Alcohol consumption: from biochemistry to behavior, health, and beyond.

Alcohol metabolism is a multi-step biochemical process driven primarily by hepatic oxidation of ethanol to acetaldehyde and then to acetate. Although this pathway protects the body from prolonged ethanol exposure, transient acetaldehyde accumulation generates substantial toxicity, and chronic alcohol intake activates auxiliary pathways that amplify oxidative stress, inflammation, and tissue injury. Both metabolic capacity and physiological response to alcohol vary widely between individuals, reflecting genetic and epigenetic variation in the alcohol- and aldehyde-dehydrogenase enzymes, the cyt

First-line durvalumab and tremelimumab in poor-prognosis unresectable hepatocellular carcinoma: Phase IIIb single-arm SIERRA study.

BACKGROUND & AIMS: Clinical trials in unresectable hepatocellular carcinoma (uHCC) often exclude patients with poor prognostic factors, including impaired liver function, poor functional status or main trunk portal vein thrombosis (Vp4). The dual immune checkpoint inhibitor regimen STRIDE (Single Tremelimumab Regular Interval Durvalumab) is a standard-of-care treatment for first-line uHCC. The phase IIIb SIERRA (NCT05883644) study previously reported early safety of STRIDE in participants of poor prognosis. Here, we present safety and efficacy results from the primary analysis of the SIERRA st

Stratifying Cardiovascular Risk in Patients With HCC Receiving Atezolizumab/Bevacizumab: A Comparison of ESC and CARDIOSOR Scores.

BACKGROUND & AIMS: The management of comorbidities has become increasingly relevant in unresectable hepatocellular carcinoma (uHCC) as immunotherapy prolongs survival. Atezolizumab plus bevacizumab (AB) is a standard first-line option, but bevacizumab carries a risk of cardiovascular toxicities. Data on cardiovascular risk stratification in patients with cirrhosis remain limited. Identifying patients at increased cardiovascular risk is therefore clinically important, particularly given the availability of bevacizumab-free alternatives. METHODS: We retrospectively analysed prospectively collec

Microwave ablation versus stereotactic body radiotherapy for oligometastatic colorectal liver metastases (LAVA-CRLM): a prospective, randomised, phase II trial.

BACKGROUND: Local therapies can achieve durable disease control in selected patients with oligometastatic colorectal liver metastases. Microwave ablation (MWA) is widely used, whereas stereotactic body radiotherapy (SBRT) is a non-invasive alternative with limited prospective comparative evidence. We aimed to compare freedom from local progression and safety of MWA versus SBRT. METHODS: LAVA-CRLM was a randomised, phase II trial conducted at two centres in Copenhagen, Denmark. Adults with one to three colorectal liver metastases, each ≤4.0 cm, deemed suitable for both MWA and SBRT with curati

Multidisciplinary treatment incorporating autologous DC-CIK cell therapy induces long-term survival in advanced pancreatic cancer: a case report.

Pancreatic cancer is widely acknowledged as one of the most aggressive malignancies of the digestive system. More than 80% of patients are diagnosed at an advanced stage with an extremely poor prognosis. While contemporary combination chemotherapy and the novel RAS inhibitor daraxonrasib have modestly prolonged survival, clinical benefits remain limited, necessitating less toxic yet more substantial and durable therapeutic strategies. Herein, we report a case of a patient with advanced metastatic adenocarcinoma who achieved disease stability and long-term survival following multidisciplinary t

Pediatric Guillain-Barré Syndrome Post Organ Transplant: Acute and Chronic Complication Requiring High Index of Suspicion.

BACKGROUND: Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy most often triggered by infections such as Campylobacter jejuni, Epstein-Barr virus (EBV), or cytomegalovirus (CMV). Although transplant-associated GBS is recognized in adults, it remains rare in pediatric recipients, and distinguishing immune-mediated disease from medication toxicity or critical illness can be challenging. METHODS: We describe two pediatric cases of GBS following organ transplantation, one after liver transplant and one after small bowel transplant. Clinical features, diagnostic eva

Liposomal encapsulation redirects the in vivo disposition of paclitaxel: a comparative pharmacokinetic, tissue distribution, and excretion study in rats.

Paclitaxel (PTX) is a potent chemotherapeutic agent for solid malignancies. However, its poor aqueous solubility necessitates toxic solubilizers in conventional formulations, which lead to adverse reactions and unpredictable pharmacokinetics. Liposomal encapsulation offers a biocompatible alternative to overcome these limitations. In this study, a sensitive ultrafiltration coupled with ultra-performance liquid chromatography-tandem mass spectrometry method was developed and validated for the simultaneous determination of free and total PTX in biological samples. This method was applied to comp

Polymeric alpha-1 antitrypsin perturbs Paneth cell proteostasis and exacerbates intestinal inflammation.

