Cell envelope maintenance by PhoP is essential for Mycobacterium tuberculosis methylglyoxal resistance.

During Mycobacterium tuberculosis infections bacteria are engulfed by macrophages, a main line of defense against invading pathogens. Upon activation, macrophages increase glycolysis, producing the antibacterial aldehyde methylglyoxal. To test whether bacterial methylglyoxal resistance is required for robust infections, we sought to identify M. tuberculosis defense mechanisms against methylglyoxal. We identified phoP mutants were among the most highly sensitive strains to methylglyoxal in vitro. phoP mutants are highly attenuated in mice but a phoP mutant was even more attenuated in mice that

Nationwide longitudinal evaluation of a machine learning approach for enhanced interpretation of Xpert MTB/RIF ultra rifampicin-resistance results in low bacterial load tuberculosis specimens.

BACKGROUND: The World Health Organization (WHO) has identified tuberculosis (TB) as the leading cause of death from a single infectious agent. False-positive rifampicin (RIF) resistance results from the Xpert MTB/RIF Ultra assay are common in TB patients with low bacterial loads, especially among HIV-coinfected individuals. Hence, to distinguish genuine RIF resistance from false-positive results, this study developed and validated an artificial intelligence clinical decision support system (AI-CDSS). METHODS: Between January 2021 and March 2025, Taiwan's national TB reference laboratory recei

NucS Thr74Ile confers mutator phenotype in a globally disseminated clinical Mycobacterium abscessus subsp. abscessus clone.

OBJECTIVE: The emergence of mutations conferring resistance to first-line drugs, macrolides and amikacin, is a major cause of treatment failure in Mycobacterium abscessus complex (MABC) infections; however, mutator strains predisposed to developing drug-resistance mutations remain uncharacterized. NucS, a key DNA mismatch repair protein, maintains mycobacterial genomic stability. This study investigated whether naturally occurring NucS polymorphisms contribute to mutator phenotypes in clinical MABC strains. METHODS: The whole genome data of clinical MABC isolates obtained from public database

A 6- to 9-months oral regimen for rifampicin-resistant tuberculosis: a randomized open-label noninferiority trial in China.

OBJECTIVES: Despite advancements in shorter, oral regimens for drug-resistant tuberculosis, more options are needed to extent patient benefits. We conducted a trial to evaluate the noninferiority of an all-oral regimen compared with the injectable-containing regimen. METHODS: In an open-label, randomized noninferiority trial in China, participants with rifampicin-resistant pulmonary tuberculosis were assigned 1:1 to a 6- to 9-month all-oral regimen (levofloxacin, linezolid, cycloserine, clofazimine and/or pyrazinamide) and a 9-month injectable-containing control regimen. The primary outcome w

Bedaquiline-induced eosinophilic pneumonitis in treatment-refractory mycobacterium avium complex (MAC) lung disease.

We report the first known case of eosinophilic pneumonitis induced by bedaquiline, a drug increasingly used off-label for non-tuberculous mycobacterial infections. While bedaquiline is generally well tolerated, its potential to cause pulmonary hypersensitivity reactions has not been previously reported. This case highlights the need for heightened clinical awareness of novel adverse effects as bedaquiline use expands beyond its original indication for multidrug-resistant tuberculosis. Clinicians should consider eosinophilic pneumonitis in patients developing new respiratory symptoms and periph

ddPCR-based profiling of plasma EV-miRNA identifies a noninvasive biomarker panel for tuberculosis.

BACKGROUND: Tuberculosis (TB) is a top infectious disease killer; underdiagnosis fuels its spread. Thus, finding perspective non-invasive biomarkers for early tuberculosis (TB) is very essential. Extracellular vesicles (EVs) microRNAs (miRNAs) are promising non-invasive biomarkers for tuberculosis (TB) detection. METHODS: To investigate the feasibility of early diagnosis of TB, we examined plasma-derived EV-miRNAs obtained from patients with active TB (n = 25), asymptomatic tuberculosis (aTB) (n = 25), and latent tuberculosis infection (LTBI) (n = 25). Initially, EVs were isolated from plasma

Rv1899c, an HDAC1-ZBTB25-Interacting Protein of Mycobacterium tuberculosis, Promotes Stress Resistance and Immune Evasion in Infected Macrophages.

Rv1899c, a previously identified HDAC1-ZBTB25-interacting protein of Mycobacterium tuberculosis, plays a crucial role in bacterial adaptation and immune modulation. Recombinant M. smegmatis-expressing Rv1899c (MS_ Rv1899c) showed enhanced survival under acidic and oxidative stress compared to vector controls, along with improved early intracellular growth in THP1-derived macrophages. This was accompanied by reduced reactive oxygen species (ROS), diminished cytokines associated with inflammation and downregulation of autophagy proteins ATG5, Beclin, and LC3, which ultimately skewed the immune r

3-Gene-TB-SCORE Accuracy for Tuberculosis Disease Diagnosis Is Not Affected by Immune-Mediated Inflammatory Disease Comorbidity.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains a major global health threat. Approximately one-quarter of the world's population has an Mtb-specific immune response and are classified as having tuberculosis infection (TBI), with a lifelong 5-10% risk of developing TB disease. This risk is elevated in individuals with immune-mediated inflammatory diseases (IMID) undergoing immunosuppressive therapies. To evaluate the diagnostic accuracy of the 3-gene TB-SCORE for TB disease in individuals within the TB spectrum, including those with TBI-IMID in a low TB endemic country,

Validation of Housekeeping Genes for Normalizing RNA Expression in Real-Time PCR in Tuberculomas and Peripheral Blood Mononuclear Cells for Pulmonary Tuberculosis Patients.

