Differentiating Autoimmune Hepatitis From Sulfasalazine Toxicity: The Value of Clinicopathologic Correlation.

Sulfasalazine is one of the commonly used disease-modifying anti-rheumatic drugs (DMARDs) for many inflammatory and autoimmune illnesses. Sulfasalazine is generally safe with consistent monitoring, as it can rarely cause clinically significant drug‑induced liver injury (DILI). Diagnosing DILI can be challenging in patients with autoimmune diseases due to positive baseline autoantibodies that may mimic autoimmune hepatitis (AIH) or lupus‑associated hepatitis. This case highlights severe hepatocellular DILI temporally associated with sulfasalazine exposure in a patient with Sjögren's disease and

Dynamic Granulocyte Turnover Inferred From cfDNA Methylation Predicts Immunotherapy Benefit in Liver-Metastatic Small Cell Lung Cancer: A Prospective Biomarker Study.

Liver metastases (LM) in small cell lung cancer (SCLC) are associated with a limited response to immune checkpoint inhibitors (ICIs). In this trial, we evaluated the efficacy of first-line envafolimab plus chemotherapy in SCLC patients with LM and explored potential predictive biomarkers. Patients received envafolimab in combination with chemotherapy for four cycles, followed by envafolimab monotherapy until disease progression or unacceptable toxicity. The primary endpoint was progression-free survival (PFS). Secondary endpoints included safety, objective response, overall survival, and ident

New-Onset Convulsive Status Epilepticus Temporally Associated With Accidental Inhalation of Billygoat Weed (Ageratum conyzoides) Smoke: A Case Report.

Status epilepticus is most commonly associated with structural, metabolic, infectious, autoimmune, or toxic etiologies. Plant-related toxic exposures are uncommon causes of new-onset status epilepticus and remain poorly characterized. Ageratum conyzoides (billygoat weed) is an invasive plant widely distributed in tropical and subtropical regions and is frequently used in traditional medicine. We report the case of a 76-year-old male with no prior history of epilepsy, cerebrovascular disease, dementia, head injury, alcohol use, or substance abuse who developed generalized convulsive status epil

The global status of aflatoxin M1 in cow milk 1990-2025: An assessment of occurrence and dietary exposure.

Aflatoxin B1 (AFB1), a carcinogenic mycotoxin in food and feed, is bio-transformed in the mammalian liver to aflatoxin M1 (AFM1), a potentially toxic metabolite excreted in cow milk. Hence, humans are exposed to AFM1 through dairy consumption. Despite its toxicity, numerous studies continue to report AFM1 occurrence levels (milk-borne concentrations) above the safety limits, indicating persistent exposure risks. A consolidated data set of global AFM1 occurrence levels spanning from 1990 to 2025, coupled with a data driven assessment of trends in AFM1 occurrence and exposure is not available in

Decoding circRNA-miRNA crosstalk in the pathogenesis of hepatic fibrosis: A comprehensive review.

Hepatic fibrosis is a pathological condition characterized by persistent activation of hepatic stellate cells and the deposition of extracellular matrix components. It can occur due to various reasons, including viral infection, metabolic dysfunction, radiation exposure, cholestatic disorders, and environmental toxicants. Emerging evidence indicates that circular RNAs (circRNAs) strongly influence fibrosis by modulating microRNA (miRNA) activity and downstream signaling pathways, yet little research has been done on this topic. This comprehensive review systematically evaluates and synthesizes

Risk and mechanisms of artificial sweetener-induced metabolic dysfunction-associated steatotic liver disease: Insights from multi-level bioinformatics and in vitro assays.

The association between artificial sweeteners (ASs), which are widely used sugar substitutes, and metabolic dysfunction-associated steatotic liver disease (MASLD) remains unclear. In this study, we employed an integrated approach to investigate the potential risk and mechanisms of AS-induced MASLD. Using network toxicology, we predicted 534 potential overlapping targets of common ASs (acesulfame, aspartame, cyclamate, neotame, saccharin, and sucralose) and MASLD. Enrichment analysis revealed that these targets participated in multiple pathways, including metabolic regulation, inflammatory resp

Deciphering and improving human homogentisate 1,2-dioxygenase function through knowledge gaining directed evolution: implications for alkaptonuria.

