Periaortitis and Retroperitoneal Fibrosis in a Patient With a Normal IgG4 Level: A Case Report and Review of the Literature.

We present a case of IgG4-related periaortitis with retroperitoneal fibrosis (RPF) in a 66-year-old man with a complex medical history, including coronary artery disease, hypertension, and prediabetes. The patient presented with abdominal and flank pain, and imaging revealed inflammatory changes surrounding the abdominal aorta and splenic artery. Treatment with corticosteroids resulted in significant symptomatic improvement. This case is compared with previously reported cases of periaortitis, including those associated with endovascular aortic repair (EVAR) and idiopathic IgG4-related disease

Gastrointestinal complications of cocaine use: a narrative literature review.

BACKGROUND: Cocaine is the second most widely used illicit substance worldwide and exerts potent sympathomimetic effects by inhibiting the reuptake of norepinephrine, dopamine, and serotonin. While cardiovascular and neurological complications are well recognized, gastrointestinal (GI) manifestations are increasingly reported. These arise from multifactorial mechanisms including vasospasm, endothelial dysfunction, thrombosis, ischemia, and direct mucosal toxicity, often with severe clinical consequences. OBJECTIVE: This narrative review summarizes the mechanisms, pharmacokinetics, pharmacodyn

Pharmacological treatment of idiopathic retroperitoneal fibrosis.

Idiopathic retroperitoneal fibrosis (IRF) is a rare disease characterized by inflammatory infiltration around the aorta, which can lead to clinical symptoms and complications, the most common of which is ureteral obstruction, potentially resulting in secondary renal failure. A search of medical databases was conducted to compile a review article summarizing the immunosuppressive treatment options for IRF. In addition to urological treatment, immunosuppressive drugs are used in the treatment of retroperitoneal fibrosis. The cornerstone of therapy remains glucocorticosteroids (GCs), whose effect

Postpartum Retroperitoneal Fibrosis With IVC Obstruction: A Case Report.

Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory condition producing inflammatory masses in the retroperitoneum that can obstruct retroperitoneal structures, including vessels and ureters. RPF typically presents in middle-aged men and may be associated with IgG-4-related disease (IgG-4 RD). This report describes a 34-year-old woman who presented 6 months postpartum with left leg swelling and lower abdominal pain. Contrast-enhanced CT revealed a homogenous retroperitoneal soft-tissue mass encasing the aorta and iliac arteries, with inferior vena cava (IVC) obstruction and collateral v

Treacherous Pitfalls in Liver Transplantation for Budd-Chiari Syndrome: When the Devil Hides in the Shadows-A Case Report.

INTRODUCTION: Budd-Chiari syndrome (BCS) is a rare vascular liver disorder characterized by obstruction of the hepatic venous outflow, involving the hepatic veins and/or the inferior vena cava (IVC). Etiology varies geographically, with thrombophilic conditions and myeloproliferative disorders predominating in Western countries, and membranous IVC obstruction being more frequent in Eastern countries. Liver transplantation (LT) remains the definitive treatment for patients with acute liver failure or advanced disease refractory to medical and interventional therapies, but it presents unique sur

[Rational diagnostics and differential diagnosis in IgG4-related disease].

Immunoglobulin G4 (IgG4)-related diseases are a heterogenous group of chronic inflammatory systemic disorders characterized by fibrosing inflammation with infiltration of IgG4-positive plasma cells. They can affect nearly any organ system. Typical manifestations include autoimmune pancreatitis, sclerosing cholangitis, lymphadenopathy, retroperitoneal fibrosis, and inflammatory orbitopathy as well as involvement of the salivary and lacrimal glands. Each manifestation may present in isolation or in combination with others. Diagnosis requires careful exclusion of malignant or other inflammatory c

Factors associated with elevated serum IgG4 in rheumatic diseases: A retrospective single-center observational study.

Elevated serum immunoglobulin G4 (IgG4) levels have been reported in various rheumatic diseases; however, the factors associated with IgG4 elevation remain incompletely understood. This study aimed to evaluate the frequency of elevated serum IgG4 in different rheumatic diseases and to explore its association with disease classification based on typical inflammatory characteristics and autoantibody status. This retrospective single-center study included 255 patients with rheumatic diseases who underwent serum IgG4 testing between January 2017 and December 2022. Patients were categorized for exp

Simultaneous idiopathic mediastinal and retroperitoneal fibrosis presenting with acute respiratory failure: A case report and literature review.

