The Overlap Between Crohn's Disease and Intestinal Tuberculosis: A Never-Ending Story.

The prevalence of Crohn's disease has increased over the last few decades, even in developing countries, whereas that of intestinal tuberculosis has decreased, which places both diseases at an epidemiological crossroads. Crohn's disease and intestinal tuberculosis share many clinical, endoscopic, imaging, and pathological features, which sometimes make differential diagnosis very difficult; an accurate diagnosis is, however, very important since an erroneous treatment can worsen the evolution or delay proper therapy. The association between past TB infection and Crohn's disease can make the di

Monomorphic Epitheliotropic Intestinal T-Cell Lymphoma Concomitant with Nontuberculous Mycobacteria Infection: A Case Report.

Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare non-Hodgkin lymphoma originating from mature T lymphocytes within the intestinal epithelium. It is a distinctly heterogeneous malignant tumor with a highly aggressive nature and poor prognosis. Nontuberculous mycobacteria (NTM) are ubiquitous and can infect immunocompromised hosts. NTM infections pose an increased risk of morbidity and mortality in patients with hematological malignancies, with a reported prevalence of 1.4% in those with lymphoma. Here we present a case of a 47-year-old patient, who experienced diarrhea f

Assessment of an in situ hybridization test for the diagnosis of paratuberculosis (Mycobacterium avium subsp. paratuberculosis infection) in goats.

Paratuberculosis (PTB) is a chronic infection caused by Mycobacterium avium subsp. paratuberculosis (MAP). The gold standard test for detection of MAP is culture, which requires 8-16 wk. We reviewed archived intestinal cases of goat PTB and performed RNA scope-based chromogenic pan-mycobacterial in situ hybridization (ISH) on formalin-fixed paraffin-embedded (FFPE) tissue in 20 cases and 5 negative controls. Cases included 11 multibacillary and 9 paucibacillary infections, 2 of which had postmortem autolysis. At the time of diagnosis, we compared the results with ancillary testing (MAP culture

Peritoneal and nodal abdominal tuberculosis presenting as intestinal obstruction mimicking malignancy: a case report.

BACKGROUND: Abdominal tuberculosis (TB) is an uncommon form of extra pulmonary TB that often presents with nonspecific symptoms and poses significant diagnostic challenges. Its clinical and radiologic features can closely mimic malignancy and other abdominal pathologies, frequently leading to delayed diagnosis and management, especially in resource-limited settings. CASE PRESENTATION: We report a 20-year-old Eritrean male presenting with a 1-month history of abdominal pain, fever, weight loss, and later respiratory symptoms. Initial evaluation suggested pneumonia; however, persistent symptoms

Prevalence of Cryptosporidium spp. among sputum smear-positive tuberculosis patients in a tertiary care center in Punjab, India.

BACKGROUND AND OBJECTIVE: Tuberculosis (TB), caused by Mycobacterium tuberculosis, ranks among the deadliest communicable diseases, significantly contributing to global mortality and morbidity. By weakening the immune system, TB increases susceptibility to intestinal parasites like Cryptosporidium spp. These intestinal parasitic infections (IPIs) are widespread, especially in regions with poor sanitation and inadequate hygiene. The problem is particularly acute in developing countries like India, where the co-infection rate of TB and IPIs is notably high. The primary purpose of this study was

Effect of Antitubercular Drug Macozinone (PBTZ169) on the Intestinal Microbiota and Immunity in Inbred Mice.

Inbred BALB/c mice were intragastrically administered with a new antitubercular compound macozinone (PBTZ169) or a combination of isoniazid, rifampicin, and pyrazinamide (HRZ) for six months; control animals received no antitubercular drugs. After 6 months, the microbiota of the large intestine was analyzed. Unlike HRZ combination, macozinone practically does not affect the microbiota: its composition remained the same as in the control group. In lung tissue, differences in the expression of some genes regulating the immune response were found. Two months post-infection with Mycobacterium tube

Rare Complications in Crohn's Disease: Diagnostic and Therapeutic Challenge of Dermal Abscesses Caused by Nontuberculous Mycobacteria Following Infliximab Therapy, and Fluoroquinolone-Related Achilles Tendon Rupture.

