Targeting Achilles' Heel: Copper-Based Nanoantibiotics Synergistically Disrupt the TCA Cycle to Induce Bacterial Metabolic Cell Death.

Central carbon metabolism is essential for bacterial survival, making it a promising target for antimicrobial strategies. Bacterial cuproptosis-like and ferroptosis-like deaths are tightly linked to central carbon metabolism, yet their complete mechanistic cascades remain poorly understood. In addition, both death pathways are constrained by intracellular glutathione (GSH). Herein, we constructed chitosan-copper nanoclusters (CS-CuCs) as nanoantibiotics that selectively accumulate within bacteria to synergistically disrupt the tricarboxylic acid (TCA) cycle. CS-CuCs induces copper overload, im

Stercoral Ulcer-related Lower Rectal Perforation Associated with Persistent Rectal Fecal Impaction: Three Case Reports and Clinical Implications.

Stercoral ulcer-related lower rectal perforation is an uncommon but potentially under-recognized condition. We report three cases of elderly women with localized lower rectal perforation associated with persistent rectal fecal impaction. All patients presented with hematochezia without generalized peritonitis despite the continued passage of loose or muddy stools. Serial computed tomography revealed persistent fecal impaction in the lower rectum during the clinical course. Continued bowel movement does not necessarily indicate effective rectal evacuation. Recognition of persistent rectal fecal

Aligning policy and clinical guidelines for albumin use in cirrhosis: insights from Indonesia Hepatology Medical Workgroup Meeting.

BACKGROUND: Cirrhosis continues to be a major health burden in Indonesia, with high mortality and morbidity related to complications including ascites, spontaneous bacterial peritonitis (SBP), and hepatorenal syndrome (HRS). Although international guidelines strongly recommend albumin therapy for these complications, in Indonesia the adoption of international guidelines has been limited by universal healthcare policy and albumin indications defined in the National Formulary. The Indonesia Hepatology Medical Workgroup Meeting (December 2025) was convened to address barriers to the implementatio

Proximal jejunal perforation by an ingested toothpick: a rare case report.

INTRODUCTION: Toothpick ingestion is a rare event that may cause gastrointestinal perforation, abscess, or peritonitis. Jejunal involvement is exceptionally uncommon. CASE PRESENTATION: A 45-year-old man, previously operated on for appendicular peritonitis, presented with generalized abdominal pain and vomiting. A CT scan revealed pneumoperitoneum with a linear foreign body perforating the proximal jejunum. Laparoscopy showed diffuse peritonitis and a toothpick perforating the antimesenteric border 70 cm from the duodenojejunal flexure. Conversion to laparotomy was required because of adhesio

A diagnostic cascade from cholecystitis to Meckel's diverticulum rupture: a case report.

BACKGROUND: Meckel's diverticulum (MD) is the most common congenital anomaly of the gastrointestinal tract, yet it rarely becomes symptomatic in adults. Perforation is an uncommon and often overlooked complication that may mimic more frequent abdominal conditions, particularly in the postoperative setting. MD presenting shortly after unrelated laparoscopic surgery is exceptionally rare and can pose a significant diagnostic challenge. CASE PRESENTATION: A 36-year-old woman presented with acute right upper quadrant pain and was diagnosed with calculous cholecystitis. She underwent laparoscopic

Microbacterium resistens Peritoneal Dialysis-Associated Peritonitis: A Case Report.

Peritoneal dialysis (PD)-associated peritonitis is most commonly caused by Gram-positive organisms; however, infections due to unusual pathogens can present important diagnostic and management challenges. Microbacterium resistens is an environmental Gram-positive bacillus rarely implicated in human infections, and PD-associated peritonitis due to this organism has been reported only once previously, to our knowledge. We report a case of persistent PD-associated peritonitis caused by M resistens in a 65-year-old man receiving automated PD combined with once-weekly hemodialysis. The initial epis

Outcomes of modified single trocar laparoscopic catheter placement for maintenance peritoneal dialysis in children.

