First-line immunotherapy for advanced hepatocellular carcinoma: a network meta-analysis of randomized trials with overall and HBV/HCV-stratified efficacy and safety.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have revolutionized the front-line treatment of advanced hepatocellular carcinoma (HCC). However, the comparative efficacy and safety of different ICI-based regimens-and their consistency across etiologic subgroups [HBV, HCV, and non-HBV/non-HCV (NBNC)]-remain uncertain. This study conducted a Bayesian network meta-analysis (NMA) of recent randomized controlled trials (RCTs) to compare the first-line immunotherapy strategies both overall and by viral etiology. METHODS: A systematic search of PubMed, Embase, the Cochrane Library, and Web of Scien

Immune checkpoint inhibitor-associated hepatitis and colitis: current understanding and clinical approaches.

Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment by harnessing the immune system to target tumors. The introduction of ipilimumab, an anti-CTLA-4 monoclonal antibody, marked a major milestone in immuno-oncology when it was approved by the U.S. Food and Drug Administration (FDA) in 2011 for advanced melanoma. By blocking CTLA-4, ipilimumab enhances T-cell activation and promotes antitumor immunity. Subsequent development and approval of anti-PD-1 and anti-PD-L1 antibodies, such as nivolumab and pembrolizumab, since 2014 have broadened the scope of ICI therapy to various

A shared danger: Amanita mushroom poisoning in a married couple.

We present the case of a married couple intoxicated by mushrooms of the Amanita genus, developing acute toxic hepatitis. The man progressed to severe hepatitis with coagulopathy, requiring Intensive Care Unit admission and plasmapheresis, while the woman had a favorable evolution. Both received treatment with N-acetylcysteine and silymarin, resolving the condition without the need for a transplant. Early diagnosis of this condition and appropriate management are essential to prevent fatal outcomes.

CRISPR screen identifies MAP2K3 as a key target for oleuropein to alleviate deoxynivalenol-induced hepatic injury.

INTRODUCTION: Deoxynivalenol (DON), a widely prevalent mycotoxin in temperate climates, causes significant hepatic injury upon the consumption of contaminated cereals. However, the key targets and mechanisms underlying DON-induced inflammatory hepatic injury remain unclear, thus hindering the development of targeted therapeutics. OBJECTIVES: This study aimed to identify the key host targets mediating DON-induced inflammatory hepatic injury and its underlying molecular mechanism, as well as to discover potential therapeutic agents for alleviating this damage. METHODS: We established a J774A.1

Human leukocyte antigen pharmacogenetics in infectious diseases: anti-infective drug toxicity and host immune response.

Human leukocyte antigen (HLA) polymorphisms are central to anti-infective pharmacogenetics and to inter-individual variability in infection outcomes and vaccine responses, but clinical relevance depends on whether an association changes treatment or prevention decisions. This narrative review is organized around two complementary pillars: first, severe, typically T cell-mediated adverse drug reactions to anti-infective agents, where HLA can support prevention when genetic effect, phenotype precision, and therapeutic alternatives converge; and second, selected, replicated HLA-region association

Immune-mediated hepatitis requiring second-line immunosuppression: how long is required? An observational study.

BACKGROUND: Immune-mediated hepatitis is a common toxicity and accounts for ⁓20% of immune checkpoint inhibitor (ICI) related deaths. There is broad consensus as to the current approach for acute management for immune-mediated hepatitis. There is little data regarding the duration of immunosuppression in severe immune-mediated hepatitis. METHODS: A prospective observational analysis was performed at a specialist oncology hospital in England from 20th May 2018 to 19th April 2024. The need for second-line immunosuppression, the agents used and their duration were analyzed. The primary outcome w

Juglone Attenuates CCl₄-Induced Hepatic Injury in Mice With Associated Modulation of Oxidative Stress and Inflammasome-Related mRNA Expression.

Carbon tetrachloride-induced liver injury is a well-established model of toxic hepatitis, characterized by marked oxidative and inflammatory damage. It remains a relevant experimental system because broadly effective therapeutic options are still limited. Juglone, a natural naphthoquinone derived from Reynoutria japonica, has demonstrated antioxidant and anti-inflammatory potential. However, mouse evidence combining standard liver injury measures with stress- and cell-death gene profiling remains limited. Building on juglone studies in injury and fibrosis, we tested juglone in a CCl4 model. Ma

The utility of whole exome sequencing in diagnosing Wilson disease: A case report.

Wilson disease (WD) is an autosomal recessive disorder of copper metabolism caused by mutations in the ATP7B gene, resulting in toxic copper accumulation in the body. Diagnosis is typically based on biochemistries, including low serum ceruloplasmin and elevated 24-h urine copper excretion, with Kayser-Fleischer (KF) rings being a supportive feature. We describe a 7-year-old girl who presented with isolated elevation of serum alanine aminotransferase (ALT), low serum ceruloplasmin, and presence of nonspecific autoantibodies. Liver biopsy revealed elevated hepatic copper content (241 μg/g dry we

SLIT2 as a key regulator and therapeutic target in liver injury.

