[Stage-adjusted treatment for haemorrhoidal disease].

Haemorrhoidal disease is one of the most frequent disorders in western countries. The aim of individual therapy is eradication of symptoms achieved by normalisation of anatomy and physiology. Treatment is orientated to the stage of the disease: First-degree haemorrhoids are treated conservatively. In addition to high fibre diet, sclerotherapy is used. Haemorrhoids of the 2nd degree prolapse during defecation and return spontaneously. First-line treatment is rubber band ligation. Third-degree haemorrhoids that prolapse during defecation have to be digitally reduced. The majority of these patien

Long-term results after stapled haemorrhoidopexy for third-degree haemorrhoids.

BACKGROUND: Stapled haemorrhoidopexy (SH) is associated with low postoperative pain but, when performed for advanced piles, carries high recurrence rates. The aim of our study was to assess our long-term results after SH for third-degree haemorrhoids. METHODS: A total of 126 consecutive patients (67 men and 59 women) with third-degree haemorrhoids underwent SH in our unit between 1998 and 2002. Of these, 120 (95.2%) were followed up in the outpatient department after a median interval of 61.5 months (range, 38-84 months). RESULTS: During the postoperative period, 7 patients (5.8%) experience

Prospective randomized clinical trial on suction elastic band ligator versus forceps ligator in the treatment of haemorrhoids.

OBJECTIVE: This prospective randomized clinical trial was undertaken to compare the use of a single-operator vacuum suction ligator and the traditional forceps ligator in terms of pain perception following the procedure, intra-procedure bleeding and other complications. METHODS: One hundred consecutive patients with second- and third-degree haemorrhoids presenting between July 2002 and September 2003 were randomized into suction and forceps groups for rubber band ligations. They were equally distributed in both groups, with a mean age of 48.7 years (range, 15-83 years). The immediate, 24-hour

[The Longo and Milligan-Morgan hemorrhoidectomy. A prospective comparative study of 300 patients].

INTRODUCTION: Stapled haemorrhoidectomy according to Longo for treating reducible anal and mucosal prolapse appears to be very simple technically. In order to investigate the feasibility and to delineate the value of this procedure it was compared to standard open haemorrhoidectomy according to Milligan-Morgan in a prospective study. METHOD: In 1998 and 1999, 300 patients with third-degree haemorrhoids were operated on either with a Milligan-Morgan or a Longo technique. Intraoperative and postoperative performance--complications, length of surgery, consumption of analgetics, hospital stay, re

Circumferential mucosectomy (stapled haemorrhoidectomy) versus conventional haemorrhoidectomy: randomised controlled trial.

BACKGROUND: Haemorrhoidectomy is commonly an inpatient procedure because it is frequently associated with postoperative pain. Day case haemorrhoidectomy is a similar operation to that used on inpatients but with different strategies for managing postoperative pain. Circumferential mucosectomy (stapled haemorrhoidectomy) may be associated with less postoperative pain than conventional haemorrhoidectomy. We compared stapled haemorrhoidectomy with conventional haemorrhoidectomy in patients with third degree haemorrhoids. METHODS: We randomly assigned 22 patients to conventional haemorrhoidectomy

A randomized controlled trial of rubber band ligation versus infra-red coagulation in the treatment of internal haemorrhoids.

OBJECTIVE: Despite the presence of numerous non-surgical therapies for the treatment of haemorrhoids, none of these therapies has clearly been proven to be superior. The effectiveness and patient tolerance of rubber band ligation (RBL) and infra-red coagulation (IRC) in the treatment of haemorrhoids was assessed. DESIGN: Prospective randomized trial. SETTING: Academic hospital (tertiary care). PARTICIPANTS: A total of 133 consecutive patients (73 males, 60 females, mean age 48 years (range 19-82)) with internal haemorrhoids, and without concomitant anorectal disease, were randomized to rubb

New thoughts on the aetiology of haemorrhoids and the development of non-operative methods for their management.

Haemorrhoids constitute a frequent clinical problem. A variety of conservative treatment options have been proposed, the majority of which can be safely performed on an outpatient basis. Haemorrhoidectomy, however, remains the ultimate treatment for large, third degree haemorrhoids.

A clinical trial of hydroxyethylrutosides in the treatment of haemorrhoids of pregnancy.

The safety and efficacy of 500 mg O-(beta-hydroxyethyl)rutosides given orally twice daily in the treatment of 97 patients with first-, second-, or third-degree haemorrhoids were investigated in a double-blind, randomized placebo-controlled trial. The rutosides produced a significant (P less than 0.001) improvement in patient-assessed subjective symptoms (pain, bleeding, exudation and pruritus) compared with placebo. There was also a significant (P less than 0.001) improvement in clinician-assessed subjective and objective signs (bleeding, inflammation and dilatation of the haemorrhoidal plexus

Anal dilatation, lateral subcutaneous sphincterotomy and haemorrhoidectomy for the treatment of second and third degree haemorrhoids. A prospective randomized study.

