The frequency of metastatic involvement of eight 'target' organs from primary adenocarcinomas of the rectum and squamous cell carcinomas of the esophagus correlates significantly with target organ arterial blood flow (ml/min/g) when the primary cancers were in the upper third of the rectum or lower third of the esophagus, where the initial main venous drainage was into the portal system. Twenty patients had their haemorrhoids treated by cryosurgery and were followed up for periods ranging from 6 to 18 months. The main advantages of this technique were that the periods of hospitalization and convalescence were short, reliance on an anesthetists was less, the major symptom of bleeding was eliminated and the period of postoperative discharge not as long as in other reported series. However, patients with third degree haemorrhoids tended to have a persistent skin tag and depigmentation of the perianal skin was not uncommon.

One hundred patients with second- or third-degree haemorrhoids were randomly allocated to haemorrhoidectomy (50) or rubber band ligation (50). Forty-two in each group presented with rectal bleeding; haemorrhoidectomy relieved 36 and rubber band ligation relieved 31 of this symptom. All patients had prolapsing haemorrhoids at presentation. The results of haemorrhoidectomy alone and anal dilatation alone for the treatment of 20 patients with second or third degree haemorrhoids were assessed prospectively over a 12-month period. After haemorrhoidectomy the symptoms of bleeding, lump, pruritus and discharge disappeared, and anal continence was maintained. Thirty-seven cases of second and third degree haemorrhoids have been treated by cryosurgery. Although the number of cases treated is small and the follow-up is short, we are greatly encouraged by the early excellent results. It is a simple and effective procedure with minimal complications, and it is especially recommended for those patients who are medically unfit for general anaesthesia. The rapidity of this procedure combined with the painless operative and postoperative course enables the patient to leave hospital the next day.