BACKGROUND Mycobacterium mucogenicum is a rapidly growing nontuberculous mycobacterium that rarely causes cutaneous disease. Patients receiving tumor necrosis factor-alpha (TNF-alpha) inhibitors are at increased risk for opportunistic and granulomatous infections, including nontuberculous mycobacteria. To our knowledge, this is the first reported case of a cutaneous infection caused by M. mucogenicum in a patient receiving infliximab for inflammatory bowel disease (IBD). CASE REPORT We describe a 57-year-old man with Crohn's disease on infliximab who presented with dermal abscesses, ulceration

Whole transcriptome analysis of peripheral blood identifies systemic innate immune responses and translation inhibition in subclinical Holstein cattle naturally infected with Mycobacterium avium subsp. paratuberculosis.

Bovine paratuberculosis (PTB), caused by Mycobacterium avium subsp. paratuberculosis (MAP), is a chronic granulomatous intestinal disease that leads to substantial economic losses in the global dairy industry. Current diagnostic tests have limited sensitivity, as they can reliably detect only animals that have advanced stages of disease characterized by diffuse lesions and with the presence of clinical signs, but fail to identify those in earlier or subclinical stages, with focal or multifocal lesions in gut tissues. Previous studies have suggested that multifocal granulomas prevent lesions pr

Is the gut a reservoir of non-tuberculous mycobacteria in people with cystic fibrosis?

Non-tuberculous mycobacteria (NTM) represent a significant disease burden in the cystic fibrosis (CF) population. Whilst the acquisition of NTM to the respiratory tract is predominantly attributed to environmental exposures, the contribution of the gut-lung axis to NTM pulmonary disease remains poorly understood. In this study, we retrospectively examined 1285 stool microbiomes from 121 children and 52 adults with CF, and 135 healthy control (HC) children, in order to assess the prevalence, abundance and clinical significance of NTM in the gut. We detected various species of low clinical conce

Pancreatic Tuberculosis Complicated With Duodenal Bulb Fistula Mimics Walled-Off Necrosis: A Case Report.

We described a female patient whose abdominal imaging revealed a suspicious malignant mass in the neck of the pancreas, accompanied by signs of pancreatitis and walled-off necrosis. The diagnosis of granulomatous inflammation was ultimately established via endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB), with subsequent special staining and polymerase chain reaction confirming pancreatic tuberculosis. This case underscores the value of EUS-FNB in obtaining high-quality tissue samples, accurately targeting pancreatic lesions, avoiding unnecessary surgery, and enabling prompt and appro

Adverse Events Associated with Advanced Therapy Compared with Conventional Therapy in Patients with Inflammatory Bowel Disease: A Common Data Model Analysis.

BACKGROUND/AIMS: Although clinical trials have reported adverse events (AEs) associated with advanced therapies (ATs) in patients with inflammatory bowel disease (IBD), the risks of AEs may be underestimated due to the selective nature of the study populations. METHODS: This retrospective, observational study from 13 hospitals using a common data model compared the risks of AEs between patients receiving ATs and those not receiving ATs. Cox proportional hazards models and Kaplan-Meier analyses were conducted following propensity score matching. RESULTS: Patients with IBD receiving ATs exhibi

Basal plasmacytosis and eosinophilia for distinguishing inflammatory bowel disease from gastrointestinal tuberculosis on mucosal biopsy.

AIMS: Accurate distinction of inflammatory bowel disease (IBD) and gastrointestinal tuberculosis (GITB) on mucosal biopsies remains challenging, especially in South-East Asia. Our previous meta-analysis highlighted that mucosal basal plasmacytosis (BP) can help in identifying IBD although evidence is limited. This study was planned to evaluate the utility of mucosal BP and combined presence of BP and mucosal eosinophilia (ME) (BP+ME) in differentiating ulcerative colitis (UC), Crohn's disease (CD) and GITB using endoscopic mucosal biopsies. METHODS: We retrospectively analysed 500 mucosal bio

Mycobacterium avium Subspecies paratuberculosis and Colorectal Cancer: Putting MAP on the Map.

