Prognostic Model for Survival After Upfront R0 Gastrectomy in Peritoneal Oligometastatic Gastric Cancer.

PURPOSE: Peritoneal metastasis from gastric cancer (GC) is generally considered incurable. Although conversion surgery is the preferred strategy, surgeons frequently encounter clinical scenarios requiring upfront surgery. Currently, objective criteria for predicting prognosis after upfront R0 gastrectomy remain undefined. MATERIALS AND METHODS: This retrospective multicenter cohort study analyzed data from 792 patients derived from the PASS-META cohort and an independent validation dataset collected between 2014 and 2021. The final analysis included 80 patients with peritoneal oligometastasis

A Rare Case of Acquired Haemophilia A Following Sigmoid Adenocarcinoma: Case Report and Literature Review.

Acquired haemophilia A (AHA) is a rare autoimmune bleeding disorder resulting from the development of inhibitory autoantibodies against factor VIII (FVIII). Malignancy is a recognised association, although colorectal cancer remains an uncommon underlying cause. We report a 68-year-old woman with sigmoid adenocarcinoma who developed severe postoperative haemoperitoneum associated with progressive activated partial thromboplastin time prolongation, an initial FVIII activity of 52 IU/dL that subsequently declined to 2 IU/dL, and development of an FVIII inhibitor measuring 9.7 BU. The case highlig

Chicken cathelicidin-3 protected mice from avian pathogenic Escherichia coli infection mediated by triggering chemokine CCL3 pathway.

Host defense peptides (HDPs) possess both direct bacteriocidal and immunomodulatory activities. In this study, chicken cathelicidin-1 (cCATH-1) and chicken cathelicidin-3 (cCATH-3) were selected based on their markedly different minimum inhibitory concentrations (MICs) against avian pathogenic Escherichia coli (APEC) strains, with MICs of cCATH-1 being 4- to 16-fold lower than those of cCATH-3. However, following intraperitoneal challenge with APEC strain E058 (4 × 10⁶ CFU/mouse), 80% of cCATH-3 treated mice survived while 20% of cCATH-1 treated mice survived. Flow cytometry (FC) revealed sign

Peritoneal loose body: an incidental finding, a challenging diagnosis and multimodal assessment to avoid resections.

A man in his early 40s presented with urinary symptoms and was diagnosed with a right ureteric calculus, hydronephrosis and urinary tract infection. A non-contrast CT scan of the kidneys, ureters and bladder revealed an incidental pelvic mass in the rectovesical pouch, initially suspected as a gastrointestinal stromal tumour/fibroma. Multidisciplinary management prioritised cystoscopy-guided double-J stenting for obstruction and infection. In a second admission, ureteroscopic lithotripsy with stenting and a laparoscopic low anterior resection were planned simultaneously. Following ureteroscopi

Novel peritoneal catheters with silver particles lower rates of peritonitis in patients with cirrhosis and refractory ascites.

BACKGROUND: Treatment options for refractory ascites (RA) in cirrhosis are scarce. When TIPS insertion is contraindicated, tunneled peritoneal catheters (cPeCa) offer an alternative treatment. However, cPeCa are associated with high incidences of infection-related complications. Novel PeCa embedded with silver particles (sPeCa) have been introduced to address this issue. METHODS: We evaluated all patients with cirrhosis and RA treated with PeCa or ascites pump at Hannover Medical School from 2012-2023. Analysis was conducted utilizing a multivariable Competing-Risk-Analysis and afterwards a 3

First reported case of peritoneal dialysis-associated peritonitis caused by Citrobacter sedlakii.

Peritonitis remains a significant complication in patients undergoing peritoneal dialysis (PD), often necessitating hospitalization and catheter removal. While gram-positive organisms predominate, gram-negative pathogens, including Citrobacter species, are less commonly implicated. This report describes the first documented case of PD-associated peritonitis caused by Citrobacter sedlakii, a rare gram-negative bacillus of the Enterobacteriaceae family. We detail the clinical presentation, management, and outcomes of a 31-year-old female patient with recurrent peritonitis, highlighting the chall

A Novel Enterococcus Phage Exhibits Potent Anti-biofilm Activity Against Multidrug Resistant Strains and Protects Mice from Peritonitis Sepsis.

Enterococci are opportunistic pathogens that circulate among humans, animals, and the environment, and multidrug-resistant (MDR) enterococci have emerged as a growing One Health concern. In this study, a lytic Enterococcus phage, PEX, was isolated from horse-stable sewage using an equine Enterococcus faecium strain as the host. The antibacterial activity of PEX against MDR Enterococcus isolates of both human and animal origin was systematically investigated, and its therapeutic efficacy was evaluated in both in vitro and in vivo models. Phage PEX was isolated and purified using the double-laye

Tumor-intrinsic IFNγ signaling and niche adaptation drive early colonization in ovarian cancer metastasis.

