[Conservative therapy of liver failure].

Hepatic failure is characterized by decreasing liver function involving synthesis, regulation and detoxification. In most cases, acute liver failure results from acute viral hepatitis. Furthermore, intoxications or metabolic decompensation can cause fulminant hepatic failure. Chronic liver failure is defined as progressive decline in liver function in previously existing liver disease, mostly liver cirrhosis. Clinical manifestation of both acute and chronic hepatic failure is determined by hepatic encephalopathy and severe disturbance of the hemostatic system. Causal therapy does not exist; th

Significance of serum human hepatocyte growth factor levels in patients with hepatic failure.

Serum human hepatocyte growth factor levels were measured using a newly developed enzyme-linked immunosorbent assay kit in patients with liver diseases. Serum human hepatocyte growth factor levels were increased in correlation with derangements of prothrombin time, total bilirubin and other parameters reflecting hepatocellular dysfunction in 112 patients with chronic liver disease. The levels were positively correlated with serum AST and ALT levels in 59 of these patients whose prothrombin times were within the normal range. Abnormally increased serum human hepatocyte growth factor levels were

The pathologic spectrum of the nephropathy associated with alpha 1-antitrypsin deficiency.

To further define the clinicopathologic spectrum and pathogenetic mechanism of the nephropathy associated with alpha 1-antitrypsin (A1AT) deficiency, we evaluated renal specimens from 34 patients with chronic hepatic disease, including 20 with A1AT deficiency (study patients), and correlated these findings with urinalysis evaluation. Glomerular lesions were noted in 79% (15 of 19) of A1AT patients with the PiZZ phenotype, including seven with mesangio-capillary glomerulonephritis (MPGN and focal segmental MPGN), six with mesangial proliferative glomerulonephritis (Mes GN), one with diffuse end

Neurochemical studies of hepatic encephalopathy.

Despite intensive research, the neurochemical basis of hepatic encephalopathy (HE) has not been defined. Theories that are currently favoured to explain the cerebral dysfunction that accompanies acute or chronic hepatic failure include: (1) ammonia acting as the putative neurotoxin; (2) perturbed monoamine neutrotransmission as a result of altered plasmo amino acid metabolism; (3) an imbalance between excitatory amino acid neurotransmission, mediated by glutamate, and inhibitory amino acid neurotransmission, mediated by gamma-aminobutyric acid (GABA); and (4) increased cerebral concentrations

[The dietary protein contribution and hepatic encephalopathy in cirrhosis].

Hepatic encephalopathy is a neuropsychiatric syndrome, which can occur in the clinical course of acute (fulminant) or chronic hepatic failure of various aetiology; reversible metabolic abnormalities without neuronal structural changes are frequently found in this condition. High blood ammonia levels, an imbalance between plasma concentrations of branched-chain and aromatic amino acids, false neurotransmitters and neurotransmitters receptor changes in CNS are the commonly recognized pathogenetic mechanism of this syndrome. Protein malnutrition is a frequent occurrence in liver cirrhosis, especi

Prevalence of antibodies to hepatitis C virus in Chinese patients with viral hepatitis and hepatic failure.

Anti-HCV assay with ORTHO kits was done in 100 blood donors and recipients and 374 cases of viral hepatitis, including 65 cases of fulminant, subacute and chronic hepatic failure. None of the 100 blood donors and recipients showed positive anti-HCV response. Anti-HCV was positive in 7.6% of the patients with chronic persistent hepatitis, 9.7% of the patients with chronic active hepatitis and 23.1% of the patients with liver cirrhosis. High prevalence of anti-HCV was observed in subacute hepatic failure (60.8%) and chronic hepatic failure (53.9%). Fifty-two (83.9%) of 62 anti-HCV positive cases

Hepatic encephalopathy. Current concepts of the pathogenesis.

Plasma antigen levels of the lipoprotein-associated coagulation inhibitor in patient samples.

Human plasma contains an inhibitor of tissue factor-initiated coagulation known as the lipoprotein-associated coagulation inhibitor (LACI) or also known as the extrinsic pathway inhibitor (EPI). A competitive fluorescent immunoassay was developed to measure the plasma concentration of LACI in samples from normal individuals and patients with a variety of diseases. The LACI concentration in an adult control population varied from 60% to 160% of the mean with a mean value corresponding to 89 ng/mL or 2.25 nmol/L. Plasma LACI levels were not decreased in patients with severe chronic hepatic failu

Chronic acquired hepatic failure: MR imaging of the brain at 1.5 T.

The results of MR imaging of the brain at 1.5 T in 42 adults with non-Wilsonian chronic hepatic failure are reported. T1-weighted images demonstrated increased signal in the globus pallidus in 30 patients and in the putamen in 21, while T2-weighted images demonstrated no corresponding alteration in signal intensity. Symmetric low intensity in the central portion of the globus pallidus on spin-density and T2-weighted images in two patients correlated with regions of calcification on CT scans. Increased intensity on T1-weighted images also occurred in the mesencephalon surrounding the red nucleu

Cryptogenic cirrhosis: clinico-biochemical comparison with alcoholic and viral etiologies.