Alpha-1 antitrypsin (AAT) is a serine protease inhibitor that protects tissue from neutrophil elastase and other proteases, particularly in the lung. Mutations in SERPINA1, including the Z mutation, lead to AAT deficiency (AATD), characterized by reduced circulating AAT and increased risk of pulmonary emphysema, liver cirrhosis, and hepatocellular carcinoma. Beyond these well-characterized manifestations, AATD has been associated with panniculitis, rheumatoid arthritis, and glomerulonephritis. Emerging evidence has also suggested a link between AATD and inflammatory bowel diseases (IBDs), alth

Organophosphate flame retardants exposure disrupts serum lipid metabolism and induces hepatotoxicity: Evidence from the population and mouse model.

Organophosphate flame retardants (OPFRs) are emerging pollutants that pose increasing health risks. The association between urinary metabolites of OPFRs (m-OPFRs) and lipid metabolism remains unclear. This study evaluated lipid metabolism disorders of OPFRs in a population-based observational study and C57BL/6 mice. Urinary m-OPFRs, serum lipid molecules, and biochemical biomarkers in serum were measured. Linear regression and quantile-based g-computation analyses assessed associations between m-OPFRs and lipid markers. Benchmark dose and 95% lower confidence limit (BMDL) for m-OPFRs were dete

Evaluating Associations Between Cumulative Use of Chronic Liver Injury-Inducing Antiretroviral Therapy and Hepatocellular Carcinoma, By Chronic Liver Disease Status.

BACKGROUND: Prolonged exposure to hepatotoxic antiretroviral therapy (ART) could lead to chronic liver injury, which might increase risk of hepatocellular carcinoma (HCC). We used causal inference methods to evaluate if cumulative exposure to ART with known mechanisms of chronic liver injury is associated with HCC among people with HIV (PWH), by chronic liver disease (CLD) status. METHODS: We conducted a cohort study of antiretroviral-naïve PWH with and without diagnosed CLD who initiated ART in the Veterans Affairs (VA) between 1999-2023. Exposure to ART with known mechanism of chronic liver

Hepatocyte CD146 and soluble CD146 promote hepatic stellate cell activation via the integrin αvβ1-p38 MAPK signaling axis to drive liver fibrosis.

During chronic liver injury, hepatocytes promote liver fibrosis by interacting with hepatic stellate cells (HSCs). However, the key intracellular factors that regulate intercellular communication in hepatocytes remain poorly understood. Although cluster of differentiation 146 (CD146) is predominantly recognized as an endothelial cell marker, our study found that CD146 is substantially upregulated in hepatocytes under fibrotic stress and may play an independent role in transcellular signaling. This study aimed to elucidate the functions and underlying mechanisms of hepatocyte CD146 in liver fib

Synergistic Effect of Newcastle Disease Virus and Hydroxyurea in Apoptosis of Human Hepatocellular Carcinoma Cells (HepG2).

INTRODUCTION: Despite widespread use, chemotherapy often leads to adverse effects and resistance. Hydroxyurea (HU) is commonly used in combination with other chemotherapeutic agents. Efforts have been made to enhance its efficacy and reduce associated toxicity. Combination therapy appears to be the most effective approach for targeting cancer cells through multiple mechanisms. Newcastle disease virus (NDV) has recently been investigated as a potential cancer therapy. This is the first study to evaluate the combined cytotoxicity of NDV and HU in HepG2 cells. MATERIALS AND METHODS: The effects

Cuproptosis and copper metabolism regulate hepatocellular carcinoma progression and therapeutic response to sorafenib.

Hepatocellular carcinoma (HCC) remains a leading cause of cancer-related death, and systemic therapy with sorafenib is limited by primary and acquired resistance. Copper is now recognized both as a growth-promoting cofactor in HCC and as the trigger of cuproptosis, a copper-dependent form of regulated cell death executed through ferredoxin-1 (FDX1)-mediated reduction of Cu2+, aggregation of lipoylated tricarboxylic acid (TCA) cycle proteins such as dihydrolipoamide S-acetyltransferase (DLAT), loss of Fe-S cluster proteins and proteotoxic mitochondrial stress. This review is deliberately restri

Body Mass Index-Related Changes in Tibial Morphology: Considerations for Intramedullary Nailing.

Glypican-3 (GPC3) mRNA and protein are expressed in >80% of human hepatocellular carcinomas (HCC) but not in normal tissues except for placenta and fetal liver. The oncofetal antigen GPC3 is a glycosylphosphatidyl inositol-anchored membrane protein and may be secreted. It is a novel tumor marker for human HCC: GPC3 protein was present in sera from 40-50% of HCC patients, but was not detected in sera from patients with liver cirrhosis or chronic hepatitis, or in sera from healthy individuals. alpha-Fetoprotein (AFP) and PIVKA-II (protein induced by vitamin K absence or antagonist-II), are well

Clodronate: The Influence on ATP Purinergic Signaling.

ATP is involved in numerous physiological functions, such as neurotransmission, modulation, and secretion, as well as in cell proliferation, differentiation, and death. While ATP serves an essential intracellular role as a source of energy, it behaves differently in the extracellular environment, where it acts as a signaling molecule capable of activating specific purinergic receptors (P2YRs and P2XRs) that modulate the response to ATP. Extracellular ATP signaling is a dynamic area of research, with particular interest in ATP's effects on inflammatory conditions and pain modulation. Clodronate
