The geography of waiting: surveillance latency as the hidden driver of Ebola risk in East Africa.

Ebola virus disease remains a persistent public health threat in East Africa, yet preparedness strategies continue to emphasise response after outbreaks are recognised rather than the period preceding detection. This Perspective argues that surveillance latency the cumulative delay between the earliest clinically meaningful signal and organised public health action is a critical but insufficiently measured determinant of Ebola risk. Although diagnostic and reporting delays have been widely documented, they are often treated as isolated operational challenges. We introduce the concept of the ge

Adaptation of Bayesian decision analysis for clinical trial design to binary outcomes: illustration for Ebola virus disease treatment.

When designing trials for high-mortality diseases with limited therapies, the conventional 5% type I error rate (α) may be unsuitable. A higher α could benefit seriously ill patients lacking therapeutic alternatives. Bayesian decision analysis (BDA) integrates multiple health consequences into trial design. We extend BDA for trials with binary outcomes to calculate optimal sample sizes (n) and α in an Ebola outbreak context. We consider fixed, two-arm randomized trials with binary outcomes and two types of clinical trial losses: post-trial loss, for not approving an effective treatment or appr

The vaccination paradox: how East Africa's success in Ebola vaccine preparedness may be creating new public expectations for future outbreaks.

This perspective synthesizes published evidence on Ebola vaccination and outbreak preparedness in Africa. Successful vaccination against Zaire ebolavirus has strengthened vaccine-based outbreak control but may also create a preparedness risk when public expectations exceed species-specific vaccine availability. We propose the Vaccine Expectation Gap, Vaccine Anticipation and Species-Specific Vaccine Literacy as linked, hypothesis-generating constructs, supported by a Preparedness Trust Contract for practical implementation. The framework draws on expectation-disconfirmation and crisis and emer

Resurgence of Ebola in the Democratic Republic of Congo: Lessons Learned and Future Directions.

INTRODUCTION: The Democratic Republic of the Congo (DRC) remains the global epicenter of Ebola virus disease (EVD), having experienced more than 15 outbreaks since the disease was first identified in 1976. The 2025 resurgence in Kasai Province, caused by Orthoebolavirus zairense, resulted in over 60 confirmed cases and a case fatality rate exceeding 65%, highlighting persistent vulnerabilities in outbreak preparedness and response despite previous control successes. This article examines the drivers, response strategies, and public health implications of the 2025 Ebola outbreak. METHODS: This

Metabolic basis of post-infectious sequelae after Ebola virus disease.

INTRODUCTION: Ebola virus disease (EVD) survivors often present with clinical sequelae after acute disease resolution, called post-Ebola syndrome (PES). Why some survivors develop these sequelae and others do not is poorly defined. Altered metabolism has been noted in acute EVD but has not been studied in PES. Here, we identified distinct metabolic pathways in EVD survivors with PES compared to asymptomatic survivors and potential biomarkers of PES. OBJECTIVES: The purpose of this study was to determine if EVD survivors with PES demonstrated differential metabolism compared to asymptomatic EV

From Ebola response to adaptable preparedness beyond a single virus.

The Democratic Republic of the Congo (DRC) has been an important setting for Ebola innovation, including ring vaccination with recombinant vesicular stomatitis virus-based Ebola vaccine and the integration of clinical research into emergency response. These advances strengthened preparedness for Zaire ebolavirus (EBOV), but the 2026 Bundibugyo virus disease (BVD) outbreak highlights a remaining limitation: many preparedness capabilities remain closely linked to individual pathogens. Unlike EBOV, Bundibugyo virus (BDBV) has no licensed vaccine specifically indicated for prevention, and current

Endogenous anti-Ebola virus GP1,2 antibodies interfere with detection of secreted glycoprotein in the blood of EVD patients.

Ebola disease (EBOD) is a highly infectious disease with severe hemorrhagic symptoms. It is caused by orthoebolaviruses, which include the Ebola virus (Orthoebolavirus zairense) (EBOV) causing Ebola virus disease (EVD), the most lethal EBOD to humans. Through co-transcriptional polymerase stuttering and subsequent proteolytic processing, its GP gene produces multiple protein products, including secreted glycoprotein (sGP) and structural membrane-associated trimeric GP1,2. Although sGP is known to circulate in the blood, its physiological significance in disease pathology remains speculative. W

Clinical and Operational Implementation of Ansuvimab under Expanded Access for Ebola Patients in the Democratic Republic of the Congo.

BACKGROUND: Ebola Virus Disease (EVD) is a zoonosis caused by Orthoebolavirus zairense (EBOV). Outbreaks of EVD have occurred in the Democratic Republic of Congo (DRC), and an Expanded Access Program (EAP) with ansuvimab was implemented. OBJECTIVES: The primary objectives were to treat individuals with EBOV infection and to provide post-exposure prophylaxis (PEP) to those with high-risk exposure. STUDY DESIGN: We reported a descriptive expanded-access case series from the 11th, 12th, and 13th EVD outbreaks in the DRC. Following each outbreak declaration, the ansuvimab EAP protocol was update

Dosimetric impact of deep inspiration breath hold (DIBH) technique in right-sided breast cancer radiotherapy: a retrospective analysis.

BACKGROUND AND PURPOSE: While deep-inspiration breath hold (DIBH) is widely used to reduce cardiac dose in left-sided breast cancer radiotherapy, its dosimetric benefits for right-sided cases remain underexplored. This study evaluated DIBH's impact on target coverage and organs at risk (OARs) in right-sided breast cancer, and investigated associations between dose-volume changes, lung volume, and anthropometric indices. METHODS: We retrospectively analyzed 33 patients with right-sided breast cancer treated using DIBH. Seventeen patients received whole-breast (WB) irradiation following post-br

Documenting coral spawning in East Africa: first in situ observations from Zanzibar for three reef-building species.

Broadcast coral spawning is a vital reproductive event for many reef-building species and is essential to the resilience of coral reef ecosystems. Understanding spawning dynamics is key to assessing the reproductive health and resilience of these ecosystems. While extensively documented in the Indo-Pacific and Caribbean, coral spawning remains understudied in the Western Indian Ocean (WIO), a region facing rapid environmental change and increasing coral decline. This study presents the first in situ coral spawning records from Zanzibar for three Scleractinian species: Galaxea astreata, Favites

Identification of pathogenic fungi causing ocular infections using full rRNA operon sequencing with Oxford Nanopore Technologies.

Although fungal eye infections are a major cause of visual impairment worldwide, standard clinical laboratory methods remain slow, insensitive, and limited in their taxonomic resolution. Sequencing of the full ribosomal RNA (rRNA) operon provides a comprehensive marker for fungal identification. In this study, twenty fungal isolates associated with ocular infections were obtained from Srinagarind Hospital, Thailand, and characterized using four identification approaches. Initial hospital-based routine identification relied on conventional morphological methods and matrix-assisted laser desorpt
