Randomized Phase II/III Trial Comparing Hepatectomy, Followed by mFOLFOX6 With Hepatectomy Alone for Liver Metastasis From Colorectal Cancer: Long-Term Results of JCOG0603.

JCOG0603 demonstrated improved disease-free survival (DFS) with adjuvant mFOLFOX6 after hepatectomy compared with hepatectomy alone in colorectal liver-only metastasis (CRLM), but the overall survival (OS) data were immature. Here, we report OS after long-term follow-up. Eligible patients with colorectal adenocarcinoma and an unlimited number of liver metastases were randomized to adjuvant mFOLFOX6 or hepatectomy alone. DFS was the primary endpoint, and OS was a secondary endpoint. Between March 2007 and January 2019, 151 and 149 patients were randomized to adjuvant chemotherapy and hepatectom

Local Control in a Retrospective Series of Magnetic Resonance Guided Stereotactic Body Radiation Therapy for Colorectal Liver Metastases: A Single-Institution Experience.

PURPOSE: Image guided stereotactic body radiation therapy (SBRT) serves as a key local treatment for liver metastases. As a next-generation technology, magnetic resonance guided SBRT (MRgRT) offers theoretical advantages over computed tomography guided SBRT for the management of liver metastases. However, clinical evidence regarding the efficacy of MRgRT for colorectal cancer liver metastases (CRLM) remains lacking. METHODS AND MATERIALS: This retrospective study included patients with CRLM who underwent MRgRT at a single institution between September 2021 and December 2023. Local control (L)

Lenvatinib plus transarterial chemoembolization and PD-1 inhibitors as conversion therapies for unresectable intermediate-advanced hepatocellular carcinoma: a phase 2 trial and exploratory biomolecular study.

Conversion therapy remains an uncommon strategy for managing unresectable hepatocellular carcinoma (uHCC) due to limited evidence supporting its efficacy. To address this gap, we initiated a prospective phase 2 multicenter trial (NCT04997850) comparing the LEN-TAP regimen, combining lenvatinib, transarterial chemoembolization (TACE), and PD-1 inhibitors, against TACE alone in uHCC patients. The study's primary outcome was salvage liver resection (SLR) rate; secondary measures included objective response rate (ORR), overall survival (OS), event-free survival (EFS), recurrence-free survival (RFS

High copper levels induce oxidative stress and inflammatory processes in a cell culture model of Wilson's disease.

Wilson’s disease (WD) is a genetic disorder resulting from mutations in the ATP7B gene that lead to copper overload in hepatocytes. To gain a deeper understanding of the cellular mechanisms underlying WD, a reanalysis of the GSE1073236 dataset was conducted and validated by comparative analyses in a cell culture model of HepG2 and HepG2 ATP7B knockout (ATP7B-KO) cells, as well as in primary hepatocytes from WD patients and controls. Increased expression levels of genes associated with autophagy, oxidative stress and inflammation were observed in copper-treated HepG2 ATP7B-KO cells and in prima

Nuclear receptors-mediated lipid metabolism dysregulation: A potential mechanism linking PFOA/PFOS to hepatocellular carcinoma.

Hepatocellular carcinoma (HCC), a prevalent liver malignancy, is closely associated with dysregulated lipid metabolism. Endocrine disrupting chemicals (EDCs) can bind to nuclear receptors (NRs) and potentially induce carcinogenesis, but their specific influence on HCC progression remains unclear. To investigate this relationship, we combined bioinformatic analyses with experimental validation in the present study. Differential expression analysis of public HCC datasets (GSE14323, GSE17548, and GSE25097) identified candidate genes, which were further refined via weighted gene co-expression netw

Sorafenib nanomedicine in HCC: nano-bio interactions and combination therapies.

Hepatocellular carcinoma (HCC) remains a leading cause of cancer-related mortality worldwide, with systemic therapies offering only limited benefit. Sorafenib, the first-line multi-kinase inhibitor, is constrained by poor pharmacokinetics, dose-limiting toxicities, and the rapid emergence of resistance. Nanotechnology-based drug delivery systems (NDDS) provide promising strategies to address these limitations. This review summarizes recent NDDS innovations that enhance sorafenib's therapeutic efficacy in HCC. Major nanocarrier classes, including lipid-based, polymeric, inorganic, and biogenic

Revisiting prophylactic corticosteroids to mitigate severe immune-related adverse events in hepatocellular carcinoma.

