We herein describe an autopsy case of acute-on-chronic liver failure (ACLF) in a patient with primary biliary cholangitis (PBC) that was triggered by an exacerbation of autoimmune hepatitis (AIH)-related features. A 62-year-old woman was diagnosed as having PBC 12 years prior and had been maintained on ursodeoxycholic acid. She presented with fatigue, and was found to have acute exacerbation of liver injury. BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) is one of the most common chronic liver diseases in children and adolescents. NAFLD ranges in severity from isolated hepatic steatosis to nonalcoholic steatohepatitis (NASH), wherein hepatocellular inflammation and/or fibrosis coexist with steatosis. Circulating microRNA (miRNA) levels have been suggested to be altered in NAFLD, but the extent to which miRNA are related to NAFLD features remains unknown. APPROACH AND RESULTS: We collected cross-sectional clinical data for a derivation cohort (n = 86) and a validation cohort (n = 431), totaling 517 subjects with liver biopsy. Advanced liver fibrosis was defined by the METAVIR pathological score (F ≥3). Dual cutoff values for diagnosis were explored.
