A novel technique to characterize Klebsiella pneumoniae populations indicates that mono-colonization is associated with risk of infection.

Klebsiella pneumoniae and related species are a common cause of healthcare-associated infections. The gut is a major Klebsiella reservoir, and gut colonization is a risk factor for developing an extraintestinal Klebsiella infection. Patients can be colonized by multiple Klebsiella strains or even species in the gut simultaneously, and there is high concordance between the gut-colonizing and infection-causing strains. The detection and characterization of colonizing strains is critical for a better understanding of the progression to infection and for developing interventions for colonized pati

Dietary supplement use throughout the colorectal cancer disease trajectory: a prospective cohort study.

PURPOSE: Dietary supplement use is common in the general population, but literature addressing supplement use throughout the disease trajectory in colorectal cancer (CRC) survivors is limited. METHODS: Data from the COLON study, a prospective cohort among participants diagnosed with stage I-IV CRC, were used. Participants were asked to fill out a dietary supplement questionnaire at diagnosis (n = 1934), 6 months post-diagnosis (n = 1769), 2 years post-diagnosis (n = 1533), and 5 years post-diagnosis (n = 1198) containing questions on type, brand, frequency, and doses of supplements. Analysing the implementation and transferability of an enhanced care pathway integrating systematic supportive cancer care and patient-partner support for people living with anal cancer: Protocol for a mixed-methods study in two French cancer centres.

BACKGROUND: Supportive cancer care (SCC) is essential for comprehensive oncology care. It addresses treatment-related adverse effects, reduces long-term sequelae and improves patients' quality of life. Patient-partner (PP) programmes positively impact psychological well-being, patient empowerment and supportive care. Nevertheless, access to SCC remains inconsistent, fragmented and unequally distributed, while PPs are rarely integrated into routine cancer care pathways. Anal cancer is a predominantly localised cancer with high survival rates. Treatment is associated with acute toxicity and ofte

An Exosome-Based Affinity Screening Platform for Identifying Bioactive Compounds against Colon Cancer.

Exosomes, nanoscale extracellular vesicles carrying native membrane proteins, play crucial roles in intercellular communication and are critically involved in colorectal cancer progression and drug resistance, thereby offering a biologically relevant platform for targeted drug discovery. In this study, we developed an innovative immobilized exosome affinity screening platform (Exo@SiO2-NH2) using HT-29 cell-derived exosomes to selectively capture antitumor bioactive compounds from Sophora flavescens. Through UHPLC-MS/MS analysis, four compounds─Kushenol I, Kurarinone, 2'-O-MethylKurarinone, an

Elevating Patient-Family Voices in Necrotizing Enterocolitis.

A diagnosis of necrotizing enterocolitis (NEC) often affects families for a lifetime. Given the profound burden of NEC, families are uniquely positioned to harness the power of their experience to advocate for and contribute to research. Clinicians and scientists can also collaborate with families affected by NEC to improve care as well as prevention and treatment strategies. Elevating the voices of patient-families affected by NEC can help advance the field by helping identify meaningful gaps in science, conveying the urgency of improving outcomes, and providing knowledge only known by famili

Communicating About Necrotizing Enterocolitis in the Neonatal Intensive Care Unit.

Necrotizing enterocolitis (NEC) is a serious, sometimes fatal complication of prematurity and a challenging diagnosis to discuss with parents. In this article, we will discuss communication topics and strategies before, during and after a diagnosis of NEC, including the protective role of mother's own milk, the inability to fully eliminate the risk of NEC, the possibility of fulminant NEC, and long-term sequelae. We argue for clear and transparent communication to partner with and empower families, especially at the time of NEC diagnosis and at key transitions of care throughout the neonatal i

Donor Milk: Practical Considerations for Necrotizing Enterocolitis Reduction.

The practical considerations for using pasteurized donor human milk (PDHM) require a standardized yet individualized approach. Mother's own milk should be prioritized with PDHM serving as a bridge. Neonatal intensive care unit should focus on early feeding and a base diet of human milk MOM supplemented with PDHM. Human milk is critical to reducing necrotizing enterocolitis risk in very preterm infants. Given variability in PDHM composition, strategic fortification is necessary to optimize growth. Reliable milk handling, staff and family education, and shared decision-making with parents are es

Eliminating Disparities and Optimizing Equity in Necrotizing Enterocolitis.

Necrotizing enterocolitis is a devastating complication of prematurity with high mortality and lifelong morbidities. The burden of disease is inequitably distributed, with higher incidence and mortality in racialized, socioeconomically disadvantaged, and geographically marginalized populations. These disparities reflect structural inequities in maternal health, preterm birth risk, quality of care, and access to protective interventions such as mother's own milk and donor human milk. Reducing necrotizing enterocolitis disparities requires multilevel solutions spanning health policy, equitable p

Abdominal Radiograph and Abdominal Ultrasound for Necrotizing Enterocolitis Diagnosis and Management.

