BACKGROUND: Acute liver injury (ALI) represents a serious clinical condition characterized by rapid progression toward hepatic functional decompensation, potentially culminating in liver failure. Despite proposed therapeutic interventions, effective management strategies remain limited. Total flavonoids from Apocynum venetum leaves (TFAP), recognized for their antioxidant and anti-inflammatory properties, have demonstrated multi-organ protective effects in prior studies. However, the precise mechanisms underlying TFAP's role in attenuating ALI pathogenesis remain poorly elucidated.

Alcohol-related hepatitis (AH) is a severe, life-threatening liver inflammation caused by chronic heavy drinking, with high short-term mortality despite abstinence and supportive care. The pathophysiology involves a compromised gut-liver axis, activation of Kupffer cells, stimulation of hepatic stellate cells, and progressive fibrosis. Increasing evidence suggests that microRNAs (miRNAs), small non-coding RNAs that regulate gene expression post-transcriptionally, play a role as modulators of these processes. Understanding dysregulated miRNAs in AH may provide insights into novel diagnostic and

INTRODUCTION: Stereotactic ablative radiotherapy (SABR) is routinely used in patients with early stage NSCLC who are not candidates for surgery. Despite excellent local tumor control, it is associated with a high rate of regional and distant recurrences. Combining stereotactic radiotherapy with PD-1 inhibitors has demonstrated encouraging results for patients with curative intent, but how it affects the tumor microenvironment and pathologic response remains unclear.
METHODS: We designed a phase II, open-label, single-arm study aimed to evaluate the efficacy and safety of neoadjuvant nivolumab

Alcohol-associated hepatitis (AH) is a severe and life-threatening form of alcohol-associated liver disease with no approved treatments for reducing long-term mortality. Cytolysin-producing E. faecalis in the gut microbiota of AH patients has been reported as highly correlated to mortality. We investigated whether we could reproduce this correlation in a cohort of Danish patients with AH. Fecal samples from 28 hospitalized patients with AH were analyzed for cytolysin-producing E. faecalis and were followed for 1 y after hospital admission. The primary endpoint was comparison of 180-d mortality

Acute kidney injury (AKI) is present in 30%-40% of hospitalized patients with cirrhosis. Its incidence is higher in patients with severe alcoholic hepatitis, spontaneous bacterial peritonitis, and acute-on-chronic-liver failure (ACLF). Kidney injury is an important landmark event in the natural history of cirrhosis as it is associated with higher mortality. Overwhelming systemic vasodilation, cardiac dysfunction, hypoperfusion, endotoxemia, and direct nephrotoxicity predispose patients with cirrhosis to kidney injury. Infection is present in 25% of patients with decompensated cirrhosis and 35%

Zieve syndrome is a rare alcohol-related disorder characterized by a triad of Coombs-negative hemolytic anemia, cholestatic jaundice, and transient hyperlipidemia. Its recognition is often delayed due to overlapping features with other hepatic pathologies, particularly alcoholic hepatitis. We present the case of a 35-year-old man with a history of alcohol use disorder, autoimmune hepatitis, and malnutrition who presented with jaundice, severe anemia, and hyperlipidemia. Laboratory evaluation revealed hemolytic anemia and cholestasis, and imaging showed hepatomegaly and hepatic steatosis. A cli

Alcohol-related liver disease (ALD) is the most common liver disease worldwide; however, its underlying molecular mechanisms remain poorly understood. Here, we identify ethanol-mediated hemolysis and erythrophagocytosis as major contributors to ALD pathogenesis using both in vitro and in vivo models, as well as surrogate markers such as heme oxygenase-1 (HO-1) and CD163, a scavenger receptor for hemoglobin-haptoglobin complexes. A key initial observation was the direct optical evidence of serum hemolysis in heavy drinkers, which diminished after one week of alcohol withdrawal. In parallel, sol

