Corticosteroids in severe alcohol-related hepatitis: A multicenter randomized clinical trial.

BACKGROUND & AIMS: Severe alcohol-related hepatitis (AH) has high short-term mortality, and corticosteroid therapy is recommended in the absence of contraindications. However, its benefit in patients with early spontaneous bilirubin improvement remains unknown. We aim at determining whether corticosteroid therapy improves survival compared with placebo in this specific population. METHODS: In this multicenter, randomized, placebo-controlled trial conducted in 10 Belgian hospitals (from February 2018 to May 2024), patients aged ≥18 yr with biopsy-proven severe AH and spontaneous bilirubin decl

Association and Mechanism of Coexposure to Perfluoroalkyl and Polyfluoroalkyl Substances and Blood Heavy Metals in Metabolic Dysfunction-Associated Steatotic Liver Disease.

INTRODUCTION: Heavy metals can increase perfluoroalkyl and polyfluoroalkyl substances (PFAS) adsorption. However, no research has examined the relationship of coexposure to PFAS and heavy metals in metabolic dysfunction-associated steatotic liver disease (MAFLD). METHODS: Data were obtained from the National Health and Nutrition Examination Survey database (2017-2018 cycle). The influence of PFAS and heavy metals on MAFLD or controlled attenuation parameter (CAP) was analyzed using logistic or linear regression, weighted quantile sum regression, and Bayesian kernel machine regression. Emerging role of FUS in TGFB1 and COL1A1 transcription dependent on GADD45B to induce NASH-fibrosis.

Fused in sarcoma (FUS), a DNA-RNA binding protein, affects gene transcription, while its role in non-alcoholic steatohepatitis (NASH)-fibrosis is not well understood. In this study, immunohistochemistry and western blot analysis were used to detect the expression of FUS in liver samples from patients with NASH and in LX-2 cells. Immunofluorescence staining showed that FUS co-localized with growth arrest and DNA damage 45β (GADD45B) in hepatic stellate cells (HSCs). Chromatin immunoprecipitation combined with quantitative PCR and luciferase reporter assays were performed to validate the binding

Mitochondrial Calcium Signaling in Hepatocyte Health and Disease.

Calcium (Ca2+) is vital in hepatocyte metabolism and plays a dual role in liver mitochondrial function: Physiological Ca2+ stimulates respiration and mitochondrial dynamics-processes crucial for proper metabolic functioning. However, Ca2+ overload can be catastrophic, leading to mitochondrial dysfunction and the halt of metabolic processes. This dichotomy plays out in liver diseases such as metabolic dysfunction-associated steatohepatitis (MASH) and alcoholic liver disease (ALD), where excess lipid and alcohol, respectively, result in pathological changes in this precarious Ca2+ balance, impai

Two-Step Machine Learning Model Enhances Identification of High-Risk Metabolic Dysfunction-Associated Steatotic Liver Disease.

BACKGROUND: Metabolic dysfunction-associated steatotic liver disease (MASLD) is highly prevalent. Existing non-invasive models for detecting fibrotic metabolic dysfunction-associated steatohepatitis (MASH) perform poorly, partly due to discordant fibrosis-inflammation relationships. We aimed to develop and validate a machine learning-based stepwise strategy to improve identification of high-risk MASLD (F ≥ 2 + NAS ≥ 5). METHODS: A multicenter retrospective cohort study analysed 840 biopsy-proven MASLD patients (after quality control from 934) across eight centres. Logistic regression and mult

Bile salt hydrolase activity as a rational target for MASLD therapy.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is the most prevalent chronic liver disease in the United States, yet therapeutic options remain limited. Emerging evidence implicates the gut‒liver axis and intestinal permeability in disease pathogenesis. Previous studies in animal models and human cell culture indicated that bile salt hydrolases (BSHs), which are gut bacterial enzymes that deconjugate host-derived bile acids, damage intestinal barrier integrity and cause liver damage through the generation of unconjugated bile acids (UBAs). However, the relevance of these find

Genome-wide DNA methylation patterns for indicators of liver steatosis: a longitudinal multiomic study.