Accurate normalization of qRT-PCR data in pulmonary tuberculosis (TB) research requires reference genes whose expression is invariant across clinically relevant matrices, yet no studies have addressed this in lesion tissue and blood concurrently. We assessed the expression stability of eight popular housekeeping genes-ACTB, B2M, GAPDH, HPRT1, PPIA, RPL13A, UBC and YWHAZ-in lung tuberculomas and peripheral blood mononuclear cells (PBMCs) from TB patients. Standardized extraction and amplification yielded Cq values that were ranked by geNorm, NormFinder, BestKeeper and comparative Delta CT, with

The Impact of Body Mass Index on Latent Tuberculosis Infection: Combined Assessment in People Living with HIV.

BACKGROUND: Tuberculosis (TB) is a leading cause of death among people living with HIV (PLHIV). While body mass index (BMI) affects TB risk, its association with latent tuberculosis infection (LTBI) in PLHIV is unclear. High-transmission settings, such as prisons, may further increase LTBI risk, yet this relationship has not been studied across both prison and community populations of PLHIV. METHODS: We conducted a dual cross-sectional study of PLHIV in Jiangsu Province, China, recruiting participants from a prison hospital in 2021 and community healthcare facilities from July to November 202

Fulminant hepatitis during treatment for per-extensively drug-resistant tuberculosis: A case report and call for improved patient monitoring.

RATIONALE: Drug therapy is the most effective therapy for tuberculosis; however, it can also lead to a common and serious adverse reaction known as antituberculosis drug-induced liver injury. The incidence of antituberculosis drug-induced liver injury ranges from 2.0% to 28.0%. PATIENT CONCERNS: Here, we report the successful treatment of drug-induced fulminant hepatitis in a 19-year-old patient with extensively drug-resistant tuberculosis. DIAGNOSES: The patient was treated with a combination of bedaquiline, linezolid, cycloserine, clofazimine, amikacin, ethambutol, and pyrazinamide. This c

An unusual case of cutaneous tuberculosis and paradoxical TB in a patient with spondyloarthritis treated with adalimumab.

Cutaneous tuberculosis (TB) is an uncommon condition, the diagnosis of which can be challenging. Biologic disease-modifying anti-rheumatic drugs (bDMARDs) are associated with a higher risk of opportunistic infections, including TB. We present a case of cutaneous TB in a 56-year-old male with psoriatic arthritis on adalimumab. He developed skin lesions on his thumb and axilla, and Mycobacterium tuberculosis was isolated in cultures sent from a skin biopsy. Four months after commencement of TB treatment, he presented with a new onset of seizures. An MRI head showed two tuberculomas, with no furt

IFN-γ-Driven macrophage responses in the immunity to Mycobacterium tuberculosis and Mycobacterium leprae.

Interferon-gamma (IFN-γ) is a key stimulator of macrophage defense against Mycobacterium tuberculosis (Mtb) and Mycobacterium leprae (M. leprae). Both pathogens adopt measures to circumvent the effects of the immune system, weakening the impact of IFN-γ and enabling them to survive in the cells. This review synthesizes how IFN-γ overdose transacts the JAK/STAT1-IRF1-transmitter to encourage maturation of phagolysosomes, reactive oxygen and nitrogen product generation, LC3-associated phagocytosis (LAP), autophagy, and improved antigen presentation and juxtaposes these pathways in tuberculosis a

Direct RNA sequencing reveals multilayered epitranscriptomic remodeling in macrophages upon Mtb infection.

Understanding the host transcriptional and epitranscriptomic response to Mycobacterium tuberculosis (Mtb) infection is vital for decoding mechanisms of immune evasion and persistence. Here, we employed Oxford Nanopore Technologies (ONT)-based direct RNA sequencing (DRS) on human THP-1 macrophages infected with Mtb. This third-generation sequencing approach enables full-length transcript analysis and simultaneous detection of RNA modifications without reverse transcription or amplification. We uncovered extensive alternative splicing events, widespread shortening of poly(A) tails, and significa

In silico identification of potential inhibitors of Mycobacterium tuberculosis DNA gyrase from Phytoconstituents of Indian medicinal plants.

DNA gyrase, the sole type-II topoisomerase in Mycobacterium tuberculosis, plays a critical role in the bacteria's survival by catalyzing the DNA topology change and is considered a crucial drug target for tuberculosis treatment. The emergence of drug resistance in Mycobacterium tuberculosis, specifically against the second line of drugs (fluoroquinolones) targeting DNA gyrase, necessitates the identification of novel compounds that can effectively interact with the enzyme. In this study, phytochemicals from the Indian medicinal plant database were systematically screened to identify potential

Plasma exosomes reveal insights into bovine tuberculosis pathogenesis and diagnostic opportunities.

Bovine tuberculosis (bTB) is a chronic infectious disease caused by the Mycobacterium bovis (M. bovis). Rapid, cost-effective, and accurate diagnosis of bTB remains a significant clinical challenge globally. In this study, we performed a comprehensive proteomic analysis to evaluate the discriminatory power of plasma and plasma exosomes for bTB diagnosis. We compared protein expression profiles across three groups: M. bovis-negative controls (bTB_N, n = 10), M. bovis-positive cases (bTB_P, n = 10), and co-infected animals (Other_P, n = 10) with Brucella, infectious bovine rhinotracheitis virus