Dual immune checkpoint inhibitors (ICIs) targeting cytotoxic T-lymphocyte antigen-4 (CTLA-4) and programmed cell death-1 (PD-1) or its ligand (PD-L1) have reshaped the treatment landscape of unresectable hepatocellular carcinoma (HCC). However, their clinical uptake remains limited by the high incidence of severe immune-related adverse events (irAEs), underscoring the urgent need for effective prophylactic strategies. Corticosteroids are traditionally viewed as antagonistic to antitumour immunity and have long been prohibited from prophylactic use in ICI-only regimens. Yet this dogma is increa

Identification of functional genetic components modulating toxicity response to PFOS using genome-wide CRISPR screens in HepG2/C3A cells.

Perfluorooctane sulfonate (PFOS) poses significant health and environmental risks due to its persistence and widespread use and has been linked to various adverse outcomes, such as liver toxicity. Although the molecular responses and toxicity effects of PFOS exposure have been extensively studied, considerable uncertainty remains regarding the causal mechanisms leading to PFOS-associated adverse effects. To help bridge this gap, we conducted CRISPR screens in HepG2/C3A human liver cells exposed to IC25 (170 µM) of PFOS to identify genes and pathways influencing PFOS-induced cytotoxicity. Using

Discovery of Novel Selenocyanate Derivatives as Histone Deacetylase 6 Inhibitors for the Treatment of Hepatocellular Carcinoma.

Histone deacetylase 6 (HDAC6) is a pivotal epigenetic regulator and is involved in the tumorigenesis and progression. Our previous study revealed that HDAC6 is overactivated in hepatocellular carcinoma (HCC), and identified SelSA as a novel HDAC6 inhibitor (HDAC6i) for HCC therapy. In this work, we optimized this inhibitor and discovered 14a as a novel HDAC6i with better efficacy against HDAC6 and HepG2 cells than SelSA through structure-activity relationship (SAR) studies. The flow cytometry indicated that 14a markedly induced apoptosis and cell cycle arrest at the S phase in HepG2 cells. Mec

FOXM1 influences DNA methylation to augment TACC3 alternative splicing directed by KAT2A in hepatocellular carcinoma.

BACKGROUND/AIMS: Hepatocellular carcinoma (HCC) is characterized by profound transcriptomic dysregulation, yet the mechanism(s) by which DNA methylation is coordinated with chromatin modifications to regulate alternative splicing during tumorigenesis remains poorly understood. METHODS: Using prospectively paired multi-omics data obtained from metabolic dysfunction-associated steatotic liver disease (MASLD)-HCC patients and coupled with a premalignant MASLD cohort, we have uncovered a previously unrecognized gene-regulatory axis centered on TACC3 isoform-switching. RESULTS: In the non-tumoral

Anti-PD-L1 biomimetic nanobubbles for ultrasound-triggered reprogramming of cold hepatocellular carcinoma for immunotherapy.

Hepatocellular carcinoma exhibits profound resistance to immune checkpoint blockade due to its dense stromal architecture and immunosuppressive tumor microenvironment with an immune-excluded phenotype. To overcome this therapeutic challenge, we developed PD-1-displaying biomimetic nanobubbles (NB@CM-H-PD-1) that orchestrate a trimodal strategy combining PD-L1-targeted checkpoint blockade, sonodynamic therapy, and acoustic cavitation-mediated stromal remodeling to transform immunologically "cold" HCC into an immune-responsive state. These engineered nanobubbles comprise a perfluorocarbon core,

Decrypting potential mechanisms linking ochratoxin A to hepatocellular carcinoma: an integrated approach combining toxicology, machine learning, molecular docking, and molecular dynamics simulation.

BACKGROUND: Ochratoxin A (OTA), a common food-borne mycotoxin, is a potential human carcinogen, yet the specific molecular mechanisms linking it to hepatocellular carcinoma (HCC) remain unclear. METHODS: We integrated network toxicology to predict OTA targets and intersected them with HCC transcriptomic data to identify key candidate genes. Functional enrichment analysis was then conducted. Multiple machine learning algorithms were applied to screen and validate core genes. Furthermore, molecular docking and molecular dynamics (MD) simulations were employed to evaluate the binding stability b

1,6-Dihydroxynaphthalene as a potential environmental obesogen induces lipid accumulation and liver developmental defects in zebrafish.

1,6-Dihydroxynaphthalene (1,6-DHN), a key dye intermediate and metabolite of polycyclic aromatic hydrocarbons, may pose environmental endocrine-disrupting risks. However, its toxicological effects remain poorly understood. Using zebrafish as a model, this study systematically examined the impact of 1,6-DHN on liver development in larvae. Exposure to 1,6-DHN significantly inhibited liver growth, induced abnormal lipid and glycogen accumulation, and promoted excessive reactive oxygen species (ROS) production, leading to lipid peroxidation. Transcriptomic analysis revealed disrupted expression of