Cryptogenic cirrhosis defines a group of undetermined etiology that would either be caused by factors as alcohol, virus and others or be due to still unknown etiological factors. To test these hypotheses we have looked for similarities or differences in clinico-biochemical presentation of 196 cases of alcoholic, viral and cryptogenic cirrhosis. Age, jaundice, spiders, palmar erythema and factor V showed a statistically significant difference of the cryptogenic cirrhosis when compared with both alcoholic and viral etiologies. On clinico-biochemical grounds it could be suggested that cryptogenic

Blood coagulation variations induced by carbon tetrachloride inhalation in Wistar rats.

Research into the development of chronic hepatic failure induced by CCl4 inhalation and the effect of this damage on blood coagulation was performed on Wistar rats. Exposure to CCl4 (325 ppm) consisted of the administration of 22, 26, 29, 31, 34, 39, 40, 45, and 49 doses, respectively. To determine and evaluate the nature and degree of CCl4-induced lesions at different stages of the treatment, histological alterations in liver samples were evaluated. Fatty infiltration in the centrilobular parenchymal cells observed after administration of the first doses was seen to spread toward mid- and per

[Initial clinical experience with the use of an auxiliary liver device].

The authors developed an artificial liver apparatus that comprised 2 running classifiers of blood or one running classifier and mass exchanger. The functionally active link of the apparatus is a suspension of living isolated xenohepatocytes obtained from the liver of pigs weighing 18-20 kg by a combined enzymatic and mechanical procedure. More than 100 in vivo laboratory experiments and 30 operations were performed in 20 patients with acute and chronic hepatic failure. Three patients died, the remaining showed significantly better biochemical parameters of blood and full recovery or stable rem

The effect of ciprofloxacin on antipyrine metabolism.

The effect of multiple-dose ciprofloxacin on antipyrine metabolism was studied in patients suffering from bacterial infections. The patients were given antipyrine 15 mg/kg intravenously before and after ciprofloxacin treatment. The dosage of ciprofloxacin was 500 mg bd by mouth for 8-10 days. Blood samples were taken at 0, 2, 4, 6, 10 h. Antipyrine total clearance was significantly decreased after ciprofloxacin treatment (0.85 +/- 0.45 vs. 0.52 +/- 0.24 ml/min/kg): elimination rate constants for antipyrine were decreased in all patients after ciprofloxacin, whereas no change in volume of distr

Temporary liver support using a new type of sorbent. A urethane sheet embedded with powdered charcoal.

A new sorbent, urethane sheet embedded with powdered charcoal (UPC, BESPORE), which had been developed for use in a temporary liver support system, has been clinically applied since 1984. This system consists of a UPC plasma-perfusion circuit (using a recirculation system) followed by a supplementary 0.8 L plasma exchange, using hemodialysis for the purpose of performing both removal and supplementary therapy at the same time. On average, approximately 40% of both bilirubin and bile acid are removed, with the osmolality gap reduced by 30%. This system is effective for prolongation of life in p

Effects of octopamine on renal function in anaesthetized dogs.

Increased levels of octopamine in adrenergic nerve terminals and plasma have been implicated in the circulatory and renal disturbances of chronic hepatic failure. Little is known about its renal actions in normal animals. In the present study, DL-octopamine was administered both i.v. and into one renal artery of anaesthetized dogs in doses ranging between 25-200 micrograms/min (1.6-20 micrograms/kg/min). Octopamine was hypertensive in doses of 100 micrograms/min and more and this change was associated with a significant decrement in GFR and renal perfusion. This amine also exerted a direct tub

Nutrition support of the adult liver transplant candidate.

Liver transplantation now provides a viable alternative to patients with end-stage hepatic failure. Because of the damaging effects of liver failure on other organ systems, transplant candidates frequently suffer from moderate to severe malnutrition. Chronic hepatic failure can result in deranged metabolism of not only macronutrients and micronutrients but also various hormones. Patients are frequently in a catabolic state. Wasting of the skeletal muscle mass occurs and synthesis of secretory proteins and clotting factors decreases. Hepatic encephalopathy and fluid/electrolyte imbalances often

[A comparative study of free amino acid levels in the serum and cerebral cortex in hepatic failure rats].

In order to elucidate the role of amino acid changes in hepatic encephalopathy, free amino acids in serum and cerebral cortex were measured in the experimental hepatic failure models. The acute and chronic models were produced in adult male Wistar rats by carbontetrachloride (CCl4) administration. Acute model was produced by a single intraperitoneal injection of CCl4 (0.35 ml/100 g, B.W.) after the daily pretreatment of phenobarbital (6 mg/100 g, B.W.) for 4 days, while chronic model was established by successive injections CCl4 (0.20 ml/100 g, B.W., twice/week) for 13 weeks. [Glucose metabolism in the rat cerebral cortex in chronic hepatic failure with reference to glucose-derived amino acids].

In order to investigate metabolic derangements in hepatic failure brains, glucose utilization was studied in regard to glucose-derived amino acids. For this purpose, chronic hepatic failure models were produced in adult male Wistar rats by successive carbontetrachloride injection (0.20 ml/100 g, B. W., twice/week) for 13 weeks. They were confirmed to develop chemical changes compatible with hepatic failure, showing markedly elevated serum levels of NH3, GOT and ALP. Animals were killed by decapitation during fasting and the brains were removed immediately. After the parietal cortical slices we
