Nontraumatic perforations of the small intestine.

Источник пометил публикацию как отозванную

Nontraumatic perforation of the small intestine is very rarely found as a cause of abdominal disease. A series of 8 patients admitted to our hospital from 1990-1994 was reviewed. Underlying conditions were typhoid ulcers, (two patients), adhesions (two patients), hemorrhagic ileitis, (one patient), radiation enteritis (one patient) and SLE (one patient). The cause remained unclear in one patient (idiopathic). As surgical therapy, resection and anastomosis were preferred in order to make thorough histologic examination of the perforated bowel possible. One anastomosis leakage with spontaneous c

Perforation of the small bowel: a case report.

Nontraumatic perforation of the small bowel is rare. A 71-year-old man presented with a perforated jejunum 8 weeks after receiving streptokinase therapy for acute myocardial infarction. This was complicated by a bleeding duodenal ulcer, renal failure, ischemia of one toe and confusion. Renal biopsy showed cholesterol emboli. Examination of the resected specimen of jejunum revealed multiple cholesterol emboli but no other disorder. Cholesterol embolization has been associated with angiography, vascular surgery and thrombolytic therapy for myocardial infarction. Acute renal failure and ischemia

Ostomy or intestinal anastomosis in cases of peritonitis.

Twenty-six patients showing peritonitis due to nontraumatic acute abdomen were submitted to ostomy. Mean age was 51 years (range 25-83), being 13 males and 13 females. Bowel obstruction (BO) was the most frequent cause of peritonitis (11 cases), followed by intestinal perforation (IP) (8 cases), acute mesenteric infarction (AMI) (5 cases), and acute abdomen of inflammatory/infectious origin (AAIO) (2 cases). Brook's ileostomy was performed on 65% of the patients. Jejunostomy was performed only in 4 patients, leading to a bad evolution. Overall mortality was 54%. Primary ostomy or anastomosis i

[Non-traumatic perforation of the small intestine].

An overview of a series of patients treated for peritonitis over the last 5 years showed that in 64 cases (6.8%) the infection was due to a non traumatic perforation of small intestine. The predominant aetiology was typhoid fever (39 cases), other causes for the peritonitis were perforation of an abdominal diastasis (10 cases) or a Meckel's diverticulum (8 cases), and perforation due to an acute ileitis (2 cases), a non Hodgkins malignant lymphoma (2 cases), a necrotizing enteritis (2 cases) and a jejunal tuberculoma (1 case). Subcutaneous emphysema of the lower extremity of abdominal origin.

Three cases of subcutaneous emphysema of the lower extremity due to abdominal disease are reported. These were due to (a) perforation of the sigmoid, (b) perirectal abscess, and (c) non-traumatic metastatic gas gangrene due to emphysematous cholecystitis. The mechanisms and anatomical pathways are discussed.

[Non-traumatic perforations of the small intestine].

A series of 23 patients is reviewed admitted to our hospital for non traumatic perforation of the small intestine, a condition found very rarely as a cause of acute abdominal disease. The causes were primary small bowel neoplasia (four patients), adhesions (five patients), intraluminal foreign bodies (two patients), small bowel metastases (two patients), diverticulum (two patients). In seven patients a perforation was found that was neither related to any gross alterations of the bowel wall nor was specifically distinguished in histology. Non-traumatic perforation of colon and rectum.

Free perforation of the colon and rectum is an acute surgical condition. Possible factors affecting the prognosis include age, sex, underlying disease, surgical procedures, duration from onset to surgical treatment, general condition before surgery and degree of contamination. A series of 50 cases with acute, nontraumatic perforation of the colon and rectum treated at Chang Gung Memorial Hospital from 1979 to 1986 were reviewed and studied retrospectively according to these prognostic factors. The mortality rate was highest in the group with colo-rectal cancer (45%). The mortality rate was 50%

Stercoraceous perforation of the colon.

Non-traumatic colon perforations are usually caused by malignancy, diverticulum and colitis. They rarely result from stool impaction. A 74-year-old woman with stercoracous perforation of the colon is reported. She had a long history of constipation and hypertension treated irregularly with cathartics, enemas, diuretics and beta-blockers. Resection of the perforated colon with a proximal diverting colostomy and Hartman's procedure was performed. The patient tolerated the operation and subsequent reanastomosis 6 weeks later. In the case of elderly patients with constipation, early management of

[Non-traumatic colic perforation in free peritoneum. Apropos of 61 cases].