Patients with second or third degree haemorrhoids were randomized for treatment by anal dilatation, lateral subcutaneous sphincterotomy or haemorrhoidectomy. The patients were studied by means of proctoscopy, anal manometry and symptom grading preoperatively, two months and one year after the procedure. After one year, more than half of the patients were symptomless in each treatment group. Both anal dilatation and sphincterotomy gave poor results in 25% of the patients, compared with 9% after haemorrhoidectomy. Young age was related to good results in anal dilatation or sphincterotomy, but no

Maintenance bran therapy for prevention of symptoms after rubber band ligation of third-degree haemorrhoids.

Proctoscopically diagnosed third-degree haemorrhoids were treated with rubber band ligation (RBL) in 92 patients, who were randomly assigned to management with or without twice-daily intake of unprocessed bran for 18 months. RBL was performed maximally five times at 2-week intervals. Patients who were symptom-free 10 weeks after first banding were considered cured and were enrolled in a follow-up study. The two treatment groups did not differ in regard to clinical or proctoscopic characteristics. The number of RBL required for cure was lower in the bran-treated group (p less than 0.01). The in

Manual dilatation of the anus and elastic band ligature: an effective short stay alternative to formal haemorrhoidectomy for prolapsing haemorrhoids.

Fifty patients with second or third degree haemorrhoids were treated by manual dilatation of the anus (MDA) and elastic band ligature under general anaesthesia. All patients had prolapsing haemorrhoids at presentation associated with bleeding in 46, and pain in 39. A final review by questionnaire was undertaken one to three years later, at which time 44 patients remained symptom-free. Forty-three had experienced only mild or moderate discomfort postoperatively. Mean time from operation to pursuing all normal activities was 10 days. MDA and elastic band ligature under GA is a simple, quick and

A prospective study of the effect of haemorrhoidectomy on sphincter function and faecal continence.

The effect of haemorrhoidectomy on anal sphincter manometry and continence to faeces and rectally infused saline was investigated in 24 unselected patients with third-degree haemorrhoids. Haemorrhoidectomy resulted in the abolition of ultra-slow waves and significant decreases in basal and squeeze pressures, though these were reduced to normal values from abnormally high levels. Pressures remained at normal levels up to a year after surgery. An association was found between mucous soiling following surgery and an abnormally low postoperative squeeze pressure. There was no alteration in the fun

How do surgeons treat haemorrhoids? A study with special reference to Lord's procedure.

Forty-eight patients treated for haemorrhoids by Lord's procedure have been followed up for 5 years. Preoperative anal pressure and motility studies suggest that the best long-term results occur in patients with an active internal sphincter. Anal pressure was reduced by dilatation and remained static over the next 5 years. It was not possible to predict which patients would have later recurrence of symptoms from anal pressures measured one year after dilatation. Excellent clinical results were obtained in patients with first- and second-degree haemorrhoids, but in those with third-degree haemo

Lord's procedure for haemorrhoids: a prospective anal pressure study.

Forty-eight patients treated by Lord's procedure were studied prospectively with preoperative and several postoperative anal pressure recordings over at least 5 years. Excellent long term results were obtained in the patients with first and second-degree haemorrhoids but only just under half of those with third-degree haemorrhoids were symptom-free. There was some evidence that patients with the most active internal sphincter had the best clinical results. Pressure was significantly reduced by dilatation and pressure remained static over the next 5 years. Five patients developed recurrent symp

Cryohaemorrhoidectomy-preliminary experience.

Twenty patients had their haemorrhoids treated by cryosurgery and were followed up for periods ranging from 6 to 18 months. The main advantages of this technique were that the periods of hospitalization and convalescence were short, reliance on an anesthetists was less, the major symptom of bleeding was eliminated and the period of postoperative discharge not as long as in other reported series. However, patients with third degree haemorrhoids tended to have a persistent skin tag and depigmentation of the perianal skin was not uncommon.

Comparison of rubber band ligation and haemorrhoidectomy for second- and third-degree haemorrhoids: a prospective clinical trial.

One hundred patients with second- or third-degree haemorrhoids were randomly allocated to haemorrhoidectomy (50) or rubber band ligation (50). Forty-two in each group presented with rectal bleeding; haemorrhoidectomy relieved 36 and rubber band ligation relieved 31 of this symptom. All patients had prolapsing haemorrhoids at presentation. One year after treatment 45 haemorrhoidectomy and 43 rubber band ligation patients were assessed. Haemorrhoidectomy relieved 44 of 45 patients and rubber band ligation relieved 34 of 43 (P < 0.05). Anal dilatation and haemorrhoidectomy. A prospective study.

The results of haemorrhoidectomy alone and anal dilatation alone for the treatment of 20 patients with second or third degree haemorrhoids were assessed prospectively over a 12-month period. After haemorrhoidectomy the symptoms of bleeding, lump, pruritus and discharge disappeared, and anal continence was maintained. Proctoscopy showed the occasional development of secondary piles high in the anal canal. Of those patients having an anal dilation alone, nearly half had symptoms and one had a mucus leak. Anal continence was unalttered in the remainder. Proctoscopy indicated the loss of anal veno