BACKGROUND: Peritoneal dialysis (PD) access remains a major challenge in pediatric kidney replacement therapy. Although laparoscopic approaches are increasingly favored, pediatric evidence regarding single-trocar placement remains limited. METHODS: This retrospective cohort study included children with kidney failure or advanced chronic kidney disease who underwent a modified single-trocar laparoscopic Tenckhoff catheter insertion between January 2020 and November 2021. All required urgent-start PD on the day of insertion. The technique used a transumbilical 10-mm operative laparoscope, obliq

Contained transverse colonic perforation caused by an ingested fish bone presenting as chronic abdominal pain: a case report.

INTRODUCTION AND IMPORTANCE: Foreign body ingestion is common; however, gastrointestinal perforation is rare and often poses a diagnostic challenge, particularly when symptoms are chronic and nonspecific. Colonic perforation due to ingested foreign bodies is uncommon, with the transverse colon being an especially rare site due to its mobility and wide lumen. CASE PRESENTATION: We report the case of a 19-year-old male who presented with central abdominal pain and intermittent diarrhea for 1 month, followed by acute worsening of abdominal pain, fever, nausea, and vomiting over the proceeding 3

Giant ectopic splenic volvulus: a rare surgical emergency.

BACKGROUND: Splenic volvulus is a rare condition most often associated with a wandering spleen (WS) due to congenital or acquired ligamentous laxity. A giant ectopic splenic volvulus in adults without identifiable risk factors is exceptionally rare. CASE PRESENTATION: We report the case of a 46-year-old woman with no significant past medical history who presented with acute abdominal pain. Computed tomography (CT) revealed a giant ectopic spleen measuring approximately 20 cm, with torsion of the splenic pedicle, consistent with splenic volvulus. Emergency surgery was performed. Successful laparoscopic management of idiopathic sclerosing encapsulating peritonitis: a case report.

INTRODUCTION AND IMPORTANCE: Idiopathic sclerosing encapsulating peritonitis (SEP), also termed "abdominal cocoon syndrome," is an extremely rare cause of intestinal obstruction characterized by the encasement of small-bowel loops within a fibrous membrane. Although SEP has traditionally been treated by laparotomy, laparoscopy has emerged as a safe alternative when feasible. CASE PRESENTATION: We describe a 32-year-old man with no significant past medical history who presented with acute abdominal pain and vomiting. An abdominal computed tomography (CT) scan demonstrated clustered ileal loops

Peritoneal Dialysis-Associated Peritonitis due to Pasteurella multocida: A Rare Case of Stray-Animal Exposure and Fluoroquinolone Resistance.

Pasteurella multocida, a Gram-negative coccobacillus commonly found in the oropharyngeal flora of cats, is a rare cause of peritoneal dialysis-associated peritonitis. We report the case of a 44-year-old patient with exposure to stray cats who presented with abdominal pain and cloudy dialysis effluent. Laboratory tests showed leukocytosis and elevated C-reactive protein (CRP), and we initiated empirical intraperitoneal cefazolin therapy. Peritoneal fluid culture yielded P. multocida, which was resistant to fluoroquinolones but susceptible to penicillin, cefotaxime, trimethoprim-sulfamethoxazole

Laparoscopic and Robotic Appendectomy: Efficiency and Cost Comparison.

Acute appendicitis is a common cause of emergency abdominal surgery, most frequently affecting adolescents and young adults. Diagnosis is often complicated by variable clinical presentations, and timely recognition is critical to prevent complications such as perforation, peritonitis, and intra-abdominal abscess formation. Minimally invasive surgery (MIS) has transformed the operative management of appendicitis with laparoscopic appendectomy (LA) established as the standard operative approach due to reduced surgical trauma, faster recovery, lower complications rates, and shorter hospital stay

Duodenal perforation masquerading as an intra-abdominal abscess: successful management with pedicled falciformopexy.

Duodenal perforation typically presents as an acute abdomen. In contrast, presentation as an intra-abdominal abscess is rare and may delay diagnosis. We report a 25-year-old male who presented with 8 days of upper abdominal pain and 4 days of progressive abdominal distension. Despite marked abdominal distension, he had no signs of peritonitis. Laboratory investigations showed marked leukocytosis. Chest radiography showed no classical subdiaphragmatic free air, whereas computed tomography confirmed pneumoperitoneum. Emergency exploratory laparotomy revealed an intra-abdominal abscess with dense