The rising incidence of early-onset colorectal cancer (CRC) remains incompletely explained despite extensive investigation into genetic, environmental and metabolic factors. Emerging evidence suggests that infectious agents may contribute to colorectal carcinogenesis. A recent study by Tehrani et al. demonstrated the presence of Mycobacterium avium subspecies paratuberculosis (MAP) in a majority of colorectal cancer lesions and in approximately half of precancerous lesions, providing a critical epidemiologic anchor. MAP, a zoonotic intracellular pathogen long associated with Crohn's disease, e

Subtle shifts in fecal microbiota diversity during early stages of Mycobacterium avium subsp. paratuberculosis pathogenesis in dairy cattle.

Mycobacterium avium subsp. paratuberculosis (MAP) in cattle causes an intestinal infection that can induce persistent inflammation or tissue remodeling. Several studies have reported differences in microbial diversity between MAP-infected and non-infected ruminants. Still, most of these studies have focused on animals in advanced stages of the disease, whether subclinical or clinical. The present study aimed to describe fecal microbiota composition in dairy cattle during early stages of MAP-associated pathological processes, which remain poorly characterized. A cross-sectional study was conduc

Candidate Faecal microRNAs as Non-Invasive Biomarkers for Bovine Paratuberculosis in Marchigiana Beef Cattle.

MicroRNAs (miRNAs) are small non-coding RNAs that play a crucial role in the post-transcriptional regulation of gene expression. Since miRNAs modulate host immune responses, they represent promising molecular biomarkers of paratuberculosis (PTB), particularly during early or subclinical stages, when conventional diagnostic tests may lack sensitivity. In this context, faecal miRNAs could provide valuable insights into intestinal immune responses and mucosal damage associated with Mycobacterium avium subsp. paratuberculosis (MAP) infection. Although miRNAs have been extensively investigated in s

Peritoneal Tuberculosis Complicated by Abdominal Abscesses: Diagnostic Challenges and Clinical Insights From Two Cases.

Peritoneal tuberculosis (PT) is a rare form of extrapulmonary Mycobacterium tuberculosis infection that may closely mimic Crohn's disease (CD) or intra-abdominal malignancy, leading to diagnostic delay, particularly in endemic areas. We report two illustrative cases complicated by localized abscess formation. The first case involved a 40-year-old woman presenting with chronic right lumbar pain. Computed tomography (CT) revealed a large right psoas abscess associated with circumferential thickening of the cecal base and terminal ileum, along with necrotic mesenteric and iliac lymphadenopathy. C

Isolated jejunal tuberculosis presenting as chronic intestinal obstruction.

Intestinal tuberculosis (TB) most commonly involves the ileo-caecal region. Isolated involvement of the jejunum without concomitant ileo-caecal or colonic disease is exceptionally rare. We report the case of a woman in her early 60s who presented with a 2-year history of postprandial abdominal pain, intermittent constipation, anorexia and weight loss, followed by acute-onset vomiting suggestive of small bowel obstruction. Contrast-enhanced CT demonstrated a short-segment jejunal stricture with proximal dilatation. Enteroscopy confirmed luminal narrowing. Ileo-colonoscopy showed no ileo-caecal

Hepatic and splenic tuberculosis following infliximab therapy for refractory ulcerative colitis: a case report and literature review.

INTRODUCTION: Tumor necrosis factor-α (TNF-α) inhibitors are first-line agents for steroid-refractory/resistant ulcerative colitis (UC), but their immunosuppressive effect elevates the risk of opportunistic infections, particularly tuberculosis (TB). Extrapulmonary TB such as hepatic and splenic TB is rare but prone to misdiagnosis. A 55-year-old male was diagnosed with UC in 2003, relieved by sulfasalazine and mesalazine for recurrence in 2021. In 2024, post-perianal abscess surgery, UC relapsed with positive cytomegalovirus (CMV), treated with glucocorticoids and ganciclovir. The patient sub

Antimicrobial resistance and vaccine-preventable infections: concurrent health security challenges after the double earthquake in Venezuela.

SCIENTIFIC BACKGROUND: Meningiomas are the most common benign intracranial neoplasms with a slow growth presented as the intracranial lesion. These tumors are without any symptoms for a long time. At the time of diagnosis it is frequently an asymptomatic tumor. In that case the therapist may well suggest a wait-and-see strategy. The therapy of meningiomas focuses firstly on the microsurgical treatment. Volume reduction can be achieved immediately after treatment. Stereotactic radiosurgery is an important non-invasive treatment option for recurrent tumors or meningiomas with partial resection.
