Imported Ebola as a stress test of hospital resilience in an era of global connectivity.

In May 2026, an outbreak of Ebola virus disease caused by Bundibugyo ebolavirus emerged in the Democratic Republic of the Congo and spread to Uganda, prompting a WHO Public Health Emergency of International Concern. With no approved vaccine or specific antiviral treatment, Bundibugyo virus poses an acute importation risk in an era of dense global air travel. This perspective frames imported Ebola as a hospital resilience stress test and proposes the Hospital Resilience 4P Framework, organizing preparedness around Prediction, Preparedness, Protection, and Partnership. We critically analyze the

Zoonotic disease knowledge, attitudes, and livestock biosecurity practices among community members in Ada East District, Ghana: a concurrent mixed-methods study.

Zoonotic diseases disproportionately burden settings where humans and livestock coexist, yet community-level knowledge, attitudes, and practices (KAP) remain poorly characterized in West Africa. This study aimed to assess KAP regarding zoonotic diseases and document household biosecurity gaps among livestock-keeping community members in Ada East Distrct in Ghana. A concurrent mixed-methods design was employed, integrating a quantitative cross-sectional survey with a qualitative focus group discussion. Livestock-keeping adults were recruited from five health facilities in Ada East District. A s

Molecular phenotypes of sex and age specific differences relating to outcome in patients with Ebola virus disease.

BACKGROUND: Ebola virus (EBOV) causes a severe, potentially fatal, disease, with outcome likely related to both viral and host factors. There is a clear correlation between viral load and outcome (survival/death) and inflammatory as well as other cellular pathways are differentially activated in the acute stage between patients who go on to survive versus those who go on to die. Age and sex may influence the course of Ebola virus disease (EVD) with potential differences at the molecular level. METHODS: To investigate whether there were age and/or sex-specific differences in patients with EVD,

Identification of host gene transcripts by machine learning and their application to predict outcome in Ebola virus disease.

BACKGROUND: In humans, Ebola virus disease (EVD) has a high mortality rate. While viral load is a known prognostic factor, it is not effective in predicting outcomes across all patients. Exploring host transcriptomic signatures may provide additional predictive value. METHODS: Here, the transcript abundance of four candidate host genes (VCAM1, TUBG1, MS4A4A, ISG15) was quantified by RT-qPCR in blood samples from hospitalised patients with Ebola virus (EBOV) collected during the 2013-2016 West African outbreak. Samples were taken for diagnostic purposes during the acute phase of infection. Ass

Pediatric Ebola Eye Disease in the PREVAIL (Partnership for Research on Ebola Virus in Liberia) III Longitudinal Cohort Study.

PURPOSE: Ebola virus disease (EVD) is associated with vision-threatening ophthalmic sequelae, yet outcomes in young survivors remain poorly characterized. We sought to distinguish ophthalmic pathology in pediatric EVD survivors relative to close contacts and adult EVD survivors. DESIGN: Cross-sectional baseline analysis of participants enrolled in the Partnership for Research on Ebola Virus in Liberia (PREVAIL) III longitudinal cohort study. PARTICIPANTS: Eighty-eight pediatric EVD survivors (ages 6-17 years), 203 pediatric close contacts (ages 6-17 years), and 463 adult EVD survivors (ages

Host-virus miRNA crosstalk in ebola virus disease.

Ebola virus disease (EVD) is a severe and often fatal illness characterized by aggressive viral replication, dysregulated immunity, and multi-organ dysfunction. Emerging evidence demonstrates that microRNAs (miRNAs), both host-derived and virus-encoded, play essential regulatory roles throughout the course of Ebola virus (EBOV) infection. Host miRNAs are widely dysregulated across different tissues and cell types during infection and contribute to antiviral defense, viral persistence, immune suppression, endothelial dysfunction, and hemorrhagic pathology. In parallel, EBOV generates its own mi

Five-year immunogenicity and safety follow-up of the PREVAC randomized Trial of Vaccines for Zaire Ebola Virus Disease.