BACKGROUND: To identify blood DNA methylation profiles related to liver steatosis, we performed an EWAS on the presence of ultrasonically-identified liver steatosis in the Young Finns Study (YFS) participants (n = 1529, 33-50y.), and on liver enzyme levels and fatty liver index (FLI) across three discovery cohorts: YFS, LURIC (n = 2371, 17-92y.) and KORA FF4 (n = 1872, 39-88y.). We further investigated the discovered associations across the longitudinal subset of YFS (n = 255), encompassing three follow-ups over 32 years, and the three-generational YFS-3G follow-up in 2018-2020. Finally, we ex

Identification of shared oxidative stress related hub genes in NAFLD and atherosclerosis using bioinformatics and machine learning.

Non-alcoholic fatty liver disease (NAFLD) is a chronic hepatic illness linked with metabolic dysfunction that is distinguished by aberrant lipid accumulation in hepatocytes. Emerging evidence indicates an elevated risk of atherosclerosis (AS) in NAFLD patients, but the fundamental biological processes are not well comprehended. The present research aims to uncover co-expressed oxidative stress-related hub genes as well as potential mechanisms shared between NAFLD and AS through bioinformatics and machine learning approaches. Gene expression profiles of NAFLD (GSE89632, Allard JP et al.) and AS

A review of my history of research on ultrasound tissue characterization of chronic liver disease for developing ultrasound clinical technology.

Ultrasound echo signals from organs contain various physical information and numerous studies have been conducted to extract new diagnostic insights from these signals. This article reviews the research and development I have undertaken in diagnosing liver tissue characteristics, reflecting on its historical progression. Initially, texture analysis aimed to identify features of liver fibrosis, but it was found to be more effective for quantifying fatty liver. Consequently, the analysis algorithm and product specifications were refined to address this need. For fatty liver, the importance of ea

Population-based cohort validation and replication reveals limited generalizability of cluster-based MASLD subtypes.

AIMS: MASLD, defined as a steatotic liver disease in the presence of one or more cardiometabolic risk factors and the absence of harmful alcohol intake, exhibits substantial heterogeneity complicating risk stratification. A prior clustering model proposed liver-specific and cardiometabolic subtypes, yet its generalizability and prognostic relevance remain unclear in the broader population. We aim to validate and replicate the prior approach in a nationally representative U.S. MATERIALS AND METHODS: We included 3300 participants with MASLD from NHANES III. For validation, participants were ass

Electroacupuncture as a therapeutic intervention for obesity: mechanisms and implications.

Obesity, a prevalent metabolic disease characterized by chronic systemic inflammation and dysregulated energy homeostasis, contributes to comorbidities such as type 2 diabetes and non-alcoholic fatty liver disease. Despite advancements in lifestyle interventions, pharmacotherapy and bariatric surgery, limitations in efficacy and safety emphasize the necessity for alternative therapeutic interventions. Electroacupuncture (EA), a contemporary form of traditional acupuncture, has emerged as a promising non-invasive intervention for obesity. This review synthesizes current evidence on the neuroend

α-Diversity analysis of hepatic transcriptome reveals distinct pathways in alcohol-associated hepatitis.

Next-generation sequencing can identify previously uncharacterized gene expression patterns in disease. Beyond differentially expressed gene (DEG) analysis, we investigated the ability of within-population diversity (α-diversity) of the transcriptome to reveal additional biological information in alcohol-associated liver disease (ALD), comparing differential Shannon diversity (DSD) to transcriptome heterogeneity changes. RNA sequencing data from normal livers and patients with early ALD and severe AH were analyzed. α-Diversity indices and percentage Shannon diversity of a gene, which refers to

Sex-specific machine learning models for carotid plaque prediction in individuals with fatty liver disease: a cross-sectional study.

INTRODUCTION: Early detection of carotid plaque prevents stroke and myocardial infarction. Individuals with fatty liver might be at an increased risk of developing carotid plaque, yet limited access to carotid artery ultrasound underscores the need for predictive models. AIMS: We aimed to construct six predictive models for males and females separately to predict carotid plaque among individuals with fatty liver disease. DESIGN: A cross-sectional study. DATA SOURCES: We included 8361 participants aged ≥40 years (4871 males; 3490 females) with fatty liver who underwent at least one health ch