Simultaneous idiopathic mediastinal and retroperitoneal fibrosis is an exceptionally rare fibro-inflammatory disorder with nonspecific clinical manifestations and challenging imaging findings. A 33-year-old man presented with acute respiratory distress and bilateral obstructive uropathy. Contrast-enhanced CT demonstrated infiltrative soft-tissue masses involving the mediastinum and retroperitoneum, encasing major vessels and the left ureter. CT-guided biopsy confirmed dense collagenous fibrosis without evidence of malignancy. Following exclusion of secondary causes, idiopathic mediastinal and

IgG4-related disease - treatment with anti-CD20, anti-CD19 antibodies, and other new drugs.

BACKGROUND: IgG4-related disease (IgG4-RD) can affect any tissue. Characteristic symptoms include tumor-like swelling of one or more organs, lymphadenopathy, involvement of the pancreatic and/or bile ducts, aortitis and retroperitoneal fibrosis, inflammatory swelling of the lacrimal or salivary glands, and nephropathy. Laboratory changes include hyperproteinemia, increased IgG4 concentration, eosinophilia, and elevated circulating plasmablasts. Presence of any of these symptoms should prompt a differential diagnosis focused on IgG4-RD. GOAL: International recommendations for treatment of IgG4

Erdheim-Chester Disease: A Diagnostic Challenge.

Erdheim-Chester disease (ECD) is a rare, non-Langerhans cell histiocytosis characterized by excessive production and accumulation of histiocytes within various tissues. The discovery of activating mutations within the Mitogen-Activated Protein Kinase (MAPK) pathway has established its neoplastic nature and enabled the use of targeted therapeutic approaches. Despite these advances, ECD remains a diagnostic challenge, often due to its nonspecific symptoms, multisystemic involvement, and consequent delay in diagnosis. We report three cases of ECD illustrating its diverse clinical presentations an

A Cardiomyopathy to Remember: A Case Report of Desmoplakin Cardiomyopathy.

We previously reported that Sildenafil enhances apoptosis and antitumor efficacy of doxorubicin (DOX) while attenuating its cardiotoxic effect in prostate cancer. In the present study, we investigated the mechanism by which sildenafil sensitizes DOX in killing of prostate cancer (PCa) cells, DU145. The death receptor Fas (APO-1 or CD95) induces apoptosis in many carcinoma cells, which is negatively regulated by anti-apoptotic molecules such as FLIP (Fas-associated death domain (FADD) interleukin-1-converting enzyme (FLICE)-like inhibitory protein). Co-treatment of PCa cells with sildenafil and

French protocol for diagnosis and management (guidelines) of IgG4-related disease.

IgG4-related disease (IgG4-RD) is a recently described entity comprising pseudo-tumoral and inflammatory conditions that were previously considered separately. It is characterised by specific histological abnormalities, including polyclonal lymphoplasmacytic infiltration, fibrosis and contingent of IgG4+ plasma cells, often associated with elevated serum IgG4 levels and a good response to glucocorticoids. The clinical presentation is highly heterogeneous and requires a multidisciplinary assessment. The diagnosis is based on the identification of suggestive clinical or radiological signs, such

Retroperitoneal Fibrosis with Pancreatic, Colonic Involvement Mimicking Malignancy: A Case Report.

INTRODUCTION: IgG4-related retroperitoneal fibrosis (IgG4-related RPF) is a rare systemic fibro-inflammatory disorder that may present as a mass-forming lesion, mimicking malignancy. Gastrointestinal involvement is uncommon, and acute large bowel obstruction as the initial presentation is exceedingly rare. CASE PRESENTATION: A 59-year-old male presented with acute abdominal pain, distension, and obstructive symptoms. Colonoscopy identified a stenosing lesion at the left colic flexure, histologically confirmed as a polypoid adenoma. Contrast-enhanced computed tomography (CT) revealed a mass in

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Unusual Causes of Obstructive Acute Kidney Injury: A Case Series.

Obstructive uropathy accounts for about 5%-10% of all cases of acute kidney injury. Unlike the pediatric population, in which congenital anomalies of the urinary tract account for the majority of cases of urinary tract obstruction (UTO), malignancies account for the majority of upper UTO in adults. We report three rare cases of obstructive acute kidney injury. One case was secondary to an inflammatory abdominal aortic aneurysm (IAAA), and two cases were secondary to retroperitoneal fibrosis (RPF). The reason for presenting this case series is the fact that but IAAA and RPF are rare and, becaus
