Liver transplantation (LT) is entering a new era shaped by changes in disease epidemiology, candidate complexity, donor utilization, and post-transplant management. The decline of viral hepatitis-related cirrhosis, together with the rise of metabolic dysfunction-associated steatotic liver disease, alcohol-related liver disease, and hepatocellular carcinoma, has transformed the LT waitlist into an older, frailer, and more cardiometabolically complex population. At the same time, donation after circulatory death and machine perfusion are expanding the donor pool and redefining the boundaries of

Gene therapy offers a fundamental approach to treating genetic disorders, malignant tumors, and refractory chronic diseases by directly modifying or replacing defective genes at the molecular level. Driven by policy support, increased investment, and technological innovation, China has achieved remarkable progress in adenoviral and adeno-associated virus (AAV) vectors, gene editing, genetically engineered immune cells, oncolytic microorganisms, and small nucleic acid drugs. This review provides a comprehensive narrative overview of the technical principles, product pipelines, and clinical adva

Data on potential differences in hepatocellular carcinoma (HCC) incidence between entecavir (ETV) and tenofovir alafenamide (TAF) in chronic hepatitis B (CHB) patients are limited. This retrospective multinational study included treatment-naïve adults with CHB without a history of HCC who initiated ETV or TAF therapy since 2016 at 23 centres in 4 countries. Baseline characteristics were balanced using inverse probability of treatment weighting (IPTW) and propensity score matching (PSM). HCC incidence and the association between antiviral regimen and HCC were analysed using competing-risk analy

BACKGROUND: Significant liver fibrosis can develop in children with chronic hepatitis B (CHB), yet liver biopsy is invasive and unsuitable for repeated assessment. We developed an interpretable, non-invasive prediction model for significant fibrosis in pediatric CHB and evaluated whether baseline risk stratification was associated with subsequent treatment outcomes.
METHODS: This retrospective study included 232 children with CHB who underwent liver biopsy, with significant fibrosis defined as Scheuer stage S ≥ 2. Patients were randomly assigned to a development cohort (80%) and a held-out te

In various chronic liver diseases, hepatic fibrosis represents a frequent pathological event. This process involves the activation of hepatic stellate cells, along with the consequent excessive deposition of extracellular matrix. Emerging evidence highlights the contribution of gut microbiota dysbiosis and intestinal barrier dysfunction to its progression, underscoring the potential of targeting the liver-gut axis. Xihuangcao, a traditional Chinese medicinal herb used for acute cholecystitis and jaundice hepatitis, exhibits antifibrotic activity, but its underlying mechanisms remain unclear. I

BACKGROUND: Steatotic liver disease (SLD) is highly prevalent in people with HIV (PWH), but factors influencing progression to hepatocellular carcinoma (HCC) and hepatic decompensation (HD) remain unclear. It is unknown if the risk of these outcomes associated with SLD differs by HIV status.
METHODS: Patients from the Veterans Aging Cohort Study-HIV (includes matched people without HIV [PWoH]) who underwent an ultrasound, computed tomography, or magnetic resonance imaging between 2001-2017 were included. Patients with diagnosed cirrhosis or chronic viral hepatitis were excluded. SLD was defin

Metabolic dysfunction-associated steatotic liver disease (MASLD) has overtaken viral hepatitis as the fastest-growing cause of hepatocellular carcinoma (HCC) in most high-income countries. What makes this epidemic clinically treacherous is a feature that distinguishes MASLD-related HCC from other aetiologies: roughly half of all cases arise in livers that have never progressed to cirrhosis, slipping through the net of conventional surveillance strategies. The molecular journey from fatty liver to malignancy unfolds through at least six interlocking programmes: lipotoxic rewiring of hepatocyte

BACKGROUND AND AIM: Steatotic liver disease (SLD) may lead to advanced liver diseases. We aimed to identify clinical risk factors for advanced fibrosis in pathology-proven SLD patients in a hepatitis B virus (HBV) endemic population, with emphasis on nonelder subjects.
METHODS: Adult SLD patients diagnosed via pathology at National Taiwan University Hospital between June 2009 and December 2023 were recruited. Advanced fibrosis was defined as F2 to F4 fibrosis. Patients were stratified by age (< 50 years defined as nonelder), metabolic dysfunction-associated steatotic liver disease (MASLD), an