The mechanisms enabling disseminating tumor cells to survive and colonize peritoneal niches during ovarian cancer metastasis remain incompletely understood. Here, we present MetTag, a single-cell barcoding and transcriptome profiling approach with time-stamped batch identifiers to resolve temporal dynamics of dissemination. Sequencing of MetTag barcodes reveals enrichment of early-disseminated clones across metastatic sites, and targeted depletion of pioneers diminishes the outgrowth of later arriving cells. MetTag-coupled single-cell RNA sequencing uncovers a distinct interferon-gamma-centric

Geographic distribution of physicians and teachers in Japan - what might we learn from the contrast between two workforce allocation systems?

INTRODUCTION: Equitable access to health care and education is recognised as a core human right in both the Constitution of Japan and international frameworks. However, the geographic distribution of professionals providing these services is governed by different institutional arrangements. METHODS: This descriptive study used data from the Japanese Ministry of Health, Labour and Welfare's Statistics of Physicians, Dentists and Pharmacists 2020; the Ministry of Education, Culture, Sports, Science and Technology's School Basic Survey 2023; and the Ministry of Internal Affairs and Communication

STRIVE: interpretable multimodal subtype discovery in Women's cancers using an attention-weighted joint variational autoencoder.

BACKGROUND: Breast, endometrial, and cervical cancers share a common challenge: molecular heterogeneity that single-modality profiling cannot fully resolve. Gene expression and DNA methylation capture distinct and partially non-overlapping regulatory programs, yet most subtyping frameworks treat them independently or merge them without accounting for modality-specific noise and scale. OBJECTIVE: To develop an interpretable multimodal framework that integrates transcriptomic and epigenomic signals for principled molecular subtype discovery and molecular patient stratification with exploratory

Neonatal Gastric Perforation Temporally Associated with Nasogastric Tube Insertion: A Case Report.

BACKGROUND: Neonatal gastric perforation (NGP) is a rare but potentially life-threatening condition associated with substantial morbidity and mortality. Approximately half of reported cases are idiopathic or spontaneous, while the remainder are attributed to identifiable causes, including congenital gastric wall defects, hypoxic-ischemic injury, and iatrogenic trauma related to nasogastric tube (NGT) insertion, particularly in premature and low-birth-weight infants. Early diagnosis and prompt surgical intervention are critical for improving outcomes. We report a case of gastric perforation tem

Spontaneous Neonatal Pneumoperitoneum: Scalpel can be Avoided - A Case Series.

The pneumoperitoneum is usually a surgical emergency in neonates. It may be idiopathic in some cases like in pre‑term and low‑birth‑weight babies. These cases can be managed conservatively and surgery can be avoided. Prudent selection of cases can prevent surgical exploration of neonates. We present three neonatal cases, in which conservative management for pneumoperitoneum was done.

Peptic perforation due to Candida spp.

The incidence of peptic perforation has not decreased despite the successful management of Helicobacter pylori (HP), suggesting other mechanisms are involved in the pathological process. Our findings support the relevance of Candida spp. in these types of perforations. These findings provide insight into the management of peptic perforations complicated by fungal infections and highlight the importance of routinely performing peritoneal culture and biopsies in these patients. We propose that Candida coinfection be considered an independent risk factor in the mortality of patients with peritoni

A superoxide anion-specific chemiluminescent probe for detection of acute inflammation.

Biomarker-responsive chemiluminescence (CL) probes have been developed as a promising technology for noninvasive bioimaging of diseases. Based on the high sensitivity of CL imaging technology, a boron dipyrromethene (BODIPY)-based CL probe (BDPH1) with a hydroxypyridine group was designed for the detection of superoxide anion (O2˙-). The activation of BDPH1 is strictly O2˙- dependent, and the CL signal persists for longer than 40 h, which is significantly longer than the signal duration of traditional CL probes. Moreover, the CL probe can detect the inflammation in a murine peritonitis model.

RORγ Hijacks HIF1α to Promote Peritoneal Metastasis of Gastric Cancer.

UNLABELLED: Peritoneal metastasis (PM) is the most common and lethal form of metastasis in gastric cancer, highlighting an urgent need for treatment strategies. In this study, we identified retinoic acid receptor-related orphan receptor gamma (RORγ) as a key driver of PM in gastric cancer tumors. Integrative bioinformatics and IHC analyses revealed aberrant upregulation of RORγ in gastric cancer tumors, particularly in metastatic lesions. Functionally, elevated RORγ promoted gastric cancer cell adaptation to hypoxia and accelerated PM formation in vivo. The hypoxic microenvironment in gastric

Macrophage MR1 antigen presentation promotes MAIT cell immunity and lung microbiota modulation.

Mucosal-associated invariant T cells (MAIT cells) mediate tissue homeostasis and antimicrobial immunity. However, the cells that express major histocompatibility complex (MHC) class I-related protein 1 (MR1) and present microbial vitamin B-derived antigens (VitBAg) to MAIT cells remain unknown. We found that MR1 expression varied across tissues and cell types. Macrophages from the lung and peritoneal cavity expressed the highest levels of MR1 and were the most efficient at capturing and presenting VitBAg to MAIT cells. Expression of MR1 in macrophages was regulated transcriptionally and induce