Necrotizing enterocolitis (NEC) is one of the most common gastrointestinal emergencies in preterm infants and is a leading cause of morbidity and mortality in this population. Although NEC is localized to the bowel, there are serious associated systemic complications with significant morbidity and mortality. Historically, the diagnostic standard has been plain abdominal radiography. However, the availability of abdominal ultrasound has been emerging as a complementary diagnostic tool allowing for direct visualization of the bowel and abdominal cavity. This article reviews the roles of both mod

Bridging the Policy-practice Gap: Reducing the Burden of Necrotizing Enterocolitis Through Advocacy.

The policy landscape influences both the delivery of health care and patient outcomes. Governmental funding priorities, legislation and regulation impact aspects of neonatal intensive care, which subsequently affect rates of NEC. We review state policies, including Medicaid coverage of pasteurized human donor milk and pending federal legislation such as the reintroduction of the 2019 Access to Donor Milk Act, which aim to reduce disparities in access. We also review federal appropriations, regulations and other measures, which may directly or indirectly, influence NEC rates. Together, these me

Biomarkers and future directions for immunotherapy in microsatellite-stable colorectal cancer: a systematic review.

The mechanisms of resistance to immune checkpoint inhibitors (ICIs) in microsatellite-stable (MSS) colorectal cancer (CRC) have 3 interconnected levels, which provide information to guide different biomarker strategies. There is a critical clinical need, as objective response rates continue to be < 10%. We suggest a biomarker stratification model on the basis of clinical actionability. POLE/POLD1 mutations, recommended by the NCCN Clinical Practice Guidelines in Oncology for Colon Cancer (Version 1.2025), represent true genomic "hard" indicators. While the benefit of ICI-containing combination

KARAP deficiency alleviates intestinal mucosal barrier dysfunction by regulating mitochondrial homeostasis in macrophages.

BACKGROUND: Killer cell activating receptor-associated protein (KARAP) has been implicated in the progression of various inflammatory and immune-mediated diseases, including ulcerative colitis (UC). Although macrophages play a pivotal role in UC pathogenesis, how KARAP regulates macrophage immune responses remains poorly understood. METHODS: In this study, KARAP expression was assessed in peripheral blood mononuclear cells (PBMCs) and colonic tissues from patients with UC using quantitative real-time PCR and immunofluorescence. Dextran sulfate sodium (DSS)-induced colitis was established in K

Maternal risk factors associated with neonatal necrotizing enterocolitis: a meta-analysis and systematic review.

BACKGROUND: Neonatal Necrotizing Enterocolitis (NEC) is a multifactorial intestinal disorder that affects 2-5% of premature infants and has a mortality rate of 20-30%. Maternal factors such as breastmilk feeding and antenatal corticosteroid therapy have been found to influence NEC development in infants, yet other maternal and perinatal contributors have not been analyzed in detail. A better understanding of the role of specific maternal factors in NEC development can help identify infants at risk. This meta-analysis aims to investigate the pooled association between Cesarean section delivery,

Review: optimizing rescue therapy in acute severe ulcerative colitis using infliximab pharmacokinetics-a practical guide for clinicians.

BACKGROUND: Corticosteroid-refractory acute severe ulcerative colitis (ASUC) remains a key management challenge despite an increasing understanding of rescue strategies. Due to the relative paucity of high-quality evidence, the use of infliximab in ASUC should be informed by pragmatic application of key clinical and pharmacological principles. This review synthesizes emerging evidence of infliximab pharmacology, describes key concepts of mechanisms of infliximab failure, and suggests an optimized approach to using infliximab in ASUC. METHODS: Relevant studies investigating infliximab in ASUC

Survival impact of involvement of lymph node stations 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic and periampullary cancers (PANODE): prospective multicentre study.

BACKGROUND: Current guidelines do not recommend routine sampling of lymph node stations (LNS) 16b1, 9, and 8a during pancreatoduodenectomy for pancreatic ductal adenocarcinoma (PDAC) and other periampullary cancers because prospective multicentre studies are lacking. This study evaluated the association between the involvement of these LNS and overall survival (OS). The role of interleukins and immunological parameters in patients with colon cancer: an optical insight.

The progression of colon cancer from benign adenoma to malignant tumors that invade and spread underscores the critical need for accurate and less invasive biomarkers. The primary objective of this work was to assess the diagnostic potential of combining immunological biomarkers and optical polarimetric characteristics in colorectal cancer. We specifically aimed to evaluate serum cytokine profiles as potential diagnostic tools and to explore tissue optical signatures in a preliminary analysis. Secondary objectives included examining postoperative immune changes and exploring the involvement of