Drug-induced liver injury accounts for approximately 10% of acute hepatitis and up to 50% of acute liver failure. Despite its clinical significance, treatment remains largely limited to cessation of the offending agent. SLIT/ROBO signaling, known for roles in organ development, angiogenesis, leukocyte migration, and cancer metastasis, has demonstrated protective effects against various organ damage. In mouse models of liver injury induced by acetaminophen (APAP), thioacetamide, bile duct ligation, and serum from patients with toxic liver disease, Slit2 expression significantly increases, while

Endoplasmic reticulum stress enhances unsaturated fatty acid-induced hepatocyte injury in human pluripotent stem cell-derived hepatic culture.

BACKGROUND: The liver is a central organ involved in lipid metabolism. Hepatocytes synthesize fatty acids and triglycerides under conditions of excess energy while decomposing fats by β-oxidation under conditions of energy deficiency. When fatty acid metabolism is impaired due to metabolic syndrome, excessive lipid accumulation in the liver increases the risk of fatty liver disease, chronic hepatitis, and liver cancer. Excessive fat accumulation causes various types of cell damage, such as oxidative stress, which causes lipotoxicity. There is a difference in lipotoxicity between saturated and

Nanotechnology-driven biomaterials for chronic liver diseases: Stage-specific strategies for advanced theranostics.

Chronic liver diseases (CLDs), encompassing a spectrum from steatosis and inflammation to fibrosis, cirrhosis, represent a major global health burden, causing approximately 2 million deaths annually [1]. The management of CLDs is significantly hampered by the limitations of conventional approaches, including non-targeted drug delivery, systemic toxicity, and inadequate diagnostic sensitivity for early-stage lesions. Nanotechnology-driven biomaterial platforms have emerged as pioneering solutions to these challenges, enabling precise theranostic strategies tailored to the distinct pathophysiolo

A retrospective comparative study of tenofovir alafenamide and tenofovir disoproxil fumarate in chronic hepatitis B patients: Renal and bone safety versus antiviral efficacy.

Long-term nucleos(t)ide analogue (NA) therapy is required for chronic hepatitis B (CHB), and drug-related renal and bone toxicities remain a major clinical concern. The real-world differences in renal and bone safety between tenofovir alafenamide (TAF) and tenofovir disoproxil fumarate (TDF), as well as their relationship with antiviral efficacy, need further validation in local populations. We conducted a single-center retrospective cohort study enrolling 102 adult CHB patients (TAF, n = 52; TDF, n = 50) who initiated therapy within the past 3 years and were followed up for 6 and 12 months. Identification, purification and characterization of mast cells in murine liver fibrosis: optimized methods, expression signatures and correlation with severity.

INTRODUCTION: Mast cells (MCs) are myeloid cells of the innate immune system. As a first line of defense, they fulfill effector functions and immune modulatory properties. Upon activation, they release pro-inflammatory mediators such as cytokines and proteases. It has been suggested that MCs may contribute to the development of liver fibrosis. However, investigating hepatic MC biology in mice is challenging due to low MC numbers and a lack of suitable detection techniques relying on MC proteins and their modifications. Here, we evaluated whether the expression strength of MC markers correlates

Circulating exhausted CD8+ effector memory cells differentiate immune checkpoint inhibitor-induced liver injury from other acute immune-mediated liver injuries.

BACKGROUND: Checkpoint inhibitor-induced liver injury (ChILI) is an immune-related adverse reaction, occurring in patients with cancer receiving immune checkpoint inhibitors (CPI). ChILI is currently managed with high doses of corticosteroids which carry their own risks and potential side effects, and the lack of available biomarkers makes monitoring patients at risk of developing ChILI a challenge. There is no specific test that distinguishes ChILI from other competing diagnoses such as acute autoimmune hepatitis (AIH) and idiosyncratic drug-induced liver injury (DILI) due to other medication

FXR overexpression restores NLRP3-mediated mitophagy and improves mitochondrial dysfunction in alcoholic liver disease.

Alcoholic liver disease (ALD) is a common chronic liver disease worldwide, directly caused by excessive and prolonged alcohol consumption. To date, there are no acknowledged therapeutic approaches for treating ALD. The reason is that ALD pathogenesis is multifactorial and only partially understood. Mitochondrial dysfunction-related mitophagy and inflammation are essential factors that play critical roles in the pathogenesis and progression of ALD. The farnesoid X receptor (FXR), a member of the nuclear receptor superfamily, plays a well-established role in liver protection, but whether and how