Human homogentisate 1,2-dioxygenase (HGD) catalyses the oxidative cleavage of homogentisic acid (HGA) to maleylacetoacetate (MAA), a key step in tyrosine degradation. Loss of HGD activity causes alkaptonuria (AKU), a rare inherited metabolic disorder characterized by toxic HGA accumulation. Current therapy with nitisinone lowers HGA levels but does not restore HGD function, motivating further investigation of HGD structure-function relationships. In this study, we applied the Knowledge Gaining Directed Evolution (KnowVolution) strategy to investigate how amino acid substitutions influence cata

Red blood cell damage defines the etiology of hepatic sinusoidal obstruction syndrome induced by pyrrolizidine alkaloids.

Hepatic sinusoidal obstruction syndrome (HSOS), a life-threatening liver disease characterized by sinusoidal endothelial cell (LSEC) damage, is frequently caused by pyrrolizidine alkaloid (PA) exposure present in numerous herb or food products. Unlike other hepatotoxins, the precise mechanism by which PAs selectively target LSECs remains poorly understood, posing significant challenges to the development of effective treatments. This study identified hemolysis as the initiating event in PA-HSOS pathogenesis through clinical and animal model analyses. PA exposure induced red blood cell (RBC) ru

Diagnostic challenges and mimicking disorders of Wilson's Disease: A comprehensive review.

Wilson disease is classically defined as an autosomal recessive disorder of copper metabolism that leads to neurological and hepatic dysfunction. It is one of the most prevalent genetic liver disorders caused by pathogenic mutations in the ATP7B gene, leading to defective copper transport and toxic accumulation in tissues. Without timely diagnosis and appropriate treatment, Wilson disease can progress to life-threatening hepatic failure and severe neurological impairment, ultimately resulting in death. Clinically, Wilson disease exhibits marked phenotypic heterogeneity, presenting with a wide

Real-world safety assessment of MET-targeted therapies: a pharmacovigilance analysis using the FAERS database.

BACKGROUND: Globally, lung cancer is the foremost contributor to cancer-associated fatalities. Of its various histological sub-types, non-small-cell lung cancer (NSCLC) constitutes the vast majority, accounting for more than 85% of diagnoses. A key driver in this pathology is the mesenchymal-epithelial transition (MET) receptor tyrosine kinase, whose biological activity is governed by the hepatocyte growth factor (HGF) ligand. MET plays a pivotal role in regulating tumour cell proliferation and migration, and aberrant activation of MET signalling is linked to the pathogenesis of NSCLC. METHOD

[Role of glutathione in regulating ferroptosis and cuproptosis in metabolic dysfunction-associated steatotic liver disease].

Metabolic dysfunction-associated steatotic liver disease (MASLD) is a fatty liver disease characterized by metabolic dysfunction as its core feature. It represents one of the most common causes of liver disease worldwide and imposes a substantial disease burden. Abnormal cell death patterns are involved in the initiation and progression of MASLD, and ferroptosis and cuproptosis have recently emerged as research hotspots due to their roles in triggering and exacerbating MASLD. Glutathione (GSH), the most important reductant in redox regulation, can promote hepatic lipid accumulation and oxidati

Fisetin attenuates CCl₄-induced liver fibrosis through modulation of TGF-β1/Smad3 and p53/Bax/Bcl-2 signaling pathways.

Liver fibrosis arises from a sustained imbalance between oxidative stress, cell death, and matrix deposition. Using the carbon tetrachloride (CCl4) mouse model, we show that the dietary flavonoid, fisetin orchestrates a broad hepatoprotective program that blunts this cascade. Male C57BL/6 mice received CCl4 for six weeks, without/with daily fisetin at 50 or 100 mg/kg. Co-treatment with fisetin preserved body weight and lowered the liver index, and markedly attenuated biochemical injury, reducing serum ALT, AST, ALP, GGT, and TBIL while restoring total protein and albumin levels. Bile acids as therapeutic agents.

Bile acids (BAs) are amphiphilic molecules traditionally recognized for their role in lipid digestion but have gained increased interest for their therapeutic potential. Among them, ursodeoxycholic acid (UDCA) is the most widely prescribed and has been FDA-approved in the treatment of primary biliary cholangitis (PBC), the most common chronic cholestatic liver disease, while also being used off-label in multiple other disorders. The therapeutic effects of BAs are linked to their capacity to modulate signaling pathways, reduce hepatocellular injury, and regulate inflammation. Their physicochemi
