Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

Chronic Hepatitis E infection in a pediatric liver transplant recipient with EBV-associated post-transplant lymphoproliferative disorder.

Chronic Hepatitis E infection is increasingly recognized in immunocompromised individuals and is primarily associated with genotype (Gt) 3, and rarely with Gt 1. Here, we report a unique case of chronic Gt1 HEV infection in a pediatric living donor liver transplant (LDLTx) recipient from India with concurrent Epstein-Barr virus (EBV)-associated post-transplant lymphoproliferative disorder (PTLD).

Ultrasound Normalized Weighted Cumulative Residual Entropy Imaging: A New Quantitative Ultrasound Method for Characterizing Hepatic Steatosis.

Background/Objectives: In this paper, we propose ultrasound normalized weighted cumulative residual entropy (NWCRE) imaging, a novel quantitative ultrasound (QUS) technique derived from the information-theoretic weighted cumulative residual entropy (WCRE) for hepatic steatosis characterization. Methods: The proposed ultrasound NWCRE imaging method adopted cumulative distribution function (CDF)-driven non-parametric estimation, and parallel computing was implemented to improve computational efficiency. For a comparison study, the previously reported ultrasound normalized cumulative residual ent

Value-based price corridor for bepirovirsen in chronic hepatitis B: an early health technology assessment in a cost-compressed setting.

BACKGROUND: We conducted an early health technology assessment (eHTA) of bepirovirsen, an investigational finite-course therapy for chronic hepatitis B. In China, low-cost generic nucleos(t)ide analogues (NAs) create a demanding comparator environment. The value-based price of a potential functional-cure strategy therefore depends on local comparator prices, threshold selection, cure durability, safety, monitoring requirements, and cured-state utility assumptions. METHODS: A lifetime Markov model simulated bepirovirsen 300 mg/week for 24 weeks plus background NAs against comparator-specific s

Impact of Cardiometabolic Risk Factors Control on Cardiovascular-Kidney-Metabolic Axis in Chronic Hepatitis B.

BACKGROUND: The impact of cardiometabolic risk factors (CMRFs) on the cardiovascular-kidney-metabolic (CKM) axis in chronic hepatitis B (CHB) remains poorly defined. METHODS: We conducted a prospective analysis enrolling patients with CHB who were on nucleos(t)ide analogues (NUCs). CMRFs were defined as per established criteria; poorly-controlled CMRFs were defined with specific cut-offs measured across 5 years. Study outcomes include fibrosis progression (liver stiffness [LS] increase by ≥ 20%), liver-related events (LREs), chronic kidney disease (CKD) progression, major adverse cardiovascul

Psoriasis: Which therapy for which patient, updated, part II. Focus on special populations and chronic infections.

Choosing systemic therapy for psoriasis remains challenging in populations underrepresented in clinical trials, including pregnant and pediatric patients, patients with chronic infections, and those with difficult-to-treat phenotypes. Since the 2019 version of this article, therapeutic options have expanded substantially, including new pediatric approvals, oral targeted agents, and biologics with increasingly phenotype-specific evidence. This second article in the continuing medical education series reviews systemic treatment selection for moderate-to-severe psoriasis in pregnancy, pediatric p

Molecular characterization and genetic diversity of hepatitis B virus among health care workers in the Bamenda Health District, Northwest Region, Cameroon.

OBJECTIVES: Hepatitis B virus (HBV) infection is endemic in Africa, with an estimated 82 million people living with chronic HBV infection. The outcome of exposure to HBV is a complex interplay between host immunity and viral factors, including HBV genotypes. Despite the high burden of HBV infection, data on HBV genotypes circulating in many African settings, including Cameroon, remain limited. This study investigated the genetic diversity of HBV among health care workers in the Bamenda Health District, Cameroon. METHODS: In this cross-sectional hospital-based study, HBV DNA was extracted from

Therapeutic advances in chronic hepatitis B and Delta: where are we now and where are we headed?

Chronic hepatitis B (HBV) and delta (HDV) infections remain major global health challenges. Nucleos(t)ide analogues (NUCs) and bulevirtide, a recently approved entry inhibitor, effectively suppress HBV and HDV replication, respectively, thereby reducing the risk of liver-related complications. However, they rarely induce functional cure, defined as sustained HBsAg loss, and generally require long-term administration. This limitation is primarily driven by the persistence of covalently closed circular DNA (cccDNA) and integrated HBV DNA, which are not targeted by existing treatments. A broad ra

Discovery of Thiazolamide-based PI4KIIIβ/HDAC Dual Inhibitors with Potent Activity against Hepatitis C Virus.

Chronic hepatitis C virus (HCV) infection remains a critical global public health burden affecting hundreds of millions of people worldwide. The emergence of antiviral resistance persistently undermines the long-term clinical efficacy of current direct-acting antiviral (DAA) regimens, creating an urgent need for therapeutics with different mechanisms of action. Host-directed multi-target therapy offers a promising avenue to overcome DAA resistance, yet dual inhibitors targeting HCV-essential host kinase phosphatidylinositol 4-kinase IIIβ (PI4KIIIβ) and epigenetic regulator histone deacetylases

SARS-CoV-2 spike protein enhances HBV replication via autophagy and aggravates inflammation in CHB patients with liver dysfunction.

This study investigated the effects of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) spike protein (S protein) on hepatitis B virus (HBV) replication and immune function in patients with chronic hepatitis B (CHB), aiming to elucidate the underlying mechanisms of S protein-HBV interaction. Through in vitro cell experiments and clinical sample analyses, S protein expression was induced in HepG2.2.15 cells, leading to enhanced HBV replication; this effect was reduced by autophagy inhibitors, confirming the critical mediating role of autophagy. Furthermore, peripheral blood mono

Beware the Muscle: Silent Exercise-Induced Aminotransferase Elevation as a Diagnostic Pitfall in Hepatology and Pretreatment Screening-A Report of Two Cases.

BACKGROUND: In clinical hepatology practice, pretreatment liver enzyme screening plays a key role in making management decisions, such as initiation of antituberculosis therapy or surveillance of chronic hepatitis B. Unrecognized extrahepatic sources of aminotransferase elevations can lead to misdiagnosis, inappropriate treatment alteration, and unnecessary invasive or costly investigations. CASE PRESENTATION: We present two cases of active young males who were undergoing liver biochemical testing for therapeutic decision-making for other underlying illnesses. Patient one was undergoing basel
