Network, we identified adults with CD and matched aspirin users 1:1 with nonusers using propensity scores incorporating demographics, comorbidities, and CD severity indicators. The primary outcome was al

Autologous platelet-rich fibrin matrix (Obsidian RFT) as a sphincter-preserving treatment for anal fistula: a retrospective cohort study and subgroup analysis.

INTRODUCTION: The treatment of anal fistula remains a delicate balance between achieving healing and preserving anal continence. Sphincter-preserving strategies, such as autologous platelet-rich plasma/fibrin (PRP/PRF) matrices, offer a regenerative approach with minimal morbidity. This study aims to evaluate the efficacy and safety of a specific autologous fibrin matrix system (Obsidian RFT) in the management of anal fistula and to identify predictors of treatment success. METHODS: We conducted a retrospective analysis of patients treated with the Obsidian RFT system for anal fistula at a si

[Fallopian tube in colonic anastomosis after sigmoid resection].

In this case report, we present a rare case in which biopsies from a malignancy-suspect anastomosis after sigmoid resection revealed fimbrial tissue from the fallopian tube. The patient had severe diverticulitis with abscess and underwent resection with primary anastomosis. Persistent atypical endoscopic findings led to repeated biopsies, eventually showing tubal epithelium without malignancy, likely due to fimbrial adherence and incorporation into the anastomosis.

Comparison of incision and drainage procedure with drainage and ligation of the intersphincteric fistula tract in the treatment of deep perianal abscess.

Patients with deep perianal abscess (DPA) and transsphincteric fistula typically require incision and drainage as emergency treatment. However, recurrent abscesses and fistulas are common. This study aimed to evaluate the impact of simultaneous ligation of the intersphincteric fistula tract (LIFT) on abscess recurrence, incontinence, and fistula healing. This study is a retrospective analysis of an institutional protocol and includes 111 patients divided into two groups: Group I (incision and drainage alone, n = 57) and Group II (incision and drainage combined with the LIFT procedure, n = 54).

Use of microfragmented autologous adipose tissue (LIPOGEMS®) as a sphincter-saving therapy for complex anal fistulas: a single-center retrospective study.

BACKGROUND: Complex anal fistulas remain a therapeutic challenge due to high recurrence rates and the risk of incontinence associated with conventional treatments. Microfragmented autologous adipose tissue has emerged as a sphincter-sparing option with potential regenerative and anti-inflammatory properties. This study aimed to evaluate the safety and efficacy of its injection into the fistula tract combined with closure of the internal opening. METHODS: A single-center, retrospective, observational study was conducted between June 2019 and May 2025. All patients with complex anal fistulas tr

Impact of Perianal Inflammatory Lesions in Crohn's Disease Patients with Anorectal Cancer.

OBJECTIVES: Perianal inflammatory lesions (PILs), such as anal fistulas and perianal abscesses, are major complications of perianal Crohn's disease (CD). These lesions reflect inflammation status of CD and pose a risk of carcinogenesis. Their presence may be a risk factor for surgical complications and poor prognosis; however, their impact on CD patients with anorectal cancer is not well understood. METHODS: This nationwide multicenter observational study included patients with CD who underwent abdominoperineal resection or total pelvic exenteration for anorectal cancer. Patients with distant

Percutaneous sigmoid fixation for management of recurrent sigmoid volvulus: a randomized controlled clinical trial.

BACKGROUND: Sigmoid volvulus stands out as a frequent surgical emergency, particularly in older adults. Initial management centers on prompt detorsion of the affected colon segment, coupled with strategies to avoid future episodes. Preventing recurrence remains a persistent clinical challenge. Advances in endoscopic technology have positioned this modality as a pivotal tool in volvulus care, offering the dual advantage of therapeutic detorsion and diagnostic evaluation of predisposing factors. METHODS: We prospectively included all patients who presented to the emergency department at Zagazig

A case of mesenteric lipoma of the small intestine in an adult patient.

INTRODUCTION: Lipomas are the most common benign tumors of adipose tissue, often presenting as slow-growing, painless masses. Although typically harmless, they can occasionally mimic more serious conditions, requiring careful evaluation and differentiation in clinical practice. PRESENTATION OF CASE: We report on a 74-year-old man who underwent surgery for an abscess in the right lobe of the liver, gangrenous-perforated acalculous cholecystitis, and a perivesical abscess. During the revision of the abdominal cavity, a dense formation with enlarged lymph nodes was found on the small intestine n

Current Therapy and Short-Term Results of Open Abdominal Management Among Non-trauma Patients.

Introduction Damage control surgery (DCS) with open abdominal management (OAM) has been established for the treatment of trauma patients; however, in recent years, it has been increasingly expanded to include the management of non-trauma patients with severe conditions. Since 2020, we have performed OAM combined with negative pressure wound therapy (NPWT) for critically ill non-trauma patients. Methods We retrospectively reviewed 43 patients who underwent OAM between April 2020 and March 2024. The primary outcome was in-hospital mortality, and the secondary outcomes were postoperative complica

Rectal Perforation Induced by Taenia saginata: A Case Report.

Taenia saginata, commonly known as the beef tapeworm, is a prevalent zoonotic cestode with an estimated global prevalence of 60-70 million carriers. This parasite is frequently located in the gastrointestinal tract, and previous reports have described complications such as acute appendicitis, Meckel's diverticulitis, pancreatitis, cholecystitis, liver abscess, intestinal obstruction, and perforation. Intestinal perforation due to parasitic diseases is a rare complication. Surgical complications associated with T. saginata are uncommon, with few reported cases of bowel perforation. Here, we pre

The diagnostic and therapeutic value of endoscopic irrigation for cecal suppurative diverticulitis: A case report.

RATIONALE: Cecal diverticulitis closely mimics acute appendicitis in clinical presentation, leading to frequent misdiagnosis. Traditional mistaken appendectomy is not only ineffective but may also delay proper treatment, risking serious complications such as perforation. Abdominal computed tomography plays a key role in differential diagnosis. This report explores the value of endoscopic irrigation as a novel diagnostic and therapeutic approach. PATIENT CONCERNS: A patient presented with symptoms strongly suggestive of acute appendicitis, prompting further diagnostic workup to rule out altern

Intestinal Ultrasound in Inflammatory Bowel Disease: Monitoring Inflammation and Treatment Response.

INTRODUCTION: Inflammatory bowel disease (IBD), including Crohn's disease and Ulcerative Colitis, is a chronic immune-mediated condition that requires long-term monitoring to prevent disease progression and complications. The adoption of treat-to-target strategies has highlighted the limitations of symptom-based assessment and the need for objective tools to evaluate both mucosal and transmural inflammation. In this setting, cross-sectional imaging has gained relevance, and intestinal ultrasound (IUS) has emerged as a noninvasive point-of-care modality for disease monitoring. AIMS AND METHODS