BACKGROUND: The World Health Organization has expanded its recommendations for prophylactic Ebola vaccination for at-risk populations. Durable vaccine-induced immunity is important for sustaining outbreak preparedness in regions with recurrent Ebola virus disease (EVD). We assessed five-year persistence of vaccine-induced immune responses in adults and children from the PREVAC trial. METHODS: Two large randomised phase 2 trials (NCT02876328), in adults and children aged ≥ 1 year, were conducted in four west African countries. Participants were randomly assigned to placebo or to one of three E

When it could have been prevented: A maternal death from postpartum hemorrhage due to genital tract lesions complicated by urinary fistula in an Ebola treatment unit in DR Congo.

BACKGROUND: Ebola Virus Disease (EVD) increases the risk for complications to a healthy pregnancy and delivery, notably post-partum hemorrhage. Additionally, successful management of post-partum hemorrhage and resource-intensive clinical interventions are difficult in the low-resourced setting of an Ebola Treatment Unit (ETU). CASE PRESENTATION: This report describes the clinical course of a pregnant adolescent, gravida 2 para 1, with a history of delivery by cesarean section only 12 months before her second pregnancy. She was vaccinated with rVSV-ZEBOV following the death of a family member

Development of Peptide Entry Inhibitors Targeting the Endosomal Receptor NPC1 Binding Site of Orthoebolavirus.

Orthoebolavirus causes severe Ebola virus disease (EVD) and deadly outbreaks in humans. This infection occurs through macropinocytosis and trafficking to late endosomes or lysosomes that utilize the receptor Niemann-Pick C1 (NPC1) to enter the cell cytoplasm. We designed peptide inhibitors based on the NPC1 receptor to target the NPC1 binding site to block viral entry. The results indicated that the ligand-based peptide inhibitors showed potent inhibition activities in vitro studies against pseudotyped or replication-competent Orthoebolavirus. Therefore, we further evaluated one of them in a m

Ebola virus disease: A persistent challenge in global health security.

Ebola virus disease (EVD) is a highly fatal zoonotic disease caused by a negative-stranded RNA virus of the genus Ebolavirus in the family Filoviridae. Since its first discovery in 1976 in Central Africa, the disease has triggered several large outbreaks with a mortality rate that can reach 90%. Ebola is transmitted through direct contact with the blood, body fluids, or tissues of infected individuals, including corpses, and can also spread in healthcare facilities if infection control procedures are not properly implemented. Fruit bats of the family Pteropodidae are thought to be the natural

Ebola Virus in Pregnancy.

An Ebola virus outbreak in the Democratic Republic of the Congo has been declared a Public Health Emergency of International Concern. In recent years, substantial advances in evidence-based clinical guidelines, vaccines, and therapeutics have been made, leading to improved outcomes. Our understanding regarding pregnancy outcomes has evolved; maternal mortality rates appear lower than previously believed, but vertical transmission and neonatal death rates remain high. Information gaps persist, particularly regarding infection with Bundibugyo ebolavirus, the species responsible for the current o

Bundibugyo virus detection during an Ebola virus disease outbreak in the democratic Republic of the Congo: an urgent call for action.

Ebola virus disease continues to pose a major public health threat in the Democratic Republic of the Congo, with outbreaks caused by different Ebola virus species, including Bundibugyo virus. We report preliminary information from an ongoing outbreak investigation based on laboratory testing conducted by the National Institute of Biomedical Research. Among suspected cases in Ituri province, 8 of 13 samples tested positive for Ebola virus (Bundibugyo species). The number of deaths reported among suspected cases is still under investigation, and full epidemiological data remain incomplete. These

Identification of Contacts With High Rates of Missed and Asymptomatic Infection Following an Outbreak of Ebola Virus Disease: A Seroepidemiological Study in Likati Health Zone, Democratic Republic of the Congo.

BACKGROUND: Ebola virus disease (EVD) is often regarded as severe and highly fatal, but growing evidence suggests that subclinical or minimally symptomatic infections occur and frequently go undetected. During the 2017 outbreak in Likati Health Zone, only 8 cases were confirmed despite many reported exposures. We evaluated the extent of asymptomatic or unrecognized Ebola virus infection and associated factors among contacts of reported cases. METHODS: In November 2017, we conducted a cross-sectional community-based serosurvey among contacts originally identified through Ministry of Health rec