BACKGROUND: Hepatocellular carcinoma (HCC) is a leading cause of cancer-related mortality worldwide. Hepatitis D is the most severe form of viral hepatitis. However, the clinical, virological, and systemic inflammatory characteristics of HDV-associated HCC remain insufficiently characterized.
METHODS: In this single-center retrospective study, we identified 162 patients with HCC due to chronic hepatitis D (HDV-HCC; n=54) or chronic hepatitis B (HBV-HCC; n=108). Plasma levels of 92 soluble inflammatory mediators (SIMs) were quantified using proximity extension assay in HCC patients [HBV-HCC (n

Chronic hepatitis B virus (HBV) infection is a leading cause of liver cirrhosis and hepatocellular carcinoma (HCC), imposing a substantial global health burden. The natural history of persistent HBV infection progresses from chronic inflammation to fibrosis, cirrhosis, and malignant transformation, driven in part by the stability of covalently closed circular DNA (cccDNA) within hepatocyte nuclei. Nucleos(t)ide analogues (NUCs) effectively suppress viral replication but rarely lead to a functional cure, necessitating long-term therapy and carrying a residual risk of resistance. PEG-interferon-

The heart and liver are closely interconnected through complex hemodynamic, neurohumoral, inflammatory, and metabolic mechanisms. Dysfunction of either organ may adversely affect the other, giving rise to a broad spectrum of cardiohepatic disorders with important diagnostic and prognostic implications. This narrative review is based on a literature search strategy of PubMed and the Web of Science Core Collection covering publications from January 2000 through June 2026, with a focus on hypoxic hepatitis, congestive hepatopathy, and cirrhotic cardiomyopathy. Cardiac diseases may induce liver in

Background/Objectives: Asian Americans (AAs) bear a disproportionate burden of chronic hepatitis B (CHB), with one in 12 living with this condition. Despite evidence that cardiometabolic conditions accelerate CHB progression and hepatocellular carcinoma risk, cardiometabolic comorbidities in this population remain critically understudied. This study provides an assessment of cardiometabolic burden and phenotypic heterogeneity among AAs with CHB. Methods: We analyzed data from the All of Us Research Program Controlled Tier Dataset v8 (n = 436 AAs with CHB). We ran descriptive analyses to examin

Hepatocellular carcinoma (HCC) is the most common liver cancer and usually develops on a background of cirrhotic liver. Early and accurate diagnosis enables potentially curative interventions. HCC surveillance, usually offered to patients at risk for HCC, such as those with cirrhosis, chronic hepatitis B infection, and previous diagnosis of HCC, is associated with earlier detection and improved survival. In these patients, the diagnosis can be established non-invasively without histological confirmation when the imaging-based LI-RADS criteria are met. Tissue biopsy is mandatory for establishin

RATIONALE: Minimal change disease (MCD) is characterized by diffuse podocyte foot process effacement on electron microscopy, minimal structural changes on light microscopy, and the absence of immune complex deposition on immunofluorescence. In adults, MCD accounts for approximately 10% to 25% of nephrotic syndrome cases and is typically idiopathic, although secondary causes such as infections, medications, malignancies, and immune disorders have been reported. The association between hepatitis B virus (HBV) infection and glomerular disease has been well established in membranous nephropathy an

Ductular reaction (DR), a defining histological hallmark of diverse acute and chronic liver injuries, is fundamentally driven by the activation of hepatic progenitor cells (HPCs) and their complex, spatiotemporal bidirectional crosstalk within the hepatic niche. As bipotent precursor cells, HPCs initiate compensatory regeneration when mature hepatocyte proliferation is compromised. However, DR is not a uniform process but a multiphase dynamic reaction: transient early activation facilitates parenchymal restoration, whereas persistent, dysregulated HPC expansion fuels progressive fibrogenesis.

BACKGROUND: Focal liver lesions (FLLs) encompass a wide spectrum of benign and malignant pathologies, and accurate diagnosis is essential for appropriate management. Although advances in imaging have improved lesion characterization, histopathologic assessment remains the diagnostic gold standard for indeterminate lesions. Data on the histopathologic spectrum and diagnostic utility of ultrasound-guided percutaneous liver biopsy (US-PLB) in sub-Saharan Africa (SSA) are limited. This study aimed to characterize the histopathologic findings of US-PLB performed for FLLs at a tertiary referral cent

BACKGROUND: Gallbladder involvement during acute hepatitis A virus (HAV) infection is generally considered an uncommon extrahepatic manifestation, although recent reports suggest it may be underrecognized. We evaluated the prevalence, clinical characteristics and outcomes of gallbladder abnormalities in a multicenter cohort of children hospitalized with acute HAV infection during an Italian outbreak.
METHODS: We conducted a multicenter observational study, including consecutive children with laboratory-confirmed acute HAV infection admitted to 4 tertiary pediatric hospitals between January an