In Japan, the establishment of diagnostic criteria for acute-on-chronic liver failure (ACLF) in 2022 has increased the focus on alcoholic hepatitis. Most hospitals in Japan lack specialized treatment units or psychiatrists for managing alcohol use disorders, leaving hepatologists to handle various aspects of the disease-a challenging task. This study retrospectively investigated the outcomes of alcoholic hepatitis in a typical Japanese hospital setting, stratified by ACLF diagnosis and other features, with the aim of identifying areas for possible improvement. We conducted a retrospective anal

Acute-on-chronic pancreatitis is frequently complicated by the formation of pancreatic pseudocysts (PPs). Haemorrhage within a pseudocyst is a life-threatening event, typically resulting from the development of a pseudoaneurysm (PA) involving adjacent arterial structures. First-line treatment is endovascular embolization, an interventional radiology (IR) approach. However, if embolization fails, remaining treatment options are severely limited, as surgery is often considered high-risk or unfeasible. We present a case of chronic pancreatitis complicated by a haemorrhagic PP complicated by PA fo

Alcohol-induced liver disease, encompassing steatosis, hepatitis, cirrhosis, and liver failure, is a significant global health burden, affecting 10-35% of individuals with alcohol use disorder. Alcoholic hepatitis, characterized by hepatocyte inflammation due to chronic alcohol consumption, arises from toxic intermediates produced during alcohol metabolism. These intermediates disrupt cellular function, trigger immune responses, and promote fibrosis, leading to cirrhosis and liver failure. Despite current treatments like corticosteroids and liver transplantation, which alleviate symptoms but f

BACKGROUND: The burden of invasive fungal diseases (IFDs) in patients with complicated alcoholic hepatitis (CAH)-defined by ≥ 2 hepatic (ascites, jaundice, liver failure, encephalopathy) or extrahepatic (coagulopathy, shock, kidney or respiratory failure) dysfunctions within 30 days-remains poorly characterised.
AIMS: To assess the burden of IFDs in CAH and compare it with bacterial pneumonia (BP).
METHODS: We conducted a retrospective nationwide cohort study of adult CAH patients in France (2012-2021). The primary exposure were IFDs. The primary outcome was 3-month mortality or liver transp

BACKGROUND AND AIMS: Disrupting liver immune homeostasis drives inflammation. Recent evidence shifts immunoregulatory focus to hepatocytes, though the mechanisms remain poorly defined. Forkhead box O1 (FoxO1) is a critical homeostasis regulator, but its function in liver immune homeostasis is unknown. We aimed to clarify the role of hepatocyte FoxO1 in liver immune homeostasis and inflammation.
APPROACH AND RESULTS: Human liver FoxO1 expression and its association with inflammation were analyzed in patients with various inflammation-related liver diseases. Hepatocyte-specific Foxo1 knockout (

Alcohol-associated liver disease (ALD) is a leading cause of liver-related morbidity and mortality worldwide. Despite growing awareness of its burden, treatment options remain limited, with abstinence as the only widely accepted intervention. Recent research underscores the critical role of the gut-liver axis and nutritional status, particularly dietary protein, in modulating ALD pathogenesis and progression. This review aims to integrate current knowledge on the interplay between gut microbiota, dietary protein, and alcohol-induced liver injury, and to evaluate microbiota-targeted therapeutic

Metallothioneins (MTs) are primarily known for maintaining cellular metal homeostasis and protecting against metal toxicity, but they also play critical roles in mitigating oxidative stress and modulating immune responses. Reduced MTs expression is associated with disease progression in several chronic hepatic disorders. In metabolic dysfunction-associated steatotic liver disease (MASLD), MT1 downregulation has been linked to the transition from simple steatosis to steatohepatitis. Additionally, evidence suggests that individuals with low MTs expression may be more susceptible to obesity, whic

INTRODUCTION: Hepatocellular injury is characterized by elevated levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT). In contrast, alkaline phosphatase levels are usually elevated in cholestatic diseases. Due to the multiples causes, it is crucial to define the etiology for proper management. The most common diagnoses are viral hepatitis, autoimmune hepatitis, Wilson's disease, drug-induced liver injury, acute alcoholic hepatitis, and ischemic hepatitis. Extrahepatic causes such as endocrine and cardiovascular conditions should also be considered.