Sixty-one non-traumatic perforations in free peritoneum are reported. The principal aetiology is diverticulitis (35 cases) followed by cancer (22 cases). 18 derivations were carried out, 34 colectomies without anastomosis, 5 ideal colectomies and 4 diverticulectomies. Total mortality was 19.7-14% for diverticulitis, 22.2% for perforations at the cancer site, 50% from diastasic perforations. Stress is laid on the need to treat colic perforation by colectomy without anastomosis, confining derivation openeum. Sixty-one cases.

[Perforations of the colon. Apropos of 74 cases].

74 colic perforations were surgically handled between 1975 and 1988. Among the non-traumatic perforations, which represent 74% of this series, 39 cases (71%) complicated the course of acute diverticulitis, 9 cases (15%) were due to cancers and 7 cases (13%) were of miscellaneous origin. Traumatic perforations (26%) were iatrogenically induced in 84% (16 cases). In 78% the perforation was located in the sigmoid or recto-sigmoid. The treatment consisted in performing either a diverting colostomy (30 cases--40%) or a colic resection (44 cases--60%) with or without immediate anastomosis (respectiv

Analysis of the morbidity, mortality, and cost of colostomy closure in traumatic compared with nontraumatic colorectal diseases.

One hundred sixteen patients with acute colorectal diseases, operated upon emergently and needing an intestinal stoma, were reviewed to determine the cost and morbidity of treatment of patients with colorectal trauma compared to other surgical illnesses. The first group (57 patients) perforating colonic or rectal trauma, the second (30 patients) perforated colonic disease, the third (24 patients) nonperforated colonic disease, and the fourth (five patients) a colonic injury, unrecognized initially but requiring subsequent treatment with a stoma. Nontraumatic perforation of the small bowel.

Small intestinal perforation is an infrequent but often fatal complication of a variety of disorders. Treatment of 76 adults with nontraumatic perforation occurring during a 21 year period was reviewed. The majority of perforations were due to mechanical causes, malignancy, and Crohn's disease. An abdominal mass (53 percent, p less than 0.05) and fever (81 percent, p less than 0.01) were most frequently documented in patients with Crohn's disease, whereas a positive result on fecal occult blood testing (77 percent, p less than 0.025) was common in those with malignant perforations. Otherwise,

[A prospective study of 776 cases of acute non-traumatic abdominal pain. Acute appendicitis and its diagnosis].

776 patients seen in our emergency ward with abdominal pain for less than one week duration were prospectively analysed. In 49% of these patients no cause was found and 19% had acute appendicitis. Among 180 appendectomized patients, 147 (82%) had acute appendicitis whereas 33 (18%) had no inflammation of the appendix. Clinical presentation with a pain duration of less than 36 hours, steady abdominal pain, guarding in the right iliac fossa and a white cell count above 12,000/mm3 (12 g/l) were the best criteria for prediction of acute appendicitis versus a normal appendix. Perforated appendiciti

[Generalized peritonitis of non-traumatic colonic origin. Reflexions on 62 cases].

The authors analyze sixty-two cases of acute generalized peritonitis with a non-traumatic colonic etiology. They point out that the prognosis depends on three main factors: the age of the patient, the time spent in diagnosis and the therapeutic difficulties. They recommend a gastrographin colonic enema to obtain earlier diagnosis, in the absence of radiological evidence of pneumoperitoneum, in elderly patients presenting with abdominal signs. They also recommend multistage surgical management combining effective drainage of the abdominal cavity with resection of the perforated segment of the c

Nontraumatic perforation of the small intestine.

Nontraumatic perforation of the small intestine is very rare in western countries, while typhoid fever is the most common cause of such perforation in other parts of the world. A series spanning 24 years is reviewed, comprising 112 adults with perforation of the small intestine resulting in peritonitis. Perforations due to Crohn's disease, strangulation, postoperative complications and malignancy are not included. Causes of perforation were ingested foreign body (24 cases), radiotherapy (18), potassium chloride medication (13), diverticula of the small bowel (10), Zollinger-Ellison syndrome (2
